I would advise you to contact the NHS directly or your local NHS Orthodontic Dentist and see if you can get an appointment with them.

I have read the information on the website, and believe I might qualify for NHS treatment (I am over 18). Reading your descriptions I think i could be classified as grade 4 or grade5, but obviously i am no expert and my assessment could be inaccurate. How would I go about getting a formal grading? What would be the next steps to find out what treatment that would allow me access to and the associated costs). Thanks

You indeed may be a grade 4 or 5 but if you are over 18 then you would not qualify for treatment on the NHS in General practice no matter how bad it is. Only a hospital could provide treatment for you and you would require a referral from your dentist. Goodluck!
