My teeth are bad because of poor hygiene and illness, can a dentist help me?

Q.
i have severe dental problems broken /missing teeth/ inflamed gum/part post still in mouth.
Have CNS illness/taking anti-epeltic drugs /amytryptiline/oxycontin for pain control. Poor nutrition four five years due to illness, had huge impact on dreadfull state of teeth. Health improving! stopped smoking/sugar substitute. Had inhilation sedation for a while and tolerated it brilliantly.Would any dentist be able to help me. My mental health is suffering because i don’t want anybody to see my mouth.
