How can I prevent receding gums?

Thursday, January 14th, 2010
Q. Im 24 and in he last 5 months my gum has began to receed very quickly at the bottom in front of my bottom teeth, i went to the dentist and he told me as long as i kept brushing it should be fine but it is getting worse do you have any suggestions as to what it may be and how to stop it, the problem is making me very paranoid that im going to loose my teeth and i dont like smiling because i dont think they look very appealing please help.,
A. It might be over or under brushing, perhaps you should consider a gum graft to rebuild the gum. Ask your dentist for a referral for a 2nd opinion to a Periodontist

What can be done to fix receding gums?

Thursday, January 7th, 2010
Q. Hi Dr. Patel, The gum line on one of my front teeth is higher than the other.Plus the tooth is misshaped to begin with. It’s like someone stuck their finger behind the tooth and slightly pushed outward causing it to be more hollow in the back, thus making the tooth stick out alittle further in front than the other front tooth and also appears rounder.Whereas the good front tooth is thicker in the back and flat in front. Yrs. ago I had surgery on the good front tooth’s gum to try to make it the same height as the recessed one, but being healthy gum it regrew the tissue that the dentist had cut away.Is there a permanent solution to this problem? Most of my teeth in the back are caps and I’m in need of some root canals I know. But the only good teeth I have are the top front ones. But I’d like THOSE to be picture perfect or all to be exact!I don’t care to sacrifice whatever is needed. Is it possible to file down the protruding front tooth even tho it’s thinner in the back and either cap or bond them all? Money is no object. My front bottom teeth gums are also starting to recede.I’m a young looking 55 but am getting long in the tooth I guess.Need suggestions!!Is there any dentist in America that you’d endorse?I really need your professional opinion!! I have a lot of work that needs done. Starting with root canals and gum restoration I’m sure. I do floss evey nite but don’t water pick. Thanks for your patience!,
A. Hi there, there are a number of things you can do but I am a little lost as I am unable to see you to give you my best options.  You may be able to have some orthodontic treatment to move the gums levels and make them level than the teeth could be reshaped and build up to match.  You need to find a good cosmetic dentist and go for a consultation.

Why have gums receeded after veneers?

Thursday, January 7th, 2010
Q. Hello, Back in May I had a veneer fitted to one of my front teeth. Prior to having the veneer fitted I was advised that lasering the gum line above the tooth would make it more symmetrical – therefore I went ahead with this too. Since this treatment my gum appears to be receding and therefore the edge of the veneer is now visible. Why has this happened? and is there anything that can be done to reverse this, or to at least stop further recession of the gum? Or will I need a new veneer? Many thanks,
A. You need to go see your dentist so they can examine it and decide if a new veneers needed.

Confused as I cannot find a reason for my smelly and rotting teeth.

Monday, December 7th, 2009
Q. I had a veneer on my lateral incisor around 4 years ago. Last year I noticed that the tooth was discoloured around the gum line and had a bad smell/taste and feeling of pressure – never pain. After several courses of antibotics my Dentist performed root treatment, twice as it was unsuccessful the first time. My tooth still smells and tastes bad and my dentist is baffled – any ideas? I think perhaps there is a gap between the veneer and my tooth and that the front shell of my tooth is rotting, could that be the case – is there anything can be done? Thanks

A. You either have the abscess still present, gum disease or decay underneath your veneer.

Due to grinding I have gum receeding and loosing my smile.

Friday, December 4th, 2009
Q. Dear Dr Tennet, For as long as I can remember I have suffered from bruxism. I sleep with a nightguard which my NHS dentist made for me but my gums seem to be receeding as a result of the grinding, which I think has now led to gum disease, and my upper anterior teeth are becoming very thin and I’m terrified of them snapping off altogether. I’m only 27 and hate my smile. My teeth seem to be wearing down and decaying very quickly and I’m terrified of having to wear a denture. Do you think crowns and possibly gum grafts may help me? I really don’t want to lose my teeth and my NHS dentist doesn’t seem to care.

A. Wear, tooth decay and receeding gums are normally caused by separate problems. It is likely that advanced treatment planning is needed. I can offer a free initial chat at our private practice to assess the likely way forward.Rob0121 643 0610

Is full-mouth periodontal debridement good for treating gingival recession.

Friday, December 4th, 2009
Q. I have mild chronic periodontitis with areas of mild gingival recession. My dentist recommends a conventional full-mouth periodontal debridement. Is this procedure correct?

A. Sounds good to me.  You need to have all the bacteria removed to remove the infection in your gums followed by regular visits with the hygienist to maintain the results – at least 3 monthly

Small gap in front teeth is getting bigger may be due to gum disease.

Thursday, December 3rd, 2009
Q. I have consulted a few doctors (dentist) So far answers were not very positive. I have gap in between my front lower teeth. They are moving and gap is becoming bigger. What can be done to either cover the gap or close it?

A. The gap could be closed with either a brace, veneers or by bonding White filling to the teeth it would be good to find out why this gap is getting bigger though as it might be asign of underlying gum disease

Best option to repair or restore bad gum?

Monday, November 30th, 2009
Q. My gums are very poor at the bottom of my mouth and have been pushed down by plaque etc ?? due to bad gum disease, I am in the process of rectifying this and also looking into implants for the teeth that it has effected. Is there anyway that the gums can then be repaired or restored so that theres not big gaps etc ??

A. Unfortunately horizontal bone loss can not be re-grown to allow you to have the black triangles filled in.  A restorative solution will be your best option

I want to have dentures but I am concerned too.

Friday, November 27th, 2009
Q. I have chronic gum disease and have done for 20 years or more. Have gone through deep cleans but find it frightening, nerve wracking, emotionally upsetting all due to hate needles, loathe dentristy anyway ie: smell, tools etc and get in a panic to the extent I vomit whenever have finished suffering the treatment. I have asked for my teeth to be pulled out and replaced with dentures of which the dental hospital have actually listened. I am awaiting news from them now as they have to get an xray from my dentist and take it from there. I have thought about having dentures for years. Cannot go through another treatment of needles and scaling etc. I just can do it. It just keeps coming back and I am always being told off and made out that I am not doing what they ask and yet I do, I do everything and yet still the gum disease comes back to haunt me. My teeth are disgusting. Stained and gums very very sore. I have bone showing where the gums receded that much. Teeth are wobbly also. I feel a freak. I have done for ages and hate smiling. When you have dentures I know it is drastsic and I know it is a sore process etc but I want it done. What I want to know is can they say take out the top row of teeth and fit dentures and then do the bottom row of teeth so as to avoid having no teeth at all. I really want an end to this horrible problem and am getting very depressed over all of this.

A. It is possible to have immediate dentures to prevent you being without teeth – these may need to be remade 6 months later as your mouth will change drammatically once the treatment is complete

Need the cause and solution for my overbite issue?

Friday, November 27th, 2009
Q. Dear Experts, I have had 2 front teeth replaced by crowns and implants as a result of an accident. Unfortunately there are some discoloring at the top of the crowns where it meet the gumline which is unsightly. What can be done to rectify this? Also, it may sound silly, but it feels as if my teeth are too short and the bite is all wrong. My jaw and jaw muscles gets really sore when I close my mouth, trying to rest the top and bottom jaw on each other. So as a result, trying to compensate for the pain, I try and rest it on the front teeth. Unfortunately, then it creates an uncomfortable, clenching overbite. It is a very uncomfortable and painful situation and I would appreciate advice on how to correct this.

A. Firstly the crown may need replacing to correct the aesthetic problem. The bite problem is more complex and will need thorough investigation and teatment, perhaps involving a bite guard worn at night and the possible correction of the crowns.

Does implants not fix a denture with bone loss issues?

Wednesday, November 25th, 2009
Q. I am due to begin work for a full upper denture on 17/11 due to the loss of all byt 4 of my upper teeth. This was due to gum disease causing bone loss, passed down from my parents! My dentist tells me that an implant to fix a denture in would not stay put due to bone loss? Is this correct? Glad I found this site, and someone to answer my question, thank you!

A. Implants can usually be placed but you may need bone grafting to increase the amount of bone available

Dentures fitted causing pain in the gums

Wednesday, November 25th, 2009
Q. Hello I had ValPlast dentures fitted and got a pain in the upper gums on wearing them. I persevered before 4 months before I had a tooth pulled from that side and thought it might be the reason and it would get better over time. Now the other side hurts as well. Sometimes one side hurts and I can chew on the other and another time the other side hurts. I thought it might be cavitations? I recently did a heavy metal detox. Do you have any idea what could be causing this?

A. Your bite may need to be evened out to prevent excessive pressure on any one part

Replaced tooth is causing gum to discolour

Monday, November 23rd, 2009
Q. HI Dr Patel, I ‘ve broke two front teeth years ago, I replaced those now my gum is gettng black, I can’t smile anymore is horrible, what shoul I do? Should i send youa picture before I make any appointment? thanks

A. You are more than welcome to send me some pictures.  The crowns you have had may contain metal which can leave grey / black edges.  One option may be new restorations which are metal free.

Dental treatments to fix gum recession issues

Monday, November 23rd, 2009
Q. Dear Dr Patel Despite regularly attending my dentist and hygenist for twenty years I have now been told that gum recession on two lower front teeth is now so bad that I am not a candidate for gum grafting. As you may appreciate I am very annoyed with my dentist for not being more proactive over my recession issues given that I raised this with him from day one of attending all those years ago. On the positive side, radiographs show a substantial amount of bone remaining and I have no mobility whatsoever. My questions are these: Even with such severe gum recession would it be possible to place implants if required in the future? Are there any techniques for gum to be ‘built-up’ to accommodate implants after tooth extraction? By the way the (very) little tissue that remains is healthy and recession has been caused apparently by crowding, thin tissue-type.

A.

At my practice we get sent alot of people for implants who have very little bone , it is something you can have assessed. I am sure you could have implants , if the bone is short it can usually be added to.

How does diabetics influence implant surgery?

Monday, November 9th, 2009
Q. Hello I am type 2 diabetic ( controlled at present with diet and excercise regime ) I have lost teeth to upper left 3no and 2no to lower right. I am considering implant replacements . Would my diabetes compromise implant surgery . Also if positive what type of implant system is the best ?

A. Uncontrolled Diabetes is a contraindication to surgery and does adversley affect the predictability of dental implants. care should be taken if you lost your teeth due to gum problems.
Most of the main systems have “Bioactive” sufaces like Osseospeed (ASTRAtech) SLA active (straumann) and Nobelbiocare, Biomet etc, I mainly use Astratech dental implants because it is proven to maintain the surrounding bone in later years.

What is the best way to cover discoloured teeth?

Wednesday, October 28th, 2009
Q. I damaged my front tooth when i was younger this made it slack I was then given root treatment and in time my tooth started to discolour, it was then suggested i get an veneer, this venner looked good but after 3 yrs it chipped and I needed to get it replaced,my second veneer was only in for 2 weeks when it cracked from the inside.I was then given my third within 6 months this one became noticeably darker, my dentist advised me to get yet another, I am just home from appt and this veneer is slightly better but at my gum area all you can see is blackness. I did say that I was’nt happy with it but he expressed that is the best he can do with what he has to work with, my own tooth is black and yellow as he showed me. i am wondering if you could advise on any other options. thanks

A. Thanks for your questions, you may wish to consider having a crown on the tooth instead of a veneer. Although it would involve more preperation of the tooth, a crown would be much less transparent hence consealing the discolouration. I would recommend that you speak to your dentist to gain his professional opinion on the merit of this in your case. I hope that his helps.

Is bone graft painful and is it safe?

Wednesday, October 28th, 2009
Q. I’ve always had lovely teeth & visited the dentist regularly.About a year ago I noticed that every time I brushed, my gums would bleed. My dentist suggested various gels for gum disease, but now my top front tooth is slightly twisted and I still bleed. After having some x-rays Ive been told I have lost bone tissue and the next tooth may also come loose.I asked about implants. It has been explained to me that because there is a loss of bone an implant would not be possible at the moment.A bone graft/implant has been suggested. Can you give me some information on this.Where is the bone taken from? Is it done under general anisthetic? I am extreemly worried. I want my teeth to look good and I dont like pain.

A. Thanks for your questions, it is possible to carry out the procedure using an artificial bone, to prevent the need for a graft from another site on your body. This procedure can be carried out under local anaesthetic, although some clinicians will be able to offer Intravenous sedation, if you feel this would help you. The aartificial bone is then allowed to integrate for a period of months, before the process to place the implants would begin. I hope this heglps.
