Q. I’ve just had a filling – there was decay under an existing filling (spotted via X-Ray) and some of the original filling had to be removed to remove the decay underneath and then place new filling on top. It has been 18 hours since the filling and I am still very sore. Is this normal? It’s 4am in the morning and I cannot sleep because of the pain/soreness. I could not brush over the recently filled area before going to bed either as it felt very sore/sensitive. I have had fillings before and cannot remember the pain post-filling lasting this long. Thank you
A. Thanks for your questions, I am sorry to hear that you are suffering. I would advise that you return to your dentist at your earliest convenience to have the filling checked, as it is keeping you awake. It is possible that the filling may just need some slight adjustments, but I would encourage you to have it checked.

Q. Dear Dr Caplan, have the fillings in my front front teeth an the look horrible i have had them since a child an now i am 23, however on my left hand side im missing a canine so i have a latral.An on my right i have a canine which has been made to look like a latral. both side have centrals. my right side has a root cannl which has started looking darker is there anything i can do ie whitening all my teeth or have crowns an these but i want my other teeth to look whiter to please help

Q. Hi doc, I am 25 year olf female. I had a filling done at the end of the jaw a month ago and did tooth cleaning. But apparently after the filling in last teeth upperjaw 2 teeth adjacent to it have become sensitive and cause pain when i chew. There should not be cavity coz i recently had a dental checkup. Only 1 teeth was treated that i already mentioned Is it coz i did cleaning and that has hurt my teeth ?
A. The area may need to settle or you may have to have the bite changed

Q. I have pain in one of my back teeth which had an existing filling. My dentist took xrays and advised of a crack. He has now cut away half of the tooth and is intending to place a Prcelin cap on it in two weeks time costing £650. There is still a large crack running down the back of the tooth that is now visible. It it correct to place cap on this now? Thanks
A. They are trying to b ond the tooth together and prevent the crack spreading. If this works your tooth should be fine for many years

A. You need to see a dentist and get an x-ray it may just need a simple filling.

Q. Hi. I read in ‘Guide to NHS dental Services’ that white fillings are provided by NHS(doesn’t specify front or back). I called a few practices to ask this and I was told that white fillings are available only for the front teeth. But my children had white fillings at the back and didn’t pay anything. So which is the truth?

Q. Hi, I’ve had a molar tooth removed recently after it fractured from eating food. I took some photos with my camera to see if it was healing well and i noticed that at the base of the cavity it looked quite dark, almost silvery like my other fillings. Is it normal to be this colour?, could the dentist have just left the lower part of my tooth etc in my jaw? Can i send the photo to get your judgement? Thanks

Q. Hi there, I suffered from bulimia when I was 18 & 19, I’m now 36 and my 2 front teeth have several fillings caused by the bulimia. I’m happy with them at the moment, although the fillings did fall off and had to have them redone but my dentist has said that I should have crowns and that Lava crowns would be the best solution. He has said that I should have them on the front top 4 teeth. Is there a problem with me leaving it as I’m happy with the fillings and think that Lava means that I will have to have a lot of my real tooth filed down and think what is the point if it looks ok and isn’t really causing me too many problems.
A. If you are happy with the fillings you have, then the best option by far is to leave them as they are. If you want to change them in the future i dont see why you cant have them changed for fillings again or even veneers as these are both much less invasive than a crown.

Q. I was diagnosed with slight TMJ many years ago and it show up now and again with a lock jaw and difficulty when eating. I used to sleep with a guard on my upper teeth. Do you think it is worth having another guard made? Also do you think an old filling on the bottom molar teeth could be contributing to the lock jaw pain? I plan to be in London during the summer. Q. In my most recent dentist visit, my dentist had replaced several of my silver amalgam fillings with composite ones (not sure which exact type but it did involve UV light and some sort of bonding agent). One of the teeth was a Molar with pretty big cavity the filling covered most of the top of the tooth/chewing surface). Afterwards, I read online that specifically for Molars and certain cavities (not sure which ones) composite fillings are recommended less than silver (amalgam) ones due to increased problems with leakage, less longetivity (about 5 yrs less than amalgam), and if compromised higher chances of having to do a root canal to fix it. At the same time, I am pretty sure he used the most recent version of composite filling used in US – because he just recently started using it and was REALLY proud of it. So the material was the latest version, not sure about his technique though since its my first experience with this particular filling method. This made me concerned, since the cavity I had was pretty deep and another dentist (couple of years back) warned me that next time I might have to do a root canal because of the depth of the cavity. My question is was my current dentist right in replacing my old amalgam fillings for a big cavity in my molar – the cavity was at the top of the tooth (chewing surface) with composite fillings instead of using silver amalgam again?
A. I do not use amalgam in my practice, i only use composite for fillings, or if the cavity covers a large part of the tooth surface then i will use a lab made restoration called a porcelian inlay. However this restoration is a lot more costly.
Without seeing your teeth, in my opinion your dentist did the right thing by replacing your fillings with composite as amalgam is not adhesive and in some cases more of the tooth surface has to be taken away in order for the filling to stay in. For this reason i do not use amalgam, along with the obvious reason of its appearance.

Q. Hi Doc I have a chipped upper left single tooth which is not broken. I have protected it with dental cerment for now. I live in a caravan and move every 28 days. I am also constantly looking after my pet dog who requires 24 hour home care due to an illness. Is the dental cerment the best temparly thing to use until I get to a dentist. The chip is at the awkward side. It is important that I protect this tooth is that correct. Any advise please. Kind regards

Just lost my filling and can’t afford to go and see a dentist…need something to block the hole so the nerve doesn’t start to play up in a couple of weeks time…. Q. Dear Dr. Patel, At the age of around 12 I chipped my front tooth after a fall leaving less than half of my front tooth behind. I had a filling applied to this, but the tooth has gradually darkened over time. I’ve heard that this is due to internal bleeding within the tooth. Yesterday, this filling fell out and I am yet again left with half a tooth. The tooth in question is a few shades darker than my other front tooth and it looks very bad. I’m an 18 year old on a gap year, with no money at all. Would the NHS be able to help with my problem because it really looks awful? Thank you
A. You should be able to get NHS treatment you might need a root canal treatment and maybe a crown, or a bonded filling and internal tooth whitening.

A. Post crowns are the last chance to try to save the tooth as you know there is very little good tooth left. Long term you may need an implant or bridge.

Q. I am a UNITED STATES 43 male, and just had checkup. I have reflux and have ground teeth into the denton. Kanines are level w/others. molars have very wide fillings. backs of front teeth have been acid washed to denton, i guess. told i need 14 crowns immediately. front upper 6 +canines+back molars. $12K. is there some insurance I can buy to offset this? or is it too late, since I already saw the DR. I am scheduled for this friday. Please respond ASAP. URGENT to know. what is your advice? do you think this is urgent/necessary??
A. If teeth have severe acid erosion the options are onlays or crowns depending on how much tooth is present. I am a British dentist and do not have knowledge of your insurance schemes

Q. Hi. I had SEVERAL fillings done about 1.5 months ago, and on occasion, those teeth will hurt, especially when I chew gum. It seems that the sensitivity to hot and cold has diminished considerably since I had the fillings done, but most times when I chew gum, those teeth hurt. The fillings are white, not silver. Is this normal?

this problem can occur several factors play a part the depth of the filling/closeness to the nerve.

as symptoms are improving over time -give it a bit longer or contact your dentist to check everything is ok as it may settle

Q. my mum forgot that a dentist appointment was scheduled for her it was ment from 11.30 to twelve and now it;s been moved to the 26th. i just don;t understand how she forgot i was there when she aranged the apppointment. you see mums mouth has a hole in one of her teeth and it needs to be fiiled as my mum has lost her filling. if my mum leaves it this song. what will happen ?

Q. Hi there. I had to have a tooth refilled yesterday as half of an old one came out whilst eating an apple. Everything went ok, but today above my teeth is very sore and the bridge of my mpouth is very sore, I presume this is because it’s the same place i was injected to numb the area?? I am a very nervous patient, and my main concern is that the replacement filling feels too big and is stopping my teeth come together properly. Is this something that will sort itself out, or will i need to get it ‘reshaped/filed down’? Also will i need to be numbed again for this to happen? thankyou
A. Hi there, it sounds like the filling may just be a little high, and as a result of this you are biting down on this tooth first. If this is the problem, then your dentist should just be able to slightly adjust the restoration for you, in order to correct the problem. This would not usually require further anaesthetic. I got my upper left Molar filled about two weeks ago it was a deep filling I was told almost to the nerve. Now I am having numbness on my left side tongue, all the outer parts of my cheek, and only half of my lips,and some of the outer skin surrounding my lips. This is all however on my left side only. My gum is a little swollen around the tooth but nothing very prominant. Will this go away or has there been trauma to the nerve? I have had it for about a week now. I also have Multiple Sclerosis and want to make sure it is not related to this since the pain came on a week later. Please help!! I do not have dental insurance so I don’t want waste a trip to the dentist if I don’t need to. This also has affected my taste of things.

Q. Hi i’ve got a darkened front tooth, that came out about 22 yrs ago, i pushed it back in myself, it has started darkening over a period of some yrs and is now looking bad especially in photographs where it shows up very bad i don’t like smilling for photos anymore. My last visit to my dentist told me that it was a lack of oxygen getting to the tooth that is the problem but he would not take it out or crown it as it was still healthy otherwise. i would love to get it capped or have a veneer over it but how can i go about getting it done, privately would seem the answer but the cost is off putting, are there any other alternatives to this ? how can i persuade my dentist to do the work for me ? any advice would be good
A. the nerve will have died during the trauma. first you remove clean and seal (root canal therapy) then an opaque veneer. private fees guide price 500-600
