Q. Hi there. I had to have a tooth refilled yesterday as half of an old one came out whilst eating an apple. Everything went ok, but today above my teeth is very sore and the bridge of my mpouth is very sore, I presume this is because it’s the same place i was injected to numb the area?? I am a very nervous patient, and my main concern is that the replacement filling feels too big and is stopping my teeth come together properly. Is this something that will sort itself out, or will i need to get it ‘reshaped/filed down’? Also will i need to be numbed again for this to happen? thankyou
A. Hi there, it sounds like the filling may just be a little high, and as a result of this you are biting down on this tooth first. If this is the problem, then your dentist should just be able to slightly adjust the restoration for you, in order to correct the problem. This would not usually require further anaesthetic. Q.
I got my upper left Molar filled about two weeks ago it was a deep filling I was told almost to the nerve. Now I am having numbness on my left side tongue, all the outer parts of my cheek, and only half of my lips,and some of the outer skin surrounding my lips. This is all however on my left side only. My gum is a little swollen around the tooth but nothing very prominant. Will this go away or has there been trauma to the nerve? I have had it for about a week now. I also have Multiple Sclerosis and want to make sure it is not related to this since the pain came on a week later. Please help!! I do not have dental insurance so I don’t want waste a trip to the dentist if I don’t need to. This also has affected my taste of things.

there are a few possibilities. the most frequent cause is temporary bruising around the injection site used to block lower molars. as you have ms I would check with your doctor if you have numbness that hasn’t improved 1 week after the fillings Robtennet

Q. Hi i’ve got a darkened front tooth, that came out about 22 yrs ago, i pushed it back in myself, it has started darkening over a period of some yrs and is now looking bad especially in photographs where it shows up very bad i don’t like smilling for photos anymore. My last visit to my dentist told me that it was a lack of oxygen getting to the tooth that is the problem but he would not take it out or crown it as it was still healthy otherwise. i would love to get it capped or have a veneer over it but how can i go about getting it done, privately would seem the answer but the cost is off putting, are there any other alternatives to this ? how can i persuade my dentist to do the work for me ? any advice would be good
A. the nerve will have died during the trauma. first you remove clean and seal (root canal therapy) then an opaque veneer. private fees guide price 500-600

Q. Hi, I recently chipped one of my front teeth, and yesterday had it filled in (I think with a resin). Since then, I’ve noticed a sticky gunge collecting around it-similar to saliva, but thicker and sticky. Is this normal due to how recently the filling was put in, or do I need to go back to my dentist.

Q. A NHS dentist at a previous check up recommended that I have small fillings put in 4 of my bottom front teeth to stop them chipping further. However, I have dental insurance with a private dentist from whom I have had rout canal treatment carried out on other teeth and he advised me that this would not be a good idea. My teeth are not painful and the chips are not very noticable. I am slao in my fifties.
A. If you do not need to get this done for pain or function and you dont think it will improve your teeth aesthetically then I would not get it done.

Q. I would like to have my amalgam fillings removed safely. Can you advise me about the procedure as different dentists use different techniques.

Whenever I replace Amalgam fillings I always use a rubber-dam. This ensure no amalgam infested water is swallowed by the patient.

Q. Hi Andrew,I recently went to the dentist(the dental clinic)and they said I need 2 small fillings,these are on the lower left and lower right molars.They will use composite filling and they will charge me £186 and £162 for each tooth,plus £79 for each Hygiene visit(2).It’s a private clinic but I find it very expensive,what do you think?
A. As a comparison only, our fees for a medium white filling are £118 and Hygienist session £48, however it maybe more important for you to feel comfortable with your dentist rather than judging on price alone

Q. I recently broke a piece off a back tooth on which I had root canal treatment a few years ago. Both of my front teeth are crowned and I was also experiencing a little “vibration” in one of them. On visiting my dentist yesterday and after taking x-rays, she says the root canal treatment I had years ago was “incomplete” (it was done at the same practice but by my previous dentist who has now left) but there is no decay so recommends just a crown as there is not enough tooth left to fill. As for the front crown I have an infection for which she is recommending root canal treatment and a new crown. She has quoted me £450 per crown plus £360 for the root canal treatment Initally I thought this sounded very expensive but after a bit of research have accepted these are probably the right prices – however my main concern is that the new crown will match the one next to it? I should be grateful for your opinion on the treatment recommended and the price – and is there a chance they won`t match? Thanks you for your time.
A. To answer your last point first – to match a new crown to adjacent old crowns or teeth is extremely hard to do. There is a chance of an incorrect colour match. Personally I do such treatment and usually manage to get a great result but this is by colour correcting the crown at the time of fit using a porcelain furnace in my surgery. This is very unusual in dentistry and despite all these efforts the colour match may not be “perfect”. As far as price goes the fees you have been quoted are very reasonable. In fact possibly to low if you are looking for a perfect match as the dentist is likely to have to make adjustments to the color over several visits.

Q. Due to an old filling falling out, but not giving any pain, just a hole,at the base of the tooth. I told the Dentist that I was able to still eat nuts with this tooth! The Dentist gave me a white filling. I rested the new filling for two days. However I am now unable to eat anything using that tooth. I went back to the Dentist complaing about the filling. The Dentist removed some of the top part of the filling incase it was a bit proud. I went back again a few days later with the same problem, he shaved a bit more off the top of the filling. Still no change re eating with this tooth. Could it be that a nerve has been pinched when the filling was done? Can it be put right? Will this be a costly thing? and should I return to the same Dentist or find another local one?
A. I would remopve the whole filling and place a temporary filling. Once the tooth has settled another filling could be placed perhaps the bond between filling and tooth isn’t strong enough?

Q. Hello, I have just been to my dentist as I was experiencing pain in my lower back teeth. I had an x-ray done and was told that my lower back teeth were fine but that I have alot of decay in my top canine tooth and that it will need fixing immediatly. She then poked and blew air into it asking me if it hurt which it didn’t and then said I would need the tooth sorting but said that it would be extremley difficult. I asked if she meant if I was going to lose the tooth or need a root canal and she said no nothing like that, she would just need to do a normal type filling but just that it was going to be very difficult. I am now scared and don’t understand. Please could you explain this to me a little more? What happens during a filling of you canine tooth and why would it be so difficult? Please please help!!!
A. I am not sure what your dentist meant either – you need to go back to her and ask her specifically what she means

Q. I had my root canal retreated. I have a gold crown and the retreatment was drilled thru the top of the gold crown, can the crown be filled with a gold filling?
A. Yes it can but the cost benefit is less than replacing the crown. A simple amalgam can be used as a short term repair as an alternative.

Q. is it normal for cotton wool to be placed in the tooth then the filling on top?? as i have just had my tooth out and keept it i scraped out the filling to see how big the hole was and to my suprise at the bottom was a lump of cotton wool could this have cause my infection i had.. waiting your reply
A. The cotton wool is used to hold the antiseptic in the tooth. By the way why would you scape out the filling??

Q. Hi, my teeth are pretty much straight but different lengths. I want to get them whitened but also want them level (its like they need filing across), is this an easy procedure and roughly how much would a dentist charge. Thanks
A. You can only file a certain amount off the teeth without them needing a veneer being made to reshape them properly. It is possible to make minor adjustments though and with tooth whitening the final result can be very good. You should budget around £500 for tooth whitening and minor recontouring and from £500 per veneer if required.

Q. I had root canal treatment severeal years ago and now the filling has been lost. My dentists told me that I won’t need a filling, but I can feel the tooth if I tap against it!! Is this normal opr should I have no feeling at all? I am a nervous patient and would like some reassuarnce as I don’t really want to start and then have to stop due to the pain,
A. the seal above a root filling is almost as important as the root seal. If you plan to keep the tooth in the longer term, I would want reassurance that the broken filling was still a good seal.

Q. Dear Dr How valid the calim of this tech” CEREC 3D CAD scanner” versus lab work interm of accuracy. I know the time is 1/600HR waiting ,but is it true NO micro annulas due to accurate dimintion of the tooth. 2-Is the ceramic filling more advance relaible,durable than the composit filling if the ceramic price is 10 times more.Do you advise patients to use it? Thanks
A. Cerec is an extremely accurate technique provided the dentist produces a very smooth and accurate tooth shape, is able to accurately take an optical impression. Prof Miels recent article shows that a 19 micron fit is possible.

Q. Can a bridge be secured at L3 and R3 to hold L21 and R12 ? Or would an implant (or two) be required at L1 and/or R1 to give more support ?
A. A bridge can be place provided the canines are healthy and have good bone support. These days I would recommend implant sif possible as they are likely to last longer and no damage will be done to the lower canines

A. I am fine- back from holiday! thank you for asking. Dentists are highly trained and there are no real common mistakes – failure to remove all the decay , over preparation, poor bonding technique, poor moisture control ETC but these are unusual. Hope your filling is successful

Q. hello, i’d just like to know if i’m dentally ‘normal’?! i’m 32 years old, female, and work as a government researcher. i’ve got 7 fillings on my teeth (a couple very large) and a crown with a root treated tooth. is this normal for my age? i realise that everyone’s different, but, in terms of the cases you see everyday, does my dental work fit my age or have i had lots done? many thanks.

Search on Adult Dental Health surveys ( UK ) for the super detail. You are dentally around DMF(T) 8 ( Decay, Missing or Filled Teeth) assuming you havent had adult teeth extracted (excluding wisdom teeth) You may be unusual in wanting to know.
