My wisdom teeth have been pulled out

Q. Hi Dr Caplan, My wisdom teeth have been pulled out (1, 16, 32 and 17 I guess). I had to get number 14 taken out after having it crowned and infected under the crown. Today I had a root canal on number 15, and number 13 has already been repaired and apparently presents another deep infection, according to my dentist. I am considering 2 options to replace missing 14, either the bridge or the implant. 15 is now “gutted”, and 13 not looking too pretty. My dentist recommends the bridge. However, I know that I am going to be losing jaw bone underneath the bridge, since it will not be stimulated by any kind of (prostetic) root. My question is: what would you recommend in my case? In case of a bridge, how heavy of a bone loss can be expected over, say, 10 years, and how long until I would have to have the bride removed and replaced with implants. I am 30 years old, rarely smoke and am in good shape. Thank you for your answer Dr. Caplan.

A.

In general implants are far more successful than bridges provided the bone is in sufficient quantity and the gums are in good health ( and you not a smoker, or have uncontroled diabetes etc).  If you are a good candidate for implants I would advise you take this route rather than bridges but I cannot be definite about this as I have not examined your mouth.

Thanks for your reply Doctor. This is the route I have chosen as well, the implant is now screwed in my jaw and waiting to be colonized by the bone. My dentist mentioned that he was able to get a slight sinus anchorage for it, and it seems that this would be the recommended procedure for implants below sinuses. However, I read elsewhere that this piercing the sinus bone should be carefully avoided, as this can cause serious trauma. What’s your idea on this? Thanks!
