can you help me komal with my daughters teeth

Q. Hello Komal, not too sure whether you will be able to help me or not; but here goes. My daughter has just turned 4. We have had a problem since here teeth first appeared. Mainly the 4 front top and 4 bottom. At first the dentists thought it was flourosis, so we treated her accordingly, however after extensive hospital tests it proved nothing of the sort. As we were advised to avoid flouride, we now realised she needed it more than ever. Her teeth on the face and cutting edges started to crumble. We were referred to the Orthodontics hospital in Bristol where she had white painted material intially applied, and then over later months almost like a white filling applied to the directly affected areas. The problem is that these filings fall off after a week or so and then her teeth become too painful for her to eat. We have been told that they can keep reapplying these surface fillings, however we were wondering whether there is a more permanent alternative? The last resort is to have the teeth removed with GA but the hospital do not think these teeth are bad enough to merit this. Due to the constant irritation and pain my daughter is in, is there a cosmetic solution that we can apply to these milk teeth? veneers? I can send you some images of the affected teeth if you believe this would help in your appraisal. Many Thanks in advance.

I understand that the situation must be quite distressing for you.
It sounds like she has had fissure sealent material placed on her teeth which is only a 50% filled resin material. This means that compared to a normal white filling it is 50% weaker. Proper white filling material would last longer on the teeth but in order to cover Isabella’s teeth with these it would take more time than the fissure sealent, moisture control is essential, quality of the tooth structure needs to be good and we also need patient complience.She is only 4 years old and therefore some of these issues may not e able to be controlled. Another reason that the sealents fall of is that they interfere with the lower teeth and ideally a small amount of drilling may be necessary which is always difficult on a 4 year old. These teeth will exfoliate when she is 5 years old and they will guide the eruption of the adult teeth. Only you know how much pain and distress Isabella is in and and if you feel it is best to have the teeth extracted then this your decision. We have to make the right decision for the patient and this may be the right one for Isabella. She will be able to use the gums to eat with until the adult ones come through. This is better than not being able to eat at all because the teeth are hurting.
If you want to give restoration one last try then it may be better to have Isabella placed under sedation, which is not as risky as general anaesthetic and have the white fillings placed.
Some private dentists have consultant anaesthetics provide this service at the dental practice.
