My new night guard are too soft and now i am stuck as to who to get in touch

Q. I have been to my local hospital for a re-placement night guard as I have TMJ. They have now told me they dont do them anymore. Basidon Essex Hospital!! Ive since had one made at a dentist but it is too soft and my GP said it is not doing the Job. I am stuck now as to who to get in-touch with as I need one urgent and that fits and does the job right for me, I am in quite a lot for pain and dont want my jaw getting locked again and ending up at hospital.

A. Go back to your dentist and see if they will ake you a hard bite raising appliance, maybe take your old one in to show them what they need to make

Q. Hello Sir, I have got severe TMJ problems, have tried everything from mouthguards to bio feedback systems with no results, Can you please advice where I could get botox treatment from and would it be of any help..Thankyou

A. Hi
the botox would stop the masseter muscle from contracting and cause atrophy hence reducing the jaw pain.
i would do a web search for an experienced practitioner who deals with this aspect as not all do, and shop around for prices

Could the problems with my bite be due to TMJ?

Q. My question is what would cause my teeth to feel as if my bite has changed. I am 40+. I see my dentist for reqular yearly checkups. In January 09 I got a cleaning done, and at that time I told the hygienist that my right side teeth feel different as if they are longer or that my bite is off. She said sometimes tartar is like a sliver and makes things feel funny. Had bite wings and only showed a couple of watches, no cavities. Well a month later they were still feeling like my bite is off, I went back to the dentist and he said that I probably am grinding and need a night quard. So I booked an appointment for the night guard, and was seen by another dentist this time. I told him my teeth still feel like they are not feeling like they bite together properly, in fact I noticed that before I was able to bite off a broken finger nail and now I cannot because of the space. He said a night guard would not help and wasn’t at all sure what to do ?What would cause a shift like this or what could be happening? Could it be my TMJ?

A. It is likely to be due to the TM Joint so a bite guard would be the first thing to try to realign your bite

I am 16 years old, i have severe TMJ, i was just wondeing if there was any operation or anything with my teeth that could help

Q. I am 16 years old, i have severe TMJ, and i have done for 4 years now. I have tried accupunture, botox, tablets and at the moment i am on2 15/100 co-codamol 4x a day, 1 dicfolenac 4x a day and 1 amitriptyline a day! I am also having physio and seeing a pyscologist, i was just wondeing if there was any operation or anything with my teeth that could help. As i’m sick of all these tablets and things not working. Thank you

A. Hi  It sounds like you are having all the appropriate treatment however, are you wearing a hard bite raising appliance, this is the best treatment for TMJ disorders.

I have had TMJ for many years

Q. I have had TMJ for many years, and the last three years I have suffered with a displaced disc which meant I had chronic facial pain and the disc used to become inflamed quite regularly. My dentist recently gave me a splint to wear 24/7 for about 8 weeks which helped with the facial pain but did nothing for the disc. I then started to experience terrible headaches with the splint so the last time I saw my detist he said he could do no more for me and sent me to a specialist in London. This specialist would like to perform open surgery to re-attach the disc and then adjust my bite. Another specialist has said I could have an arthroscopy. Is it possible you could tell me which would be best please? I was hoping the disc might go back without surgery but it looks like it might not be the case. Many thanks

I do a lot of work with patients suffering with the symptoms you have. I also beleive that surgery is a last resort.
At my clinic the type of TMJ treatment offered is via Neuromuscular dentistry. I would recommed coming in for a consultation to see if this method of treatment can help you before considering any surgery.

im a 24 years old female doctor working in Pakistan

Q. Hello doctor, i’m a 24 years old female doctor working in Pakistan. My problem is the protrusion of front incisors of my upper jaw. Xray of my upper jaw shows compactness of molar teeth behind. i want a non extraction treatment done. the protrusion makes my fascial profile and smile get affected badly, making me under confident. Kindly guide me to solution of my problem. I really need help. Hoping for a favorable response. Regards.

A. Using arch expansion techniques ayour teeth may be able to be moved into a favourabel position.  You need to have a proper assessment by an orthodontist to see what your possibilities are.

do you know anyone that offers botox for tmj treatment

Q. I am currently under the care of a facial surgeon who has referred me for physio & accupuncture twice a week for TMJ problems. I also wear a mouthguard at night for bruxism. My symptoms are very painful ears (particularly on the right side) temple headaches with accompanying thickening of this area, facial muscle pain, neck, back & teeth pain. My physio/ has suggested botox as the physio has only been successful if I keep going twice a week, as soon as I try and cut down my visits my symptoms return with a vengence. Do you or do you know of anyone that treats TMJ with Botox in the U.K? I have also been treated with muscle relaxing drugs in the past.

A. I do not recommend injecting botox to relieve you of the pain because instead of targeting the problem, you are just masking it and the problem is still going on and possibly getting worse.

I see patients every day with the symptoms that you are suffering from. A lot have been through the same procedures as yourself, standard mouthguards, botox and some even resulting to surgery, all being unsuccessfull. I practice a type of dentistry called Neuromuscular Dentistry. My jawline is misaligned and I cant open my mouth wide what can be done to fix this?

Q. My jawline seems to be misaligned or broken on the left side of my face. It makes my face look asymmetrical, and my left eye is also protruding, i also have problems opening my mouth really wide. What can i do to get this fixed?

 

A. The complaints you mention could be as a result of a number of different factors and as you can imagine it is impossible for me to diagnose this from the brief description you have provided.

The restriction in jaw opening can be as a result of Jaw joint (TMJ) problems which can be helped via wearing a certain type of appliance on the lower teeth to relax the jaw muscles and allowing a greater range of movement.

Regarding the jaw line and eye issues only a full and detailed examination will determine if these issues are related to TMJ and whether they can be improved or not.
