I have been looking into cosmetic dentistry

Sunday, September 21st, 2008
Q.
Hi Dr Rob, I have been looking into cosmetic dentistry for a little while now. I have suffered from poor oral hygiene and crooked teeth for a long time, it has got to the stage where I do not smile. I have a slight over bite, have 2 teeth that are hidden underneath my bottom set of teeth and 2 short dracula teeth. I appreciate that you cannot give me your complete view without seeing me, but any advice at this early stage would be a great help. After years of misery I really need to sort my problem out as it leading to me feeling depressed. From the information I have gathered, I understand that there are several methods of dentistry available to me i.e. tooth veneers, implants and orthodontics. I would like to pursue the road to orthodontics possibly Invisalign as I understand that this is a long term solution and I am not after a quick fix. I would like to know if your surgery could help me, or if you could possibly recommend a colleague who specialises in this area. I live in the Birmingham area and would also like to know the cost for this procedure in the Birmingham area. Keep up the good work ! Many thanks

I have just paid £20000 for a new set of teeth

Monday, September 15th, 2008
Q.
I have just paid £20,000.00 for a new set of upper and lower teeth. They have been fixed to 8 implants, 4 at top and 4 at bottom. (I have since discovered that 5-6 implants are recommended for each bridge)The job was finished only 7 days ago and already one of the teeth has broken been glued back in and has now broken again in the same place. This happened aprroximately 5 times while i had my tempary teeth. My Dentist says it’s is because i am only 43 years old and a male so my bite is stronger than a normal person having this type of work done. But each time this has happend i have been chewing soft food and not biting anything remotley hard.I feel the damage is being done when both sets of teeth come together when chewing.Would you advise me to pay for a second opinion because i am slowly loosing all confidence in my present dentist and do not think i should accept the broken tooth being glued back in everytime it breaks. I am now begining to wonder whether both sets of new teeth are lined up properly or maybe something else could be wrong with the work i’ve had carried out. Hope you can help.

I really want to sort my teeth out once and for all

Monday, September 15th, 2008
Q.
I really want to sort my teeth out once and for all!. I’m not sure where to go first. Do I go to an ‘ordinary’ dentist and then get recommended for cosmetic work? Or if I contact a ‘cosmetic’ dentist will all the problems be sorted out by that one person. My teeth are crooked but I also have gum disease which is getting much worse, particularly around one of my front teeth (in the upper jaw). I have not been to a dentist for about 5 years, also I am 43. (Definitely long in the tooth!) If someone has got severe gum disease, even if it is sorted out, what are the chances the original teeth can be corrected? And how is gum diease sorted out, how long does it take? I don’t know if this is true or not, but I’ve heard the bacteria can be eating away at teeth under the gum and into the jaw. So I’m just wondering if this damage will mean any correction work is doomed. (My teeth don’t wobble but I’m sure I’m beginning to feel some ‘shift’ and loosening)

I want a natural nice smile

Sunday, September 14th, 2008
Q.
This may be a long winded note but I’m hoping you may be able to advise me. I am 43 and my teeth are not great and need work. The work I have done to date is-

1. Removal of incisor not front tooth but next, prep for implant (specialist was not happy to progress as bone did not grow back well enough). I trust this decision and have opted for a bridge – cantilever.

2. 2 x re-root canal – 1 lower molar (first one) fine 2 top 2nd molar – can flare up occasionally. (may need an apisectomy).

3. Prep of 2 upper premolar for crowns.

I think my dentist is OK (part of a dental chain) and was recommended; not cheap but not Harley Street prices – about £1000+ a crown. Root canals were £1400 done by the specialist in restorative work. But practically every time I leave the surgery I seem to have to go back due to pain. The most recent being after a temporary crown was fitted, the whole of side of face hurt. Went back he could not see a problem drilled a bit and sent me on my way with another load of antibiotics. (Is it usual for a dentist to be looking at the internet whilst waiting for X-ray to be developed). Also, he seems to not remember my case notes (or does not look at them before I arrive). I suppose I just don’t feel comfortable with him but now feel I would have to find another and have no idea how to. Nobody I know can recommend one – my husband sees a NHS practice but I have decided to go the private route as want cosmetic work on front. I will be having 3 crowns, 3 crowns or veneers and 1 bridge on the upper and 2 crowns, couple of fillings to replace leaking on lower as well as bleaching the bottom row – the odd thing is I have been told that you should bleach first and then try and match up the natural teeth to this. He did not seem to think this was necessary. Also, on the top I have one perfectly good tooth – I think it a good shape (canine) and would ideally like to just bleach and match to this. It is the front four that are in the worst shape. But he says why am I basing my whole smile around one tooth which I can understand but I have so few ‘virgin’ teeth. I would like to keep the ones I do have. Is this not a challenge for a Cosmetic dentist working with the good teeth? May have to have another top molar removed as endodontist said it was not good enough to re-root canal (it was filled with silver points) and could be destroying the bone around other teeth. My main priority is to have a good bite as I suffer from tmj and facial pain, and clench my teeth. Sort out the foundation I want a very natural smile – not too white and Hollywood a la Jodie Marsh just an ordinary ‘you have nice teeth’ look. Definitely do not want bulbous teeth (chicklets).

I’ve got an agenesis of upper lateral incisors

Sunday, September 14th, 2008
Q.
Dear Dr. Caplan, I’ve got an agenesis of upper lateral incisors. I am almost at the end of my orthodontic treatment that closed the gaps between my teeth. I currently have old composite veneers on my canines that make them look like incisors. After I finish my ortho treatment I will need those veneers replaced. I’m pretty sure I’d like to go for ceramic veneers and because one of the options is CEREC, I would appreciate your opinion on my questions and concerns. I have seen lots of “CEREC smiles”, but the people on the pictures I have seen had a set of veneers on all front teeth. I can’t remember to see anybody with just two veneers, so I’m a bit concerned whether they would look natural between my own teeth. I’ve read on the Sirona website that the veneer is milled from a block of ceramics and that made me concerned about the contrast between the CEREC veneers made from single color block and my natural teeth consisting of several layers of various colors, translucency etc. (you know better than me :-)). I don’t want veneers on all my front teeth, because my other teeth are both healthy and well-shaped. 1. Is CEREC the best choice in this case or would be the laboratory made veneers better, more natural looking? 2. Are the CEREC veneers just bonded to the teeth and that’s it or are they somehow “coloured” to match the other teeth? I understand that the color of the block is choosed to perfectly match the color of my teeth, but now I’m talking about the slight color variations within a single tooth. Just to complete my list of preferences – the most importantant (apart from keeping the other teeth as they are) is the final result. I don’t care that much about the length of the procedure, the number of visits needed etc. Thank you very much for your time and opinion.

I have badly neglected my teeth

Sunday, September 14th, 2008
Q.
I am 62 years old and I have badly neglected my teeth, I have a partial denture and the rest of my teeth are uneven, roots showing etc. In some cases the bone has worn away. All my bottom teeth are okay but very uneven, I don’t know if crowns will be able to rectify the damage. If so I would need approx 16 crowns and a partial denture of 6 teeth. Can you advise on suitability and approx cost?
