Would a apicoectomy help reduce inflammation following a root canal or is extraction necessary?

I’ve had two RCTs from the same endodontist and in both cases, the pain remained. X-rays are fine. second opinion agreed. He will not redo the RCTs, only extract. I saw on the X-ray that sealand had leaked. Would that cause inflammation? Would an apicoectomy help? I face losing two teeth. also, is tooth ache after three days normal post-op (after two relatively pain free days?)

Yes 2-3 days post op pain is normal, if material is extruded from end of the canal then this can occasionally cause pain. It might be worth hanging in for several months before considering what to do.
