Best option for tooth grinding problem

Q. Hi I have a tooth grinding problem which has cracked a bottom tooth. My dentist has repaired this twice with a small filling but it keeps coming off. I went to my practice last week (met a new dentist) who suggested having a veneer which was ‘really tough’ and would not come off. I was quoted £700. However, my previous dentist said a veneer would not hold and he wouldn’t sell me one, he suggested a night guard £400, but I am not convinced I ‘grind’ at night I think it is when I chew food. My dentist raised the height of a rear filling to stop me grinding and now I am having trouble chewing food. I therefore have three expensive options (veneer, night guard or constant fillings) and have no idea what to opt for. Can anyone help? Regards

My bottom row of teeth is touches the inside row of teeth advice

Q. Hi, Ive recently been to the dentist and have always had good teeth with no pain or alignment issues. But during my last visit my dentist said that my bottom row of teeth is making contact with the inside of my top row of teeth at the front. There has been some minor wear and my dentist says over time it will get to the point where the top teeth could break. Is there any treatment that is available for me and is it at all likely that my teeth could break over time? Thank you

A. Hi
at this early stage a splint to wear at night if you are grinding your teeth , or a more drastic step of building the teeth back up with white filling to restore the bite but this is not always easy

its what we diagnose uptight neurotics with generally, but there are certain patients who experiance this due to stress and grinding of teeth or if they have had a lot of restorative work done at once

I have venners on my teeth and I grind. Do I need night guard?

Q. I have venners on my front four teeth I grind and clinch in my sleep but I dont know how much? Do I get Nightguard for the top, Bottom, Both, or what? Should the guard be Soft or Hard The Reason I’m asking is because I already spent a great deal on my veneers and whitning and I dont want to spend $800 Dollars on a niteguard.

A. I think you need to get your dentist to check how your teeth bite together and make sure they are biting evenly.  I would recommend an upper hard niteguard.

Is there a way o stop grinding? What are the options if crowns cannot be refitted?

Q. Hi, I am in my early 20’s and had 8 porcelain crowns (costing a small fortune!) on my upper front teeth about 4 months ago. The reason for this was mainly cosmetic as I grind my teeth at night and had severely damaged them over the years. My bite is ‘out’ (so I am told) therefore could not have veneers. The problem I have now is after originally adjusting my bite, I am now continuing to grind my teeth at night. I do wear a guard for this but can still notice the edges of the crowns becoming flattened and worry that they will not last me the year! My question is what should I do now? is there a way of putting a stop to my teeth-grinding? how many times can crowns be refitted and what are the options when crowns can not be refitted…false teeth? I would be extremely grateful for your help.

A. Hi many thanks for your enquiry, firstly, unfortunately no one can stop you grinding, this is a subconscious thing which occurs while you sleep. the second thing to note is if you are grinding the porcelain on the crowns is much tougher than the opposing natural teeth so should not wear, if anything the night guard should wear down. if you are really concerned you could wear a night guard over your crowns. crowns can be replaced many times the dentist just has to be careful to not remove more tooth tissue. if there ever can a stage when the crowns could not be replaced then there is always dental implants. hope all this information is helpful.

Headaches and facial pain is due to grinding in my teeth

Q. Hi, I have recently been advised that I am suffering from headaches, and facial pain as a result of grinding my teeth at night, and clenching my jaw when anxious. My dentist advised me that I should purchase a special guard to protect my teeth-but that it would cost me £198. I am a Full time student and cannot afford to buy this at that price. I have been told that if you go via your Dr you can sometimes get this for free, is this true, or is there a cheaper alternative? Please help!

A. I do not know of doctors providing this treatment as this is usually provided by dentists.  A hard night time appliance properly made should reduce your muscle spasms.  This is a very exact appliance, hence the costs.  I obviously understand you are a student .

My teeth are wearing away I assume because of night grinding. Is there anything dental surgery or wizardry that can help?

Q. Hello, I am 38 and my teeth (I assume because of night grinding) have been steadily wearing away. They are now all much smaller than they should be which really reflects in my smile. I no longer have incisors as they are the same height as the rest of my teeth. Is there anything dental surgery or wizardry that can help? Many thanks 

A. There certainly is!!.  This is quite common in this day and age due to more people keeping their teeth for longer.  Very often the treatment involves increasing the length of the teeth in the upper /lower or both to rebuild the lost tooth height and to improve the function of the teeth.  Once this is done it is usual to give a night guard to protect the new restorations from damaging night time grinding.  Pleae ring my practice to make a free cosmetic consultation appointment and I will be able to discuss this further with you.

I grind my teeth, which is better? Crown or Implant

Q. Hi there Over about 20 years I have had endless fillings in my lower 5th tooth back from the middle which whilst flossing 2 months ago finally disintigrated leaving enough route apparently for route canal treatment and a crown according to two detists. However the first private dentist advised a dental implant instead of a crown due to the fact I grind my teeth fairly badly and said that a crown wouldn’t last. The NHS dentist which I have just seen knew nothing about implants and simply said she could do route canal and a crown, not really mentioning that it may not last due to grinding. So bearing in mind that I could afford an implant, am I better to just bite the bullet and get an implant done as opposed to a crown which according to the private dentist I may have to replace in a couple of years? Or is the private dentist just after more money? In addition am I better to go to a special centre that specialises in implants or register with a private dentist that does them? Also if I went the route canal and crown route someone said that nowadays they can fit very slim night mouth guards to protect teeth from night grinding and if so is this true because I had one about 20 years ago and felt like a gorilla in bed!! Thanks for your help

A. If you have enough root above the gum line( approx 2mm) to allow the crown to hold onto the root as well as the post that the dentist will put into the tooth the crown should be ok.  If the root is at or below gum level then there is and increased chance of it failing.  If this is the case an implant may be the best solution.  Provided there is plenty of bone in the area then a private dentist with the correct training is likely to be able to successfully place an implant.  If you grind your teeth at night  which ever option you choose a night time appliance would be recommended.

There are hairline cracks in the veneers and crowns on my teeth and the dentist says that it is due to grinding

Q. I had veneers and crowns applied to the top 6 of my teeth 5 months ago. My two front teeth are actually crowns. I went in for a cleaning today and the dentist advised me that I have 2 hairline cracks. One on my front (crown) and one on a side veneer. He believes that I’m grinding my teeth at night and that is what caused this. I don’t think I grind my teeth at night. Isn’t it awfully early for me to be already getting cracks in the veneers/crowns? He is replacing both for free but does not know how well they will match the remaining 4 when complete and wants me to start wearing a mouthgard at night.

A. There is a huge misconception about grinding. Actually very few people grind thier teeth at night. If you do do then you have something called para function which meand the condition is habitual or you have a neurological disorder.
 In both cases if this was the reason then your teeth probably would have exhibited signs of wear before the veneers were placed and you should have been advised of fracture of veneers before hand.
 Also you would definately see some signs of wear on your back teeth. You may have muscle or joint problems. If you had none of these signs or symptoms then you are not a grinder. Without seeing you it is difficult to tell the cause of the wear but usually the reason for vertical fractures of anterior veneers is increased pressure on the teeth. Studies have shown that this is happens during chewing. The constant knocking of the lower teeth against the top teeth causes the bond to fail and this in turn causes the veneer to fracture because without the bond the veneer is extremely weak. This is all backed up by resaerch and studies. If your dentist places the new veneers in the same position as the old ones then the same thing will happen. It won’t make any difference if you wear a mouth guard or not.
Either your veneers were too long or they were locking your lower jaw back, or the palatal incline on them on them is too steep.

one of my veneers has come off and one is chipped

Q. Dear Dr. Glaister, I had veneers 4 years ago. One fell off in August 2008. Last night whilst talking on the phone one of them chipped leaving approximately half of the veneer remaining. This morning whilst brushing my teeth, a veneer on a different tooth also chipped. My Dentist (who’s a friend) is happy to carry out the remedial works today at a discounted rate but I’m very concerned that it may be something he’s not doing properly. I’ve not bumped my mouth nor eaten anything particularly veneer-breaking. Before I call him I wanted to seek advice so I do look forward to hearing from you at your earliest convenience. I really do hope you can help, I won’t smile until you do :0) Many thanks

It may well be that you are grinding your teeth at night and creating too much pressure against the veneers

At the very least I would recommend you wear a nightguard to protect the replacement veneers you are having fitted but without examining you it is impossible to say with any certainty this is what is happening

A. Hi Anyone who grinds their teeth is more likely to have a veneer pop off (debond). With this in mind if you have any type of veneer you should wear a night time guard to protect your new smile

Q. My one front tooth has lately started to push outwards , otherwise all my teeth are straight and fine , i want to know what the reason for this might be…? and what will be able to fix this!? i dont want braces! all i think the reason can be is that i grind my teeth? please some advice!!

A. Hi,

 there can be multiple causes for this – biting habits, gum disease, wisdom teeth impaction.avoiding braces is tricky there are removable braces like the inman aligner that will move the tooth relatively quickly or the invisible brace options .
tooth grinding is unlikely to influence just one tooth, feel free to have a free consult if you like

i used to grind my teeth

I grind my teeth very badly and wake up with aching teeth

Q. I grind my teeth very badly, and wake up with aching teeth. It has been suggested i get my self a bite plate for the night-time. Could you advise me where i can get one of these, without necessarily going through my dentist Kind regards

A. I’m not sure whether you mean you do not want to go to your dentist ot any dentist. If you mean that you do not want to go through any dentist then a company called oprp offer mouthguard that you can order online although they are mainly for sports reaons.
If you meant not necessarily with your dentsit then most other dentists will be able to have a mouthguard made for you.

I do recommend looking into the reason why you are grinding. Beacause although a night guard will protect the teeth at night, you are not getting to the route of the problem, but willl simply be grinding onto the nightguard instead of your teeth. I am a neuromuscular denist and deal with patients like yourself every day. Neuromuscular dentistry looks into the alignment of the bite. Some of my patients also have headached, neck and back pain and other various symptoms

I suffer from bruxism and have ground down my incisors

Q. Hello Dr Solanki, I suffer from bruxism and have ground down my incisors quite a bit. My NHS dentist seems to be unconcerned with this, but I would like them repairing as I am quite concious of how they look. I’m wondering, can this be done and how much would it cost to be done privately? Thanks.

A. Hi many thanks for your enquiry. if would be very easy to repair these worn down teeth, but also we need to consider treating your grinding issues with a stabilation splint. hope this is helpful. the cost could range from £1000 – £3000 dependant on what is needed.

I grind my teeth and have acid erosion and need crowns

Q. I have bad acid erosion on my teeth I also grind them in my sleep. I have two front crowns and will soon need the other teeth around them done the crowns are really horrible and my teeth are quite yellow. can you help me please.

 

A. The acid erosion on your teeth and grinding is one area which we need to control. the grinding is something dentists cant stop you doing but we can prescribed night guards which means if you are going to grind at least you grind the guard away as apposed to your teeth. the crowns can be replaced for more aesthetic crowns using all porcelain material. however first you could have some whitening to improve the natural colour of your teeth and then replace the crowns to match the new colour. hope all this is helpful.my advice would be to seek the help of a cosmetic dentist. if i can help please email back.

I look toothless when I smile

Q. Bit of a strange question-When i smile you can only see the very tips of my teeth and it sometimes looks like i\’m toothless, i am wondering if tooth bonding to lengthen my teeth slightly, because some of my teeth are slightly position higher than others would work or could you please suggest a device that could make my teeth more visable, please could you help i am tired of just living with it, thankyou so much i would be so grateful if you replied.

 

A. As life goes on peoples teeth naturally wear. Some peoples teeth wear more than others due to the way their teeth work and whether they grind/clench their teeth. Lengthening your teeth may restore this lost tooth structure but needs to be done very carefully otherwise the teeth will quickly chip. Your grinding pattern needs to be adjusted and may require you to wear a night guard to protect your newly lengthened teeth.

In my practice I have successfully treated many worn cases. I have had extensive training in America at the Dawson center to specifically treat such cases. Which ever dentist you decide to help you please make sure they have appropriate training in occlusion to allow your problems to be successfully treated. I would urge you to find a dentist sooner rather than later as your problem will only worsen.

My front teeth are short due to grinding and biting my nail

Q. My two bottom front teeth are very short, I think due to a habit of gritting my teeth (which I still do) and from biting my nails when I was younger. They are not unsightly and I do not have any problems with them but I wonder if they should be protected to prevent decay?

 

A. Very difficult to answer without carrying a detailed clinical examination.

If the wear is getting worse quickly then I would have to diagnose what is causing the wear. When I see patient in your same situation, we either try to stop the habit or ask them to wear a nightguard.

what can go wrong with dental veneers

Q. I have just visited a private dentist who told me that veneers would last 15-20years and cost about 7k, is this correct? After this time do the veneers fall off and what will my teeth look like then? Should I visit more than one dentist to gain a 2nd opinion? What can go wrong with this procedure.

 

A. The veneers would not come off in 15-20 yrs but may need to be renewed given that you will be biting and chewing for 15yrs it stands to reason that there may be some wear and tear.

I would always get a second opinion especially if you grind your teeth or suffer from headaches and jaw pain

I Have bad teeth gum disease wear and grinding TMJ problems

Q. Hi. I’ve very self conscious about my smile. I’ve always had problems with my teeth over the years. When I was younger I had many teeth out and they were never replaced. Since the age of 4 I have grinded my teeth with has caused wear on my teeth. Within the last year I tempted to visit the dentist again and he confirmed I had gum disease. I had treatment for this which was quite painful. The reason I went to the dentist is because I wanted my teeth straight as my bite is out of alignment and I thought it would help me with the grinding. The dentist told me I couldn’t have a brace as my teeth would probably fall out. I’m not sure now what to do, I’m very self conscious about what I look like and my two front teeth protrude. Also my jaw clicks so I think I might have TMJ. I have a real fear of the dentist as the ones I have been to have made me feel bad and haven’t been able to give me options to straighten my teeth. Do you have any advice?

 

A. I am so sorry to hear about your distress. The first thing you must do is be proud that you took that first step of visiting the dentist I know how difficult that can be. I am sorry the experience you got was not really what you wanted. I think you need to find a dentist who will listen to you and put you at ease so you can move forward. If your bone loss/ gums are so bad that orthodontics is not possible there are many other options that we could look at. I know that there is a solution for you. We need to address the jaw problems, gum disease and alignment of your teeth and you need to have a detailed consultation.
