Can you please advise me on my complex implant case?

Q. I have recently had a severe infection on a front tooth which is crowned and root filled. My dentist has advised me to have an apisectomy but I have also been advised by two reputable dentist friends to skip this procedure and to go straight for an implant. Their reasons being that they think the cyst at the end of the canal is quite large and that I will eventually end up having an implant anyway and that the apisectomy may well not work. I fell and damaged this tooth and my two front teeth next to it 20 years ago. The three are root filled and the front two have veneers on and the one next to is crowned. Please could you advise me as to what you think and also who you would recommend in the Manchester area. Thank you,
A. I would personally go straight to an implant as the procedure for an apicectomy will cause further bone loss, jeopodising any future implant treatment if the apicectomy fails. You could try Nigel Saynor, he has extensive experience in this type of complex treatment.
