will inman aligner work for overcrowded teeth
Q. ive never had a brace and my teeth are overcrowded, is an inman aligner an option for me? I am 34years old and would like to avoid getting a brace if there’s another option.
A. Inman Aligners can treat crowding in specific situations. Usually no more than 3 mm of crowding.

my dental implants fell out with my teeth
Q. I had dental implants fitted in december 07 to upper right 4 & 5 & upper left 4 & 5. I previously (3 years ago) had implants to upper left 1 2. My query is - how long should these teeth stay in place under normal circumstances? I have just returned from holiday and while I was away, the teeth from upper right 4 & 5 fell out. They hadn’t been loose in any way - they simply dropped out. Unfortunately I was on a cruise and the teeth fell into the ocean.. (yes, really!!) I had emergency dental work to fill the gaps until my return to the UK, but should I in expect my dentist to replace the missing teeth free of charge (they were only fitted 8 months ago)or is this just bad luck on my part?
A. Implants are normally very predictable. It might be that the implant is still secure within the jaw but the Bridge/ Crown has decemented from the implant abutment, This is not uncommon as sometimes a temporary cement is used for the intial cementation to allow maintainance in the early phase. It is down to the individual dentist to decide what to cover if the bridge is not avaialable for recementation.
Check your travel insurance you might be covered. If not your dentist has 3 options

What do lingual braces and invisible braces cost
Q. Hello, my 17 year old daughter has slightly crooked teeth at the bottom and was not keen on getting fixed ‘train tracks’ as she will be heading off to University soon. I was wanting to enquire about Invisible braces- and the cost for her age. She would need to have a top and bottom set. Also, a friend mentioned Lingual braces which I think would be a good idea- are they costly and are they effective? Thanks
A. Invisible braces will cost the same for a given problem - not age dependant. In my practice we charge £3400. Lingual braces are often an option but are usually more expensive due to the difficult nature of their position ( tongue side rather then lip side )

A. It depends on wehter you are interested in braces or veneers. From your question I presume it is braces. In my practice we are able to use an Inman Aligner for certain cases ( £1600) and for more complex cases we use Invisalign ( £3400). The Inman can take upto 3 to 4 months to acieve a great result. Invisalign can take between 6 months and 18 months depending on how severe the problem.

Can i find an NHS dentist for acid errosion
Q. Hi. I have a problem with the condition of my teeth and am not sure what type of treatment would be right for me. I have begun to get acid erosion on my teeth and have had to have a filling to fill 2 chips that appeared on my two front teeth. Apparently the erosion is caused by eating too much citrus fruit as i rarely drink sugary drinks etc. My two front teeth appear to be disintegrating and have gone from being lovely and straight to curved and a bit jagged. They also are a bit discoloured. This is really affecting my confidence now and i spend most of my time worrying and thinking about the state of my teeth and how they could end up. i was thinking about veneers but not sure i can afford it or if i want my original teeth to be filed down to toothpicks! I am an NHS patient - will this help with the cost of sorting my teeth out? Thanks for your advice.
A. It does sound as though veneers would be the best option for you but you may find it difficult ot find an NHS dentist who is in the position to carry out this treatment of you.

A. It is difficult to advise you without seeing you. If treatment is possible I would plan your teeth in a laboratory first and then place you into prototype teeth to see how the final smile would look this lets you preview your new smile. This is done without touching your own teeth so is completely reversible.
Often you need to have all your teeth treated to correct a jaw problem and orthodontics may be needed.

My first molar is filled and broken
Q. My first molar on the bottom right hand side of my mouth is heeavily filled and has broken. My dentist has examined it and done a temporary fix but has given me 2 options, to have a crown fitted, but has said because there is so little of the tooth itself left root is intact the crown would be quite brittle as their is very little tooth to attach the crown to, so it may fail and i may have to have the tooth out anyway. The second is to have the tooth out and be left with a gap, which i dont feel will improve my smile. Are you able to say how likely the crown is to fail, and are there any other options.
A. Many times we as dentists do try and be heroic when saving a very broken down tooth. If the root is sound but their is very little tooth above the gum and option would be to have crown lengthening involving removeing some bone around the neck of the ooth to expose mor tooth. This will allow the crown to be attached to your tooth (ferrule effect) as well as a post into your root ( you would need root canal treatment if not already carried out). This would increase the chances of success. Certainly the other option would be to have your tooth removed and an implant placed .

About 6 years ago i made a rash decision and as i was not happy with my teeth i had my 4 front top teeth crowned. i didn’t really research it and it was not thoroughly explained but i went ahead. i had a slight overbite, which they said i would still have, but didn’t really go in depth. I now have a large overbite which i have not really been happy with and does cause me problems with eating and speaking at times. i periodically get quite distressed by it and would really like to know what i could do about further treatment and where i could go. any advice would be great!
A. It is always difficult to give advice without seeing you but it sounds like the fundamantal problem is the position of your teeth. The best way to resolve this would be by orthodontics. Procedures such as Invisalign ( a series of see through mouthguards which straighten your teeth) other options are fixed metal braces (Train tracks or an Inman aligner. These options would put your teeth into a better position which may be all you need to do. After this you could have new crowns or veneers to create a new smile.

Q. I am only 24 years old and around 18 months ago I was diagnosed with advanced gum disease, and unfortuantly alot of the damage has been done, especially in the last 6 month. My two front teeth have drifted causing quite a large gap and they are slightly mobile. Lukily my dentist has reffered me to the Dental Hospital in Newcastle and I am currently under a peridontal specialist. However I wanted to know if any cosmetic procedures could be carried out once the gum disease is stable? I used to have a beautiful smile, and I dread to think nothing could be done and that i will neve have straight teeth again, will my specialist be able t do any cosmetic work through the NHS or will i have to pay privatly if any work can be done??
A. The NHS will not have funds for cosmetic treatments. If you need treatment for a functional reason and you get a cosmetic benefit, thats OK. Teaching Hospitals like Newcastle may do some very limited work as a training exercise.
What can be done is complex and very dependent on a clinical assesment. Often changing the shape of the teeth with veneers or crowns is used to hide the recession. there are also tecniques for reshaping gums.

A. it is difficult to advise you without seing you. sometimes to get an ideal orthodontic result surgery is the only option. However sometimes the teeth can be moved into a better position without surgery providing you realise the limitations of the treatments.

Q. My daughter had a root canal done last year. The crown has come out several times and my dentist insists this happens because she is eating candy and chewing gum. The last time it came out was last week and she was chewing gum… but I also have a crown from a root canal and I chew gum and eat candy and this has never happend to me.
A. Post retained crowns - I presume this is the type of crown your daughter has- are oftern the last ditched effort to saving a tooth. If there is very little tooth left the crown may only be able to attach to the post and can be very prone to coming out. Your crown may have more tooth to attach itself too.

Q. Hi I had a teeth whitening treatment done a couple of years ago with my dentist. I already have the whitening trays which were made for me at the time. However, I now need to buy more gel as the gel I had I pressume must have gone out of date even though I still have them stored in the fridge. Please could you recommend a whitening gel for me to buy online? I notice that there are varied percentages. The naturally white website sells 23% and 35%. Others seem to be lower. I don’t really want to sleep with it on so do you think I should get the 35% one and wear for 30 mins as recommended? Thanks
A. The strength that you need & the active ingredient should be balanced according to your own clinical situation. Your dentist will probably sell you small top up volumes of gel of the appropriate strength at reasonable prices.

A. The tecnique of gas and air is called ” Relative Analgesia” or R.A. There are some strict compliance regulations regarding its use which has put all but high volume users off. It is mainly used on anxious children and as such the NHS Community Dental Service would be a good place to start looking. There may not be treating adults but would be aware of anyone locally with the equipment who did.
An enquiry to either the local NHs Primary Care Trust / consultant in public dental health / dental practice adviser will start you off. Google your local PCT or use a Phone directory.

A. It depends on the size of the gap between the teeth. Bonding may be possible to close the gap completely or reduce it. Orthodontics would be the best treatment i have treated many similar smiles and it can take as little 6 months by orthodontics. If you rather have cosmetic treatment I recommend you find a good cosmetic dentist that you feel comfortable with

I am currently having my teeth straightened using
Q. Dear Dr. Solanki, I am currently having my teeth straightened using Invisalign and will be doing so for the next 8 months. My teeth have become crooked over the years (I am in my late 20’s) as a result of a baby tooth that has been stubborn and remained in my mouth. As such, I am straightening the teeth and then want to remove the cause of the crookedness (i.e. the baby tooth). I’m therefore thinking of removing the guilty tooth and inserting an implant into the jaw. The tooth in question is my upper Canine (I think it corresponds to number 6 on the dental charts). I have read quite a bit of literature but am hoping you can answer some of my questions if possible. 1.) I’ve been told that I will need to wear the final aligner in the Invisalign set for a good 3-6 months to ensure the teeth have time to settle in their final position. Assuming the baby tooth survives the forces from the Invisalign treatment, can I use these 3-6 months to remove the baby tooth and immediately fit the implant into the created hole (I read the treatment would need 3-6 months for the titanium implant to bond with my bone)? If this is so, can I then have the canine position on the inside of the aligner coated white, to look like a ‘fake’ tooth? My reason for this query is that the canine position will be very visible and I was hoping I could use the aligner to help mask the gap in my teeth! 2.) Having looked at implants, I would like an experts opinion on the use of modern all-ceramic crowns for implants. I like the look of the CEREC based crowns using Mark II blocks. However, I don’t know how much of my interest is just down to Vita’s good marketing and whether there are better alternatives on the market! If there is, I would very much appreciate your advice on the subject! What are your opinions of PFMs? 3.) Do you happen to know how much the process would cost with an all-ceramic crown in the UK? 4.) Could you recommend a decent dentist in the Cambridgeshire area that do ceramic crowns + implants? 5.) How common is it for UK-based dentists to stain the CEREC restorations and is this part of the normal procedure or does it cost extra? Thanks so much for the advice! Andy
A. hi andy many thanks for your enquiry. firstly after the invisalign treatment is done, it is essential to wear a retainer for the long term to help your teeth stabalise in to position. during this time it is feasible to place the dental implant into the canine area as long as this can be done. the dentist can then also place a false tooth in the invisalign aligner so it looks like you have a tooth.
