Q. Andrew, I am 24 years old and i have mild gum recession at the top and bottom of my mouth, the front 2 top teeth and the bottom 4 teeth. This is making me very depressed and stressed i have considered plastic surgery to help orevent this from getting any worse, would i be able to do this? What sort of cost would this incur? Q. Hello I am type 2 diabetic ( controlled at present with diet and excercise regime ) I have lost teeth to upper left 3no and 2no to lower right. I am considering implant replacements . Would my diabetes compromise implant surgery . Also if positive what type of implant system is the best ?
A. Uncontrolled Diabetes is a contraindication to surgery and does adversley affect the predictability of dental implants. care should be taken if you lost your teeth due to gum problems.
Most of the main systems have “Bioactive” sufaces like Osseospeed (ASTRAtech) SLA active (straumann) and Nobelbiocare, Biomet etc, I mainly use Astratech dental implants because it is proven to maintain the surrounding bone in later years.

Q. I’ve always had lovely teeth & visited the dentist regularly.About a year ago I noticed that every time I brushed, my gums would bleed.My dentist suggested various gels for gum disease,but now my top front tooth is slightly twisted and I still bleed.After having some x-rays Ive been told I have lost bone tissue and the next tooth may also come loose.I asked about implants. It has been explained to me that because there is a loss of bone an implant would not be possible at the moment.A bone graft/implant has been suggested.Can you give me some information on this.Where is the bone taken from? Is it done under general anisthetic? I am extreemly worried. I want my teeth to look good and I dont like pain.
A. Thanks for your questions, it is possible to carry out the procedure using an artificial bone, to prevent the need for a graft from another site on your body. This procedure can be carried out under local anaesthetic, although some clinicians will be able to offer Intravenous sedation, if you feel this would help you. The aartificial bone is then allowed to integrate for a period of months, before the process to place the implants would begin. I hope this heglps.

Q. I have a temporary on a tooth. It has been in for about 2 weeks. Just recently my gum in front of the temporary is really hurting and is bothering me above the tooth and in between the tooth. What should I do? I have flossed and no food is coming out? Could it be that I have cut the gum with my floss?
A. It does sound like a gum problem, get your dentist to check it out next week

Q. I have had one place on my lower front tooth that has been receded for 10+ years. When I first noticed it I started using the Sonicare toothbrush and have used one twice a day since that time. I have obviously purchased new ones over the years, and do not let the head of toothbrush get old and frayed. My overall dental health is excellent, my gums do not bleed when brushing or flossing. Recently (approximately two weeks ago) I noticed the gum area below the tooth was very tender and when I looked, I saw that it was extremely white all around the base of the tooth. I had been using one of those peroxide whitening mouth rinses once a day for about one year. I thought it may have been related so I stopped using it. Since then the area is still very white, with no pain (unless I mess with it.) The gumline below the white area is somewhat more red in color that the rest of my gumline, but again no tenderness or pain. I do not smoke,have never chewed (I am a female)I drink about 3-4 cups of coffee per week. I take a blood pressure med once a day and am 51 yrs. old. Should I be concerned and see a dentist or am I being an alarmist?
A. Go and see your dentist for them to assess the situation

Q. i’m 22, and i brush my teeth at least twice a day and floss at least once every day… i haven’t been to a dentist check up in like 5 years… some of my teeth seem to be kind of loose, is not my finger or anything, i’ve tried with tweezers and they actually move a little bit…. i’m very concerned… I drink tons of milk and eat very healthy!
A. See your dentist as soon as possible because you may have gum disease

Q. hi doctor i wold just like to ask you i have very severe tooth problem, i have all my 4 wisdom tooth coming out,recently my my two front tooth and tooth beside those two front tooth is moving together and causing crowding. i have 2 tooth above on both sides that havent been extracted and the tooth have left small parts inside my gums on both sides therefore it is causing my tooths to crowd because of the 2 unextracted tooth. the gums now are so swollen and my teeths have all moved forward causing my lips to be swollen and i cant smile becaue my face looks different now, you can tell that my tooth got forward and im really bothered by it. i cant go to the dentis for another 1 month because of financial problems is there anyway i can stop this? and my lips swollen? ive stopped going to school now because of this as im so embarassed of my tooth.

Q. Hello Dr., I read about enamel erosion on front teeth and I believe that this is my case, i have a few yellowish/transparent spots on my 2 front teeth. I read from one of your replies that I can’t restore the color with diet or habit, however what can I do to prevent more erosion and to at least keep it as is. thank you
A. Check your diet for acidic foods particularly fizzy drinks diet or otherwise. This is the cause of most erosion so reducing this will help prevent further enamel loss

Q. I now have the “Old-fashioned ectremely expensive complete top veneers – the type that they had to grind down all of my teeth. This was recommended because I had gum disease which was supposedly cured after surgery. Now, I have receeding gums – just one tooth at the top that shows my own tooth and the space between! It looks awful! Can anything be done to correct this?
A. Please consult a periodontist to see if a gum graft would be possible.

Q. recently i had all my teeth pulled, one wisdom or the one next to it had a terrible problem taking it out, it was all cracked up so they haad to drill, and break it out, im a diabectic and my doctor said i would’nt need antibiotics,now it seems the gums are infected, it makes my head hurt, i gets a little nautious, is their any i can do at home until i get to the dentist.

Q. What procedure is fine scale? If the dentist says your coming back for fine scale. Will it hurt?
A. It is a cleaning and it is not painful, may be a bit sensitive depending on the health of your gums.

Q. hi there…im a 23 year old,and have many propblems with my teeth,ive been 2 the dentists many of times,and the visits have resulted in me having gum infections everytime,i brush my teeth 2-3 times a day,and im always using special mouthwashs and they just dont seem to be getting any better,they look like there on there way out,im now starting to loss confindence in myself,i fill like i cant talk to people etc im on benefits and wondered if i could get help in anyway with getting them sorted..via the nhs??? many thanks…
A. Many thanks for your e-mail, usually when a patient is having continual problems with gum infections their dentist will recommend that they see a dental hygienist. In the beginning the visits may be quite regular, and on improvement of the condition the hygienist may recommend regular visits although with slightly less frequency. You can find your local NHS dentist by visiting NHS Direct or by contacting your local Primary Care Trust. I recommend that you visit a dental professional at your earliest convenience. Many Thanks. Q. Dear Dr Moore I have a bridge (1 missing tooth upper jaw the 1st molar), placed in 2003. I did not have root canal treatment to any teeth prior to placement. It is porcelain and i assume bonded to metal. When I first had it fitted I noticed little bits of cement in my mouth. I had it fitted whilst I was working in the Cz Republic and therefore I was not able to return for a check-up. I had severe pain for a while and then extreme sensitivity for 3/4/5 years or so. Sensitivity has stopped now, but I get occassional problems. But the main problem has been a continual foul taste coming from the inside gum margin with the back crown. I have visited my UK dentist several times, who assures me the crown/bridge is OK, I had x-rays etc. I did also see another dentist who also took an X-ray and said all was fine and said I had some ’slight’ gum disease and was advised to use dental brushes and mouthwash – which i do with little effect. I can pull the back crown ‘up and down’ a little, which would suggest to me it is slightly lose, perhaps indicated by the little bits of cement when it was first placed, although my dentist said the bridge is fine. Also if I run my finger along the gum margin it does ’stink’ and my mouth tastes foul at times.. My boyfriend also says my breath ’stinks’ which I am sure it does and he has a bad sense of smell. I am assuming my options are to have the bridge removed but I a concerened the ‘posts’ of the crown tooth may come away or some other problem. I have now decided I need to do something, as 6 years of bad breath is no good. Unfortunately 2 dentists have told me nothing is wrong and are reluctant to do anything. Your advice would be greatley appreciated. My apologies for the ramble. Kind regards
A. I would remove the bridge beacuse it shouldn’t move up and down at all. I would take an educated guess and suggest that one of the anchor teeth hs decayed and may need removing or root canal treatment prior to a new bridge being made. It might be worth seeing a private dentist for a final opinion on the bridge.

Q. Hi, i am considering having veneers to the the upper front jaw. i have spoken to my dentost and he said that i would only need the first four done. I am really nervous about getting it doen but it’s something that i really want done. i hate my smile and am very concious when i smile. I had a bad experience with my last dentist as a teenager and as a result of poor comunication i have been left with a crown on my front tooth. it stands out to the other teeth and there is a black line along my gum where you can see the join. Is there something i can do about this and also if i had veneers would they all develop this line in time? Please help?
A. This crown could be replaced with an all porcelain crown to eliminate the black line. The veneers are just made of porcelain so you shouldn’t have the same problem as the existing crown.

Q. Thank you for your swift reply to my enquiry regarding a swelling under my arm after an infected wisdom tooth. You said that the swelling could be related to the infection or I may have a dry socket. I have read a few websites and as I understand it dry sockets occur after the wisdom tooth has been removed – I have not had any surgery to remove my tooth just antibiotics for the infection, so could the swelling still be related?
A. Yes it could – we call it pericoronitits which is infection of the gum over the partially erupted wisdom tooth. This can be very painful and it is not unusual for the patient to have a swollen face from this seemingly minor infection

Q. hi,i had a bridge fitted on my front teeth as my crown fell out and root snapped.My dentist fitted bridge last October but feels really bad and i cannot get used to it,my dentist also advised would take my gum 5-6 months to stop healing before he could do anything with this,can you please advise will my dentist be able to rectify this and make it look better.
A. In these situations I will place a temporary bridge to allow the gum to heal properly and form a nice shape to accept the bridge. This is then changed to a permanent bridge after about 6 months – this will feel smooth and comfortable in comparison to the temporary bridge which is basically made out of plastic

Q. HI, 2 YEARS AGO I HAD TO HAVE SOME BOTTOM FRONT TEETH REMOVED DUE TO GUM DISEASE. I HAD THEM REPLACED WITH DENTURES. I HAVE ALSO LOST MOST OF MY GUM AT THE BOTTOM. THE DENTURES ARE NOW QUITE LOOSE AND AS THEY HAVE SETTLED MY TEETH AT EITHER SIDE OF THEM ARE NOW HIGHER, WHICH MAKES ME FEEL UNCOMFORTABLE. MY DENTURES DO NOT HAVE ANY GUM UNDER THE TEETH WHICH MAKES THEM LOOK LIKE FANGS. COULD YOU PLEASE ADVISE ME AS TO WHETHER A DENTIST CAN MAKE FALSE GUM DENTURES WHICH WOULD NOT MAKE MY DISAPPEARING GUM SO VISIBLE WHEN I TALK OR SMILE? A. Dentures can be made to look very realistic, replacing not only the teeth but the underlying gum as well. I suspect you had your denture placed immediately after the removal of your teeth. A new denture should solve your concerns

Q. hi DR moore I have a crown on the upper right jaw that has become sensitive to hot food etc The pain is only mild, and seems to radiate to other teeth on that top side. There is no pain when i bite on the tooth,also there is no swelling around the tooth. what could be the problem. thanks for your time.
A. There is a chance the tooth could be dying. It would be worth getting it checked over as pain with hot is a symptom of a dead or infected tooth.

A. Very, very unlikely that your face will be affected by this procedure. You may have a slightly swollen face for a week at that.

Q. hello, i got an infection on my right lower molar i belive because the molar is cracked to the root. now ive been to the emergancy room and they just prescibed me antibiotics and pain medication, i spoke with many dentists and they tell me they can not do anything untill the swelling goes down. now the swelling looks like i have a 4″ jawbreaker in my cheek. should i just have the molar removed. should i see the dentist now or wait untill the infection is gone. i dont want to pay if all the dentist is going to say is wait untill the infection goes away?
A. The reason that the dentist is telling you to let the swelling go down, is because it is very difficult to become fully aneasthetised when an infection is present to have a tooth extracted. You would be in excrutiating pain otherwise. Trust the dentist and wait for the swelling to go down. The infection won’t go away, but the symptoms will ease temporarily. Hope this helps.
