Is bridge treatment the answer for my jaw gap?

Thursday, November 5th, 2009

Q. I have a gap on my bottom jaw. The 2nd tooth never grew when I was growing up. My dentist says I could have a bridge but I am reluctant to have good teeth either side filed down. The gap is not very big but is annoying and the tooth from the upper jaw is growing downwards to fill the gap. Would an implant work? I had an implant for a bigger missing tooth on the uppper jaw which is great but this gap is smaller.

A. If the gap is less than 5mm wide then an implant will not be possible. It which case a bridge might be the best alternative

Cost for reconstructing my teeth following periodontal disease

Thursday, November 5th, 2009

Q. I am about to start looking for a dentist to do some reconstructive work on my teeth. I am about to lose many teeth due to genetic periodontitis. I am willing to have some existing teeth pulled if it makes the procedure easier – I do not hold out much hope for most of them. My preferred option would be bridges secured by implants, if possible. Can you advise me of the options I may have and the cost implications of same.

A. Hello, thanks for your message, it sounds like implants would be your best option is you still have enough bone for them. You would need an assessment and a ct scan of the jaws to decide the best treatment. As a rule you may need to budget around £1500-2000 per missing tooth if implants are used. Let me know if you’d like me to take a look for you my clinic number is 01245 268 494

Tooth filling is causing pain while chewing

Wednesday, October 28th, 2009

Q. Hi doc, I am 25 year olf female. I had a filling done at the end of the jaw a month ago and did tooth cleaning. But apparently after the filling in last teeth upperjaw 2 teeth adjacent to it have become sensitive and cause pain when i chew. There should not be cavity coz i recently had a dental checkup. Only 1 teeth was treated that i already mentioned Is it coz i did cleaning and that has hurt my teeth ?

A. The area may need to settle or you may have to have the bite changed

Need more information on jaw surgery

Tuesday, October 27th, 2009

Q. okay well im 15 and i had braces for about 4 years. before i had them i had a bad gap and my teeth were completly crooked. After i got my braces off my teethe became straight and i did not have a gap anymore but the problem is i have a bad underbite and i have a jaw that jets out (long face shaped). My orthodontist told me that only way to fix it is to get jaw surgrey. I was wondering basically all about it, what it does? if i really should get it? how much it cost? -if you need more information or any questions for me please ask.

A. You can have your jaw broken and moved.  This is a “proper ” operation and is not 100% successful.  Discuss the pros and cons with your orthodontist an surgeon.

Can the crown be placed always over the tooth?

Friday, October 23rd, 2009

Q. Hi, I have just found out that my top right rear molar, is dying as a result of a deep filling, which is now beging to rest on the nerve of the tooth. The dentist has shown me the x-ray and said that as the root is so bent and not easily identifiable, he does not believe a root canal would be sucsessful. I have also had a bad expererience of root canal’s, where within months I had absess, and lost the tooth anyway. My dentist has said that my only option really is therefore too losse the tooth. My concern with this is that due to braces and overcrowding I had already had all my wisdom teeth removed and to loose this back tooth, I run the risk of my jaw bone disolving, there is already evidence of this on the x-ray following the wisdom extraction 3 years ago. I asked if there was any other options and he told me I would need splints and a crown which would cost about £2000. I am only 31 and worry that if I don’t make the right choice now it will be too late, I can’t afford £2000 for a tooth, but as it stands a booked for an extraction the week before xmas??!! Reading some of your other responses to questions indicate the crown can be placed over the tooth, can this not always be done? Any advise would be great. Thanks

A. hi many thanks for your enquiry, my advice would be to ask to be referred to an endodontist for a second opinion. THere are many teeth which general dentists just do not have the skill to treat. however an endodontist is highly experianced at root canal treatment and can give you an idea of whether they feel it can be treated and level of sucess. once the root canal treatment is done it is then best to crown the tooth.

hope this is helpful.

Whats the best age to begin the treatment to widen the upper jaw?

Thursday, October 22nd, 2009

Q. At approximately what age should treatment begin to widen the upper jaw of my son? His upper jaw is currently narrower than his lower jaw. May thanks, John

A. This treatment can start earlier than conventional orthodontics but varies according to the loss of teeth and the type of bite problem. You should be guided by your orthodontist for individual circumstances.

I cant have dentures anymore Could you suggest?

Wednesday, October 21st, 2009

Q. please can somebody help me i am 37 years old and had all of my teeth pulled out when i was 18 with 2 wisdom teeth so with not a tooth in my mouth i have worn dentures for nearly 20 years and as im sure you know what im just about to say i wont be able to have any more dentures due to no bone and gum left im getting quite distressed about this as the plates do not fit in my mouth now and the teeth on the plate have grinded so i cant eat or chew my face feels is collapsing.would i not be able to have this done on the nhs.can there be a positive outcome

A. Hi, many thanks for your email. the first thing that should be ascertained is how much bone you have generally i have found in my experiance that once a CT scan of the jaw bones is done, there is usally more bone in the jaws than you think. The next step once the true answer is known about how much bone you have is deciding on the the way the teeth are to be restored. th reason for the CT scan would be to see if you are a candidate for dental implants.

Preferribly four implant in each jaw would give you fixed teeth which would not require removing, the cost of which would be approx £12,000 per jaw.

Two implants per jaw fixed to a denture would be removable but when in very secure. the cost per jaw would be £5000

The alternative would be dentures which is what you already have, but with a better fitting surface and new teeth, these are not ideal, but cost wise may range from £1000 – £4000

There are many practices that offer interest free credit to help finance the treatment. hope this is helpful.

My daughter is 19 how bowen therapy could help her?

Tuesday, October 20th, 2009

Q. Hello, My daughter, now 19 yrs, had ‘holistic’ dental treatment where the arch of her teeth was widened and braces worn so all her teeth could fit in. During this process the dentist did some allignment work on the actual jaw. The work went wrong and she was left with misalligned jawline. Her chin is not on centre line and one side of her jaw is lower than other and teeth on that side do not touch. She finds it hard to open the mouth wide (cannot eat an apple for example) and has tinnitus. She finds Bowen therapy helpful but the jaw is particularly bad at the moment and we need to find proper solution. Is this the kind of case you can help with? If so, how? If not, can you recommend me someone who can, please? The dentist in question was struck off, and so cannot help (probably couldn’t in any case) and we are desperate for top quality help as cannot afford to get things wrong again. Regards,

A. I would be very happy to help but this isn’t my main area of expertise. i would recommend seeing Dr julian Gleister at Umbrella

Are wax moulds bigger than the dentures?

Tuesday, October 20th, 2009

Q. Hi. I was womdering if you could help me. I have recently had all my teeth removed but wasnt offered immediate dentures so have gone without teeth for quite a while. I have had impressions made and have had the wax moulds done. How long does it take from that stage to actually having teeth? and are the wax moulds bigger than the dentures i will have because i couldnt close my mouth with the wax ones and they felt very thick. Thanks for your time.

a>

A. Wax molds or bite blocks are used to register how your jaws are related to eachother.  The final dentures will be smaller.  Dentres normally takes about 5 weeks to make.

Is bite analysis the only solution to correct the teeth?

Tuesday, October 20th, 2009

Q. my husband has been told that the way he bites is wearing down his back teeth. He has been told that the solution is to have bite analysis and then something to relax the jaw to its natural position and then build the warn teeth up with crown/veneers so they are in their correct posistions – this is costly and i wonder if this is the only solution? any ideas?

A. It is a solution, but it is too difficult to give you a proper analysis on complex treatment like this with out see him firts.

Does the expanders causes teeth to come forward?

Monday, October 19th, 2009

Q. Dear Dr.Caplan, I’m 24 years and My teeth have had moved a bit forward and my dentist suggested me expanders and bracers after that. So im wearing expanders now (upper jaw) and suppose to remove it next week. But my problem is, now my front teeth have come EVEN MORE FORWARD and looks bad than earlier.(and of course the jaw has been expanded as well and created gaps between teeth.). so my question is, other than creating gaps between teeth, does the expanders causes teeth to come forward like this ?can this be corrected? please answer. Thank you.

A. The expander does not usually cause the front teeth to move forward but if they have this can be treated with the next appliance

Can I hv another bridge done in NHS Plz advice

Monday, October 19th, 2009

Q. I had a bridge for about 15years which consisted of 2 back crowns a front crown which supported the 2 teeth that were missing on the upper jaw and the bridge was good but it started to come loose and my dentist told me that it was because my gums were receeding so it would have to be replaced so it was with a front and back crown that was nearly 5 years ago but now the bridge has broke off from the front crown, The same dentist has suggested a denture as I would not get another bridge,As dental treatmant is so expensive in the UK I was thinking about haveing it done abroad, So please could you advise me if there is any other options with the NHS about haveing another bridge done? Thank You

A. hi many thanks for your email, i am unaware that the NHS would make a new bridge if the dentist has said another bridge is not possible. my only advice is that if you think a new bridge is possible then seek the advice of a second dentist. i would personally stay clear of dentistry abroad but that is just from what i have seen and had to retreat but you pay your money and take your chances. hope this is helpful.

My front teeth hv started to ache Could this be Sinus?

Monday, October 19th, 2009

Q. HELLO DR PATEL. I HAD A ROOT CANAL ON A LARGE UPPER MOLAR. IT HAS NOW STOPED HURTING. I CAN EAT PAIN FREE ON IT, TOUCH IT PAIN FREE, TAP IT PAIN FREE. HOWEVER IT FEEL WEIRD LIKE THERE A SMALL AMOUNT OF PRESSURE IN IT . MY FRONT TEETH HAVE STARTED TO ACHE SINCE THE ROOT CANAL. MY JAW ALSO ACHES NEAR MY EARS. I HAVE HAD THE BITE ADJUSTED AND ITS PERFECT. COULD THIS BE SINUS? KIND REGDS.

A. If there was infection it could take a while to settle it sounds like the symptoms are reducing so I would wait until everything is pain free.  It could be a sinus infection or the fact that the bite was too proud.  Leave everything for at least 10 days unless symptoms get worse.

Does expanders create gaps and causes teeth to come forward?

Monday, October 19th, 2009

Q. Dear Dr., im wearing expanders (upper jaw) and suppose to remove it next week.then il be getting my bracers to push my teeth back which are abit forward earlier.. but my front teeth have come EVEN MORE FORWARD now and looks bad than earlier. so my quetion is, other than creating gaps between teeth, does the expanders causes teeth to come forward?can this be corrected?

A. hi many thanks for your email, the expander will expand the whole jaw so yes initially this will cause all the teeth to move outwards. the brace will then be placed to then bring the teeth all back into allignment and close the gaps, this should then solve the problem. hope this is helpful.

Which is the best place to get my implants done UK or abroad?

Monday, October 19th, 2009

Q. hi Dr Solanki, i am currently considering have an implant as i currently have 4 teeth lower jaw bridge.where is the best place to have this kind of treatment uk or abroad?i was lookng at one in budapest, hungary and it look quite good any advice?Thank you for yourtime.

A. hi many thanks for your email, i am against dentistry done abroad as when you get back and if you get problems its usually the local uk dentists which then need to sort the problems out and in my experiance the problems are more than the praise. however pay your money and take your chances. if you require help in finding a reputable dentist in the uk i will be happy to recommend one in your local area.

What is the ideal overbite Plz advice

Monday, October 19th, 2009

Q. Dr. Patel, I have had braces now for about 2 years and my orthodontist plans to take them off in a few weeks. I feel I my upper teeth still overlap my bottom teeth too much. I discussed it with her but she just said it was within normal range. A lot of pictures I have been seeing of people that’s had braces have a more flat facial profile. What is the ideal overbite? Should my mustache area protrude out farther than my lower lip area?

A. It depends on the position of your jaws, the teeth can be moved to the correct position but if your bottom jaw is too far behind your top jaw you will always have that appearance.  If this is the case the only real option is surgery to correct the jaw positions.  Talk to your dentist.

The base of the cavity is bit dark after having molar tooth removed Is its normal?

Wednesday, October 14th, 2009

Q. Hi, I’ve had a molar tooth removed recently after it fractured from eating food. I took some photos with my camera to see if it was healing well and i noticed that at the base of the cavity it looked quite dark, almost silvery like my other fillings. Is it normal to be this colour?, could the dentist have just left the lower part of my tooth etc in my jaw? Can i send the photo to get your judgement? Thanks
