should i have an onlay or crown

Q. Hi – I have an existing cap on a molar (actually it may be an ‘onlay’, as it is essentially half a tooth than has been cemented on to the remaining half-molar, which is fully intact – i.e., has not been ground into a peg) which has just broken in half. The dentist I have just seen has recommended a crown – which would require drilling away the remaining healthy tooth base into a peg – rather than another onlay. However, I’d prefer the onlay as it requires less intervention, and no grinding of what is otherwise a healthy half-tooth. What is your view on the pros versus cons of the two approaches? Thanks!

A. I would always go for an Onlay if it is possible. The dentist may have suggested a full crown if there isn’t enough tooth structure left for an onlay, or they may not know how to do an onlay. If you are unsure about the treatment plan you should seek the opinion of another dentist. It is very difficult to tell you for sure without seeing the tooth.
