I sucked my thumb as a child and have had problems with my smile all my life, i also have gum disease, what should I do?

I sucked my thumb as a child (I am now nearly 60) and subsequently my teeth stick out. I was called goofy at school. I have had 3 teeth removed from my upper jaw either side of my front teeth (2 on one side and one on the other) I also have gum disease. My top 2 front teeeth have been crowned and one of the crowns is moving so it all looks a bit of a mess!
I have been very self conscious for years and I would love to have a better smile but apart from being nervous of the dentist I am also very embarrased and put my hand over my mouth when I laugh!
Cn you help me, please?

The first thing you would need to assess is the gum disease. Firstly I would advise a consultation with a specialist Periodontist and take the rest of the treatment from there.
