Instant Implant?
Hi all,
I have a question for a friend of mine who lives in Turkey.
She has to have a LR6 extracted as it is heavily decayed. The tooth was root filled and crowned a long time ago in the UK.
Her Turkish dentist has put her on antibiotics for a week prior to removal.
Now comes the question , he wants to do put an implant in on the day of extraction, I am a dental nurse myself and I have never heard of placing an implant instantly in a situation like hers.
Shouldn’t the socket be packet with crystals and shouldn’t there be a waiting time to see if there is enough healthy bone left and to make sure there is no infection left?
Although, she is a moderate smoker, quite likes her drinks and has mild parodontitis.
She still has her LR7 and LR5 wouldn’t a bridge be more advisable for her?

PS: up to now she has been very happy with her dentist and he has done some really nice crowns for her before

There are certain cases where implants can be placed on the day of the extraction. Without a full consultation its difficult to say. Implants are far better than bridges. Periodontitis is not a contraindication to implants as long as it has been treated and is stable

Thank you so much for your super fast reply Dr.Khaira,
I was aware that instant implants are done in some situations but was concerned about her circumstances. She has quite some bone loss already (she hasnt been treated for periodontitis) and I thought her infection (for about 3 month now) in the LR6 area would make it advisable to wait with the implant after extraction.

I personally would recommend to let the socket heal and then place the implant. This can be done 8 weeks after the extraction.
After speaking to our implantologist, Dr Bohara, at our practice, he said he never places immediate implants in molar sites. He prefers predictability, therfore he waits for the socket to heal.

Thank you Dr.Sepehrara,
That’s what I thought as well, could you just help me one more time by advising me about socket treatment in her situation, should there be any special treatment to prepare the sight for future implants? And how long should she wait to have the implant placed?

it is very difficult to answer whether any socket grafting is required, without looking at the xrays.
In majority of cases , we do not socket graft , in the molar site. We let it heal for 8 weeks and then place the implant. In cases where there is insufficient bone after extraction, there are variety of options dependable on the situation.
1. socket graft
2. guided bone regenaration, etc
This is just an overview as it is dependable on the clinical and radiographic examination.
