I hv pain on my right jaw plz help

Q. i have been experiencing pain on my right jaw for a week now and giving me headaches. painful when i open my mouth to yawn or eat. My right ear is starting to ache too. i am currently wearing retainers up and down for 2 months now after my braces.how will dis stop? please help..tnx..

My daughter has lax joints in knees and elbows due to tmj plz advice

Q. My 13 year old daughter has had tmj for 2-3yrs has been seen at Eastman dental hospital and has tried night splint which made matters worse, follows advice and exercises.Has had physio but physio used acupuncture needles which after treatment caused her to faint and black out next day causing a visit to accident and emergency.Problem has taken over her life. Pain and spasm in right jaw considerable and jaw sees very lax. She is sporty but has lax joints in knees and elbows. She takes regular pain killers and I massage gently but all to no avail. What do you recommend?

A. Ask your dentist to refer you to his hygienist to clean off as much of this staining as possible and then try some teeth whitening.

what can I do to get the worn teeth repaired and what would be the cost?

Q. I have very worn teeth where they meet the gum line. My own dentist does not seem interested in doing anything about it or referring me to someone who can. It is very noticeable when I smile plus I am worried they are just getting worse. I would rather avoid having veneers if possible but have heard that white filling can be used. So my question really is what can I do to get the worn teeth repaired and how much would it cost?

A. If your teeth have worn so far down that they are at gum level, composite is not they way forward for you. You would just grinde the composite straight back off, as you would do with veneers or crowns. Is my jaw pain unconnected to the maryland bridge?

Q. Hi, My upper right canine tooth grew in the wrong direction in my teens and as a result this and my upper right incisor had to be removed. I had a brace on my upper teeth for about 18 months and at 15 was fitted with a maryland bridge between my central incisor and 1st premolar. I was advised this would last for about 10 years and then would then need some sort of perminant replacement. I am now 25 and still have the maryland bridge. Unfortunately this has forced my bite out of line and causes me to grind and clentch my jaw creating swelling and pain in my temporomandibular joint. I have seen several different orthodontists who have recomendended differnt things – I was also saw a TMJ specialist who suggested an £800 head scan and wearing a splint at night, he also said my jaw pain had nothing to do with my teeth at all. Is my jaw pain unconnected to the maryland bridge? And if so would it be better to have the maryland bridge replaced with a more modern appliance or try and sort out the jaw problem first?

A. The headaches could be as a result of the Bridge if it is of a bulky design but can also be due to many other factors such as stress, poor bite on your back teeth, actual migraines etc.

It may be worth asking your GP to refer you for a TMJ MRI scan to see if there is any damage to the jaw joints themselves.

If deciding to go for a new bridge it maybe best to go for a temporary bridge for a period of time to ensure it feels comfortable before having the permanent bridge fitted

Would braces make the gum recession and TMJ worse?

Q. Hi, I am 27 and am considering braces since my bottom teeth and quite wonky. Although my teeth and gum themselves are healthy, for most of my life have had a clicking jaw joint. I have been using a mouthguard but it has done little to help me stop clenching my teeth. I also have sensitive teeth and some gum recession cause either by toothbrush abrasion or maybe the clenching of my teeth.I am looking for a dentist who may be able to help me with all these issues! Would I be able to have braces or would it make the gum recession and TMJ worse? If I get braces and my gum recession increases, is there a dentist who would be able to give me a gum graft to improve this? Kind regards

Will braces help me diagnose TMJ and realign my teeth?

Q. Hi, I was just very recently diagnosed to have TMJ disorder whoch I knew I had for more than years now! You see it took me a long while before a realize the change that this TMJ is causing me. My lower and upper teeth wont meet anymore and it scares me. Anyway, I just had a dentist to check it about a week ago and told me I need braces and splints. Ive read many articles that this has no assuarance,though. But, if braces do somehow help, my family’s willing to risk 50,000 Philippine peso! What do you think of it? Braces for Tmj, i mean.

Can u advice me on the best products/procedures available for whitening?

Q. Hi Rob, I am considering in-surgery whitening and wondered if you could advise on the best products/procedure available? (highest quality/results possible) How long it lasts? A good cost benchmark etc? My teeth are in good shape so i have ruled out veneers as i think whitening will give me the results i’m after. In particular i have heard of a “bio” laser treatment but have found very little info (I assume then that this is just the normal Zoom style treatment of gel + laser). A. Most dentists should know about TMJ disorders and how to treat them. You might want to find a local dentist with more experience in this problem as they might have more knowledge in how to treat it.

How is TMJ diagnosed? Do I see my dentist or a neurologist?

Q. I have been suffering from neck pain and vertigo symptoms (headaches, dizziness and nausea) since early May 2009. It was first thought that I had an inner ear infection however,due to the severity of neck and shoulder pain, it was thought that it may be delayed onset of whiplash following a car accident (rear collision) at the end of March. I have seen a physiotherapist and acupuncturist whose treatment of whiplash has proved ineffective and has not yielded longterm results. In the meantime I have developed a problem with my teeth; I cannot bite properly without making a loud squeaking noise. My jaw is tense and clicks. I pointed this out to my dentist when having a wisdom tooth removed and she agreed that this was not normal and said that she would refer me to a specialist. Having read up on this I am confident that I have TMJ, which has developed/been exacerbated by whiplash. My question to you is therefore this: how is TMJ diagnosed? Do I see my dentist or wait for a neurologist appointment?

A. TMJ is a very misunderstood condition, missed by many dentists and not really looked for by Neurologists.

It does sound like your symptoms are similar to TMJ which can be brought about by whiplash.
I would definitely see your dentist to determine how your bite has changed and see if you could benefit from wearing a splint on your teeth to relax the muscles.

Not only do I suffer from typical TMJ symptoms but my whole left side feels tight suggestions

Q. Not only do I suffer from typical tTMJ symptoms but I also have left flank pain,pressure under left rib,tingling in my left shoulder and along arm to fingers, pain just above my left knee at front, a feeling like a tight band around my upper left calf and pain in left achilles. In fact the whole of my left side feels tight. My right side is fine. Doctor says nothing wrong with me and all in my mind. I also have a tight throat and difficulty swallowing. Thanks

A. I certainly do not think it is in your mind as these symptoms are common with TMJ sufferers – many simply do not make the connection between these symptoms and TMJ.

The jaw relationship has a complex interrelationship with the C1/C2 vertebtrae and as such when the teeth bit in an improper alignment this whole balance is affected.
The results can be pain and asymmetries in posture and alignment eg. resulting in head tilt, raised shoulder, pelvic tilt and knee pain.

A study by Bergamini et al proved that if the bite is correctly aligned then EMG recordings recorded relaxation in the soleus muscles of the calf – sounds familiar.

I would advise you to have EMG recordings of your jaw and neck muscles to detect any imbalances which I am suree you will have.

My new night guard are too soft and now i am stuck as to who to get in touch

Q. I have been to my local hospital for a re-placement night guard as I have TMJ. They have now told me they dont do them anymore. Basidon Essex Hospital!! Ive since had one made at a dentist but it is too soft and my GP said it is not doing the Job. I am stuck now as to who to get in-touch with as I need one urgent and that fits and does the job right for me, I am in quite a lot for pain and dont want my jaw getting locked again and ending up at hospital.

A. Go back to your dentist and see if they will ake you a hard bite raising appliance, maybe take your old one in to show them what they need to make

the botox would stop the masseter muscle from contracting and cause atrophy hence reducing the jaw pain.
i would do a web search for an experienced practitioner who deals with this aspect as not all do, and shop around for prices

Could the problems with my bite be due to TMJ?

Q. My question is what would cause my teeth to feel as if my bite has changed. I am 40+. I see my dentist for reqular yearly checkups. In January 09 I got a cleaning done, and at that time I told the hygienist that my right side teeth feel different as if they are longer or that my bite is off. She said sometimes tartar is like a sliver and makes things feel funny. Had bite wings and only showed a couple of watches, no cavities. Well a month later they were still feeling like my bite is off, I went back to the dentist and he said that I probably am grinding and need a night quard. So I booked an appointment for the night guard, and was seen by another dentist this time. I told him my teeth still feel like they are not feeling like they bite together properly, in fact I noticed that before I was able to bite off a broken finger nail and now I cannot because of the space. He said a night guard would not help and wasn’t at all sure what to do ?What would cause a shift like this or what could be happening? Could it be my TMJ?

A. It is likely to be due to the TM Joint so a bite guard would be the first thing to try to realign your bite

I have tried accupunture, botox, tablets and at the moment i am on2 15/100 co-codamol 4x a day, 1 dicfolenac 4x a day and 1 amitriptyline a day! I am also having physio and seeing a pyscologist, i was just wondeing if there was any operation or anything with my teeth that could help. As i’m sick of all these tablets and things not working. A. Hi  It sounds like you are having all the appropriate treatment however, are you wearing a hard bite raising appliance, this is the best treatment for TMJ disorders.

I have had TMJ for many years

Q. I have had TMJ for many years, and the last three years I have suffered with a displaced disc which meant I had chronic facial pain and the disc used to become inflamed quite regularly. My dentist recently gave me a splint to wear 24/7 for about 8 weeks which helped with the facial pain but did nothing for the disc. I then started to experience terrible headaches with the splint so the last time I saw my detist he said he could do no more for me and sent me to a specialist in London. This specialist would like to perform open surgery to re-attach the disc and then adjust my bite. Another specialist has said I could have an arthroscopy. Is it possible you could tell me which would be best please? I was hoping the disc might go back without surgery but it looks like it might not be the case. Many thanks

I do a lot of work with patients suffering with the symptoms you have. I also beleive that surgery is a last resort.

do you know anyone that offers botox for tmj treatment

Q. I am currently under the care of a facial surgeon who has referred me for physio & accupuncture twice a week for TMJ problems. I also wear a mouthguard at night for bruxism. My symptoms are very painful ears (particularly on the right side) temple headaches with accompanying thickening of this area, facial muscle pain, neck, back & teeth pain. My physio/ has suggested botox as the physio has only been successful if I keep going twice a week, as soon as I try and cut down my visits my symptoms return with a vengence. Do you or do you know of anyone that treats TMJ with Botox in the U.K? I have also been treated with muscle relaxing drugs in the past.

A. I do not recommend injecting botox to relieve you of the pain because instead of targeting the problem, you are just masking it and the problem is still going on and possibly getting worse.

I see patients every day with the symptoms that you are suffering from. A lot have been through the same procedures as yourself, standard mouthguards, botox and some even resulting to surgery, all being unsuccessfull.
