my teeth are on Invisalign but will i have to wear retainers there after?

Q. Hi. I’m currently having my teeth reset via an Invisalign treatment which is due to be complete in Jan 2010. The focus is on rotating an upper incisor so that it aligns with the rest of my teeth. I’m told that I’ll need to wear retainers at the end of the treatment and am not sure which type to go for. My dentist has a preference for fixed retainers which need to be worn for at least five years though he suggests wearing them indefinitely. However feedback from his other Invisalign patients has been that there has been regression in the molars and pre-molars which has had an impact on the more visble incisors and canines. The alternative are trays much like the Invisalign trays I currently wear. Initially I was told that I’d need to wear these all day and night for six months and then just nights after that. However I’m told by other people with knowledage of such things that wearing them every night from Day One will be more than sufficient. This sounds fine but my concern is that the teeth will constantly be moving – regressing in the day and then being corrected every night… Also I’m concerned about breaking the retainers (which has happened with several of my Invisalign trays). Plus cleaning is a concern as I can never get my Invisalign trays 100% clean no matter how much I brush them. However the latter would be my preference as I could eat and drink whenever I wanted, clean and floss properly, not worry about getting food caught in fixed retainers, etc. Could you offer your advise and expertise in helping me choose. Also I would be very grateful if you could spell out all the pros and cons of both types of retainer, your patients’ experience and perhaps even any alternatives. Kindest regards Harp

A. Retention following orthodontics has varying views depending on who you speak to.  The permanent retainers are a good option because they guarantee the teeth to remain in their final position.  However cleaning can be an issue.  Removable retainers should be worn fro 1 to 2 months 24 hours a day whilst the teeth are held in their final position.  Following this the retainers should be worn at night time for a long time, some say for life.  As you point out this allows easy cleaning.  This is the preferred method in my practice but some patients prefer the permanent retainer option.  The choice is yours

Should i wear retainers at all the times?

Q. I just got my retainers and my orthodontist said i have to wear it day and night for 6 months, is that necessary? cant i only wear it on nights? because its hard getting used to it.

A. Follow the advice of your orthodontist – they know best

Can i wear my retainers only at nights?

Q. I just got my retainers and its really geting hard to ge used to especially with my tongue being placed between the retainers and was wondering if it could be possible just to wear the retainers at night instead of day and night, My othrodontist told me to wear it for 6 months, i actually went by 1 month without wearing the retainers due to the delay of my appointment and my teeth haven’t moved at all? is it possible to wear it just for nights?

A. It may be possible but pleae discuss this with your orthodontist as a relapse at this stage would be a dreadful pity

Can you gimme some information about expandable jaw retainer?

Q. Hi Dr. Moore, I,ve heard of a new type of expandable jaw retainer that you wear behind your teeth to widen a narrow smile. I’m very interested in learning more about this, but I’m having trouble finding any info on it. Is it just too new right now? If you know anything about this, I’ll take all the info/suggestions you have. How long do you think it would have to stay on? How much would it cost? What’s it called? Could any Dentist/Ortho do it? Also, are ALL dentist’s considered cosmetic dentist’s or should I look for one that specailizes in it. Thank You.

A. You should look for a dentist with experience in the field you are thinking of. These braces are a very traditional design and not new at all. Find a local Orthodontist and ask them for a consultation to see if its suitable.

Can u please advice on crooked teeth cause the adjacent teeth to move and become loose?

Q. A couple of years ago i got braces on only my upper two front teeth. And now one tooth is moving a little bit. I think it may be one of my other teeth causing it to move, because my other teeth are still crooked. Also, sometimes when i press down on the tooth it moves. Not like its loose, just like underneath. Like its trying to click into place. This is the only tooth that I’m having a problem with. If you could help me that would be awesome.

Go back to your orthodontist to discuss this as they will get worse, do you have retainers? If not make sure you get some.

what is the best treatment for class 2 overbite

Q. hello,ive been told that i have class 2 overbite which makes my face look smaller when close teeth which never do plus my bootom teeth point backwards which makes my bottom lip look bit sunken does not alaign with top which i would really like it to would really appreciate your advise of what to do thankyou

A. The best way to treat this problem is with braces, usually a removable brace followed by a fixed brace to reposition the teeth and the bite. You would also need retainers to stop the teeth  moving back to the original position once completed.

Would spring hawley retainer be able to close this gap as it very minor movement

Q. Hi, I had braces about 5 years ago and have started to notice a very small gap (0.5mm) forming between my 2 front upper teeth. All my other teeth are fine. I have heard a spring hawley retainer would be able to close this gap as its a very minor movement. could you please clarify this and also the cost of this retainer? many thanks.

A. Yes this could be used if you have enough room in your bite to push the teeth back. We charge around £400 for these appliances.

Q. To Dr Kailesh Solanki, I’ve had a really big gap through my two front teeth since i was born. I got braces in 2007 and then had the skin cut above my gums as it was tight and pulling my teeth apart, however, once i got my braces off the gap opened immediately..i had a retainer which failed to work I now have braces again, and the gap is completely closed, my orthodontist told me once the brace is taken off i will have a wire inserted into the back of my mouth so my gap will remained closed..and that i have to have this wire in my mouth forever. Would you be able to tell me anything about this fixed wire? I am also interested in getting porclein veneers as im tired of my mouth being pulled about every month. Would you reccomend veneers? Many thanks for your time,

A. Firstly, your dentist has told you about a fixed retainer. This is a very thin wire that is bonded onto the backs of your teeth. You will not notice it there- you cannot see it, yet it will keep your teeth in position. It is a good, standard procedure that I would recommend. If this is not done- then your teeth may drift back AGAIN.
As for getting veneers, if your teeth are straightened and you like the shape, size etc of your teeth- veneers shouldn’t be an option.

My left tooth is crossing over my right one how can i sort it out?

Q. I used to have a brace when I was 14, I am now 25. There are no NHS dentits available in my area (Nottingham) so haven’t been able to see a dentist for around 2 years. My teeth used to be straight, but my Left tooth is now crossing over my right one and is very noticeable. Its really starting to worry me that my tooth might falling out as it seems to be pushing the right tooth back. What would be the best way to sort this out. I don’t have alot of money and can not find a dentist anywhere near I live.

A. It is a common problem that I see on a daily basis. People who have had braces when they were younger and the teeth have moved back.
This is because:
1. They were not given any form of retainer to wear
2. they failed to wear their retainer after treatment had finished.
You need some form of orthodontic treatment to push you teet back into a staright position. the only problem that you have is that it costs. The NHS will no longer provide you with a brace as you are over the age limit- so you will have to pay if this is what you want.

Q. I am almost done with my braces but im going to college. so they gave me the option of taking my braces off and my teeth are straight but my jaw is still a little unaligned. so i was wondering if my jaw being unaligned would ruin my teeth being straight.

A. Either way you will need retainers worn full time for 6 months then nights for at least a year to prevent the teeth relapsing.

My front teeth is rested on my other teeth how could i fix it?

Q. hi i was wondering who could help me and how .one of my front teeth is sideways in my mouth it is only slightly straight because it is rested on one of my other teeth i had braces on it few years ago and it did work but after i had finished my treatment it started to back to how it used to be and my teeth started to all either go a bit forward or back and it really gets me down. is there anyway this could be put into a normal smile as the braces worked for a bit and then went back to normal i considered getting my teeth straightened medically but i am only 17 i work but its still really expensive.

A. I cannot stress how importnat it is to wear a retainer after having orthodontic treatment. A retainer does exactly what it says- it ‘retains’ the position of your teeth. This can be in the form a small, thin wire at the back of your teeth that is hardly visible- or a removable option sthat you can wear at night to prevent your teeth from moving back again.
Your orthodonist really sould have discussed this with you after you had your braces off. It is something I see alot of. I would definately advise you to have braces again with a retainer at the end this time :)
At you age, I wouldn’t recommend any other from of treatment. Hope this has helped. and good luck

A. I would report these symptoms to the orthodontist who made the appliances as jaw pain should not be a part of the process of wearing retainers
There may be a different type of retainer that can be utilised in your case

Ive notice that the enamel on my front teeth has worn away is there a way to built it up

Q. I have just had my braces taken off and now only have a retainer to wear at night. I hadn’t noticed before but the enamel on my front two teeth and a few of the bottom front teeth seems to have been worn away at the tips. Is there any way to build it back? Also, some of my teeth have wavy lines at the bottom instead of a straight edge. Why could this be? Is there any way of fixing this? In addition to this, one of my canine teeth is particularly large and more jagged than the rest of my teeth. Can this be re-shaped or filed down? Thanks.

A. Re4contouring of your teeth is possible to reshape and restyle your smile.  The wavy lines are called mammelons and are perfectly natural – just means you have had very little wear

I havent gone a day without wearing retainer can i hold off on it for a couple of days

Q. Hi, Today i got all four of my wisdom teeth removed and i feel fine, my face is normal i just had some bloody issues but it was only for a few hours which is considered to be normal apparently.. However I was wondering if it would be okay to brush, floss and put in a clear retainer on the top teeth and a wire retainer on the bottom teeth.. I got my braces off about 3 years ago and haven’t gone a day without wearing my retainers so maybe thats why i’m paranoid my teeth are going to shift.. i was hoping to wear it tonight but im scared of getting an infection because im pretty sure retainers store some type of bacteria.. basically my question is should i hold off on the retainer for a couple of days or am i capable of wearing it today?

A. Try them in, they should be comfortable, if not then see your dentist before leaving them out for more than a couple of days as the teeth will move, he may need to adjust them so you can carry on wearing them.

Ive a hairline fracture in my front tooth and will my retainer put pressure on them

Q. Hello, I had braces 3 years ago, and i still wear the retainer a few nights a week, I recently had an accident and have a hairline fracture/crack in my front tooth. It is difficult to see, you can only see it in certain light, I am worried that if i wear my retainer it will put pressure on the crack and cause it to get worse, it is a vertical crack going from the top to the bottom. It isn’t sore and my dentist had a great deal of trouble finding it. I have been to the dentist and was told not to worry, but I am not sure about wearing my retainer. I haven’t worn it for a little while because of this, and a few of my teeth have moved a little, I don’t really know what to do! Any advice would be appreciated.

A. Start wearing the retainer again, it won’t cause any damage to a superficial enamel crack. If you start ASAP the teeth may move back to their original position in a week or so.

A. Go to your dentist and take the mould with you. They might be able to use it and make one while you wait
