<div class="bubbleBottomstrGreen"><strong>Q.</strong> Hi, My upper right canine tooth grew in the wrong direction in my teens and as a result this and my upper right incisor had to be removed. I had a brace on my upper teeth for about 18 months and at 15 was fitted with a maryland bridge between my central incisor and 1st premolar. I was advised this would last for about 10 years and then would then need some sort of perminant replacement. I am now 25 and still have the maryland bridge. Unfortunately this has forced my bite out of line and causes me to grind and clentch my jaw creating swelling and pain in my temporomandibular joint. I have seen several different orthodontists who have recomendended differnt things &#8211; I was also saw a TMJ specialist who suggested an &pound;800 head scan and wearing a splint at night, he also said my jaw pain had nothing to do with my teeth at all. Is my jaw pain unconnected to the maryland bridge? And if so would it be better to have the maryland bridge replaced with a more modern appliance or try and sort out the jaw problem first?</p>

<div class="bubbleBottomLeft"><strong>A.</strong> The headaches could be as a result of the Bridge if it is of a bulky design but can also be due to many other factors such as stress, poor bite on your back teeth, actual migraines etc.</p>
<p>It may be worth asking your GP to refer you for a TMJ MRI scan to see if there is any damage to the jaw joints themselves.</p>
<p>If deciding to go for a new bridge it maybe best to go for a temporary bridge for a period of time to ensure it feels comfortable before having the permanent bridge fitted</p>

<div class="bubbleBottomLeft"><strong>A.</strong> The T Scan is an excellent tool for the determination of the level of force being placed on teeth and where exactly that force is being directed.  It allows for the determination of areas of interferences between the bite. However as i mentioned is it simply that &#8211; a tool. There are many other techniques, perhaps not labelled as being the most modern in our digital age, but many others, which are just as effective.  Lets not forget that fillings are done day in , day out and at the end of it, a vast majority of patients do not have a problem with the bite!.</p>
<p>Each clinician is different, and it comes down to what works best in their hands. The labs we use are of the highest standard and quite frankly are able to provide restorations that do not need the T Scan to any real degree. Furthermore, the T scan only really becomes useful when you are working on a large number of teeth. In those situations i am more then likely to be reorganising the occlusion then conforming to it.  Additionally, the Cerec units themselves come with state of the art scanning of the bite, so again personally i have very little need for the T scan.</p>
<p>Essentially, an excellent step forward in bringing technology to dentistry but its not going to change the world.</p>
