    If i choose to have an NHS crown, when it arrives, if it is not adequate, and my dentist has already removed my temp crown... does my dentist still get paid for removing my temp crown in order to attempt to fit the new permanent crown but has had to send it back with the complaints, then re-fitting the temp crown? And if this process happens again and again as the crown is still not adequate, does the dentist get paid through NHS for each failed attemp?

    NHS crowns will fit and function adequately. However there can be a difference in quality. The NHS restricts fees for crowns. I can however assure you that NHS crowns should be adequate. They do not however have the facility to create highly aesthetic crowns at the front of the mouth, this is a difficult procedure to undertake and it is often possible to see NHS crowns (even many private crowns) at the front. I have seen many NHS crowns at the front which are "adequate" in cosmetics

    The quality assurance from the NHS means that there is a minimum standard, and you would not be able to demand a private level crown on the NHS. Do also be aware that there are different levels of private crowns.
