What is the procedure for tooth extraction?

Q. approx 5 yrs ago I had a root canal and crown put on my lower “canine” tooth. I was referred to an endodontist for re-treatment root canal and he diagnosed it as extra canal invasive resorption and chronic apical periodontis. He advised that I have the tooth extracted because I would eventually lose it and get an implant. How would the extraction be done? Would they have to cut the crown off first then extract the tooth, or can they extract the tooth with the crown on it? Thank you,
A. The tooth will probably be removed with the crown attached

Why is tooth next to one that was extracted wobbly?

Q. Hi. About a month ago I had to have an extraction carried out on my second to back tooth. It went smoothly and I stuffed no pain from it, but going to the dental hygenist two weeks later, he pointed out that the tooth next to it (the third from the back) is slightly wobbly. I hadn’t noticed this before, but a week later I checked and found it moved slightly back and fourth when I applied pressure to it. I phoned the denist and explained the problem, and he said it was normal for this to happen and the jaw should begin to tighten up eventually, but after a month it is still wobbly and I’m starting to get really scared that it will end up falling out despite reasurrances that this is natural. Am I worrying unduly and should I give it time to settle (and if so, how long?), or should I look into it further? I’m really worried and I think the constant fear is making me imagine other dental ailments. The tooth does seem to hurt sometimes though, so what would be my best option? Any advice would be gratefully welcomed.,
A. It might to loose as a result of the condition of the other tooth before it was removed as is there could have been bone loss in the area. It may tighten but this will  normally happen after 6 weeks so pop to your dentist and as him to take a look perhaps with an x-ray

Should I get infected tooth removed or not?

Q. I have on the right lower side the tooth next to the wisdom tooth that is little loose,and the dentist indicated that in the future IS GOING TO BE EXSTRACTED. Ihad some infection on the gum and some work done it about NINE months ago.Ican eat, no pain, if ghet worse I would consider a FIXED BRIDGE….. Should I remove the TOOTH and have a Bridge with two porcellain crowns, or continue until I have problem,infection etc.or other complications. Also in your opinion the material CERAMIC or PORCELLAIN FUSED WITH METAL? In this condition I can manage well.. Can the situation of the loose tooth generate other problem in other TEETH. Tank you please for your professional advice,
A. I would consider removing the tooth before an infection takes hold the think about an implant instead of a bridge as it sounds like the wisdom tooth might not be the best anchor for a fixed bridge

Teeth are to be extracted – what replacement options are there?

Q. HI IM A 30 YEAR OLD WOMAN AND MY TEETH ARE IN QUITE APOOR STATE. FIRSTLY I HAVE HAD SOME DENTEL WORK AS A CHILD AS MY MOUTH AND GUMS ARE NOT BIG ENOUGH TO HOLD A FULL SET OF TEETH. MY MAIN PROBLEM IS THAT MY TEETH HAVE SUFFERED THROUGH TEN YEARS OF HERION USE. MY TEETH ARE ROTTING FROM THE INSIDE OUT BECAUSE OF THIS AND NOW IS BEGINING TO SHOW AT THE TOPS OF MY TEETH NEAR MY GUMS. I am unhappy with results following removal of four bicuspids

Q. Dear doctor, Warm wishes, I have been wearing my fixed ceramic braces for the past two years now. My orthodontist advised me to remove my four bicuspids. After a long thought of nearly about 6 months, i finally removed them. But now i feel my nose looks bigger and my teeth have gone in too much. and i have lost the wide smile that i used to have without removing the four bicuspids. The gaps have not closed yet. I am afraid, my nose would look even more bigger as the gaps close. Can i ask my orthodontist to push my teeth little forward and fix artificial four bicuspids to have a wide smile as before. A. I agree if you feel you would like a wider smile the only option would be to look at having those teeth put back in.

Will I need to pay for broken teeth removale before getting braces?

Q. my teeth are broken back teeth, some back teeth have completely come out, this happened during my three pregnancies, i want to consider a brace as my fronth teeh is slightly more forward than the next one and also i have a upper tooth slightly behind the rest of my teeth, if that makes sense, wcould i just go for the brace or will i need to seek attention for the broken teeth? which i had done so but its very costly, id rather just pay for the brace, as im very self conscious.,
A. hi you will need to have the broken teeth removed or repaired before you have the braces as active decay or gum problems can lead to more decay when you are having braces

Teeth are loose and discoloured

Q. I am 66 years old and have poor teeth. My lower front teeth are loose and discoloured. I also have gum disease and I think before long the lower front teeth will need to be removed. I want to look at new teeth but do not know much about it. Dentures scare me somewhat and I wonder if some implants and whitening of the few good teeth I have would be the answer. I would appreciate your advice. I live in Portugal but come to the UK fairly often. How long would any recommended treatment take. Many thanks.,

If the remaining teeth are not too good I might suggest to remove them and provide you with an implant supported prosthesis – this can be a removable or permanently fixed bridge.  Please make an appointment with my practice when you are next in the UK – please ask for a proper examination and not just the cosmetic consultation.  I look forward to meeting and helping you.

Four year old with abcus

Is it possible to have implants where I had 2 of my teeth extracted?

I want to have 2 possibly 3 dental implants these are to fill a gap where i had 2 back teeth extracted many years ago,the remainder of my teeth are excellent and i have only 1 filling in a back tooth on the other side of my mouth.can you fore see any problems arising,also can you advise me of the costs for the treatment. Thank you,

A. It is difficult to give you exact ideas without seeing you.  If the space is in the bottom jaw then it really just depends on how much bone is available for the implants.  If it is the top jaw it can be more complicated as there is a sinus(pocket of air)  in the back of your mouth.  This can mean you may need a bone grafting (adding) treatment first.  Make sure you find a good cosmetic / implant dentist who can carry out these procedures.  You may not need 3 implants and could even have 2 implants and have 3 teeth supported from them.

Could you advise on whether I will be able to straighten teeth after implants?

Q. I have been advised that I a lower molar needs to be extracted and replaced with an implant. Can you please advise whether I would still be able to wear braces to straighten the rest of my teeth when I have this implant? I mainly need to straigthen the upper teeth but have been advised that the lower teeth also need to be straightened to correct my bite.

Should I had to wait six months before a bridge was fitted after tooth extraction?

Q. I am due to have my bottom front tooth extracted due to a snapped root, do you have to wait six months before a bridge can be fitted as i am having mine fitted on the same day and i am feeling very unsure that i should be fitted with a denture first

A. During the first six months after an extraction the bone will reshape. A denture is easier to adapt to this change. a bridge fitted after six months is likely to easier to adapt to the stable shape. bridges fitted at extraction sometimes need replacing after six months. Rob

I have trouble with dentures can I have alveoplasty after my teeth were pulled?

Q. I had my teeth pulled about two and half months ago and been having trouble with dentures fitting. now they are saying i need alveoplasty ok. today they sent me to an oral surgeon he stated that i should of had the procedure done at the time my teeth were pulled. i would like your opionon please

A. It is not unusual for dentures not to fit very well following removal of teth an immediate placement of dentures.  Many times new dentures need to be made 6 months later following a healing period. I am unable to comment on the alveoplasty issue without examining you.

Can the process of tooth extraction be quicker?

Q. I went to my dentist (NHS)for the second time re. my wisdom tooth. I was given antibiotics and she drained some of the infection. I asked to be referred to a specialist for extracion. This takes up to 6 months. I hate taking antibiotics and would rather have the tooth out asap. Can this process happen quicker i.e through private practice? What is the wait for an operation when going private and where could i go? I don’t understand why she seems reluctant to refer me and get it treated properly. I live in London. Best Wishes

What is the price range for implant?

Q. Dr. Caplan, I saw an endodontist today for a problem with a crown that I had placed in 2004. There was a root canal done on this tooth(which is the 3rd tooth from the front tooth) and then the crown was placed. But I have had a lot of trouble with the crown—has come off several times and now for the second time in 3 mo, I have decay built up under it. I am on antibiotic now and the specialist I saw today recommends that I have this tooth extracted and have an implant for this tooth. It was a baby tooth(the permanent tooth came in behind it and I had it pulled years ago). I have just a couple of questions: What is the price range of an implant? Should I have my dentist—whom I trust do the procedure or is it recommended to go to an oral surgeon? Thank you!

A. If possible I would recommend the dentist who is placing the implant to restore as well.  If your dentist has suficient ability to do both this is most likely to produce the best outcome.  Pleae siscuss the cost of this procedure with your dentist as the costs vary greatlydepending on the case

Are there any procedures to bring back my original shape of face?

Q. When i was in my teens i had a fixed brace to straighten my teeth, teeth were also removed, This totally changed the shape of my face my top lip/chin look empty even though i only had about 4 teeth out, are there any proceedures which could either bring teeth back out even if it meant wearing another brace, i dont really want veneers, i wish id been asked before the brace as id rather have had crooked teeth than a changed face! please help!!

Hi many thanks for your email, generally the brace treatment which requires extracting teeth does sometimes make your jaws and smile very narrow. my advice would be to use an expander to widen the smile this would refill your face and smile and help to close the gap from what you was like to how you are now. go for a consultation with an cosmetic orthodontist who i am sure will discuss these options with you.

My daughter may need alveoplasty is its necessary?

Q. My daughter is having two wisdom teeth pulled and they have told her she may need alveoplasty and it is not covered by insurance. She will never put dentures where the wisdom teeth were so why would it be neccessary?

A. Hi msny thanks for your email, this procedure may be required as if the extractions are very surgical, the bone might be very irregular once the teeth have been taken out and the alveoplasty will help to contour the bone so it feels much more smooth and natural in the mouth. you can ask them not to do the procedure but if the surgeon then feels it will benefit your daughter long term its a choice only you can make.

The top two teeth are trying to come out Plz advice

Q. I have a 8 year old girl who had her top 4 teeth pulled out due to decay when she was 3 years old.They are trying to come out , but the top two middle ones are protuding on top of the gums,not the bottom. Wht should we do?

My filling causing pain What shall I do?

Q. I was told by my dentist I had a leaking filling. He has re-filled but my tooth is lose and I am still in alot of pain. I am unable to eat or drink as the hot or cold causes me pain. What shall I do

Hi many thanks for your email, if the tooth in question which was filled is loose then it is safe to say there is bone loss around the tooth and the tooth is infected probably from the bone loss and gum disease around the tooth. my advice will probably consider extraction of the tooth as i think root canal treatment may not work.

Should I need to wait for 6 months to treat my extracted tooth?

Q. Hi i recently extract a tooth,now i have this empty space, the dentist said that he can do it in 6 months.is it true or is it possible for you to do it before and how much will that cost? regards

A. It is best to leave the area to heal for at least 12 weeks unless you are going to have an implant in which case it is best to have implant placed 3-4 weeks after the extraction.
