A. This seems like a question a dentist would ask. Broadly speaking you need to assess the patients general health. Then local factors such as bone pathology, gum health, tooth health. Finally design needs to be considered such as path of insertion, clasping, pretreatment tooth adjustment etc

Q. I would like to know if it is possible to have a tooth implant fitted under the following circumstances: My permanent lateral incisors were removed when I was a child (roughly 6yrs old) before they could grow after an accident which fractured my jaw. I have had dentures, bridges and now have one crown fitted on one side and nothing on the other. I am now 37 yrs old.

Q. I am due to have my bottom front tooth extracted due to a snapped root, do you have to wait six months before a bridge can be fitted as i am having mine fitted on the same day and i am feeling very unsure that i should be fitted with a denture first
A. during the first six months after an extraction the bone will reshape. A denture is easier to adapt to this change. a bridge fitted after six months is likely to easier to adapt to the stable shape. bridges fitted at extraction sometimes need replacing after six months. Rob

Q. At the moment I have a 4 tooth bridge which is at the front on my upper teeth. It is a hinge which I have been told is broken so at the moment this is only supported on the one side which is on a post. I have been told by 2 dentists that as this is a large bridge I will now have to have a denture which although not happy have accepted this is the only solution. My problem that before I had my bridge had a plate and as have a strong gag reflex didn’t not get on with this at all also when I was eating it shifted as was most uncomfortable. I have been shown a denture that is metal and doesn’t seem to cover the roof of my mouth would this be a better option than the normal partial acrylic denture for me? I have been told that this is not available on the NHS? Any suggestions greatly appreciated.
A. hi many thanks for your email, my advice is to think about dental implants which are fixed and do not require a plate. the cost of a four unit bridge on two implants are approx £5000. msny practices do offer interest free credit to help finance the treatment.

Q. i had my teeth pulled about two and half months ago and been having trouble with dentures fitting. now they are saying i need alveoplasty ok. today they sent me to an oral surgeon he stated that i should of had the procedure done at the time my teeth were pulled. i would like your opionon please
A. It is not unusual for dentures not to fit very well following removal of teth an immediate placement of dentures. Many times new dentures need to be made 6 months later following a healing period. I am unable to comment on the alveoplasty issue without examining you.

Q. hi, i have really bad teeth and have a plate with 2 teeth on at the front as i lost a crown with 2 teeth on. i am now excempt from paying so would it be possible to have the crowns replaced on the nhs?
A. Hi, thanks for your question, it would only be possible to consider replacing the crowns if you still have the root. Your denture may have been placed over the root, but the root may have been removed. Without this information, it would be difficult to say. If the root remains, it may be possible to have a post crown, however if you no longer have the root, the options to replace would be either a bridge (which would be attached to the healthy teeth on either side of the gap) or an implant (which could support a new crown). Q. please can somebody help me i am 37 years old and had all of my teeth pulled out when i was 18 with 2 wisdom teeth so with not a tooth in my mouth i have worn dentures for nearly 20 years and as im sure you know what im just about to say i wont be able to have any more dentures due to no bone and gum left im getting quite distressed about this as the plates do not fit in my mouth now and the teeth on the plate have grinded so i cant eat or chew my face feels is collapsing.would i not be able to have this done on the nhs.can there be a positive outcome
A. Hi, many thanks for your email. the first thing that should be ascertained is how much bone you have generally i have found in my experiance that once a CT scan of the jaw bones is done, there is usally more bone in the jaws than you think. The next step once the true answer is known about how much bone you have is deciding on the the way the teeth are to be restored. th reason for the CT scan would be to see if you are a candidate for dental implants.
Preferribly four implant in each jaw would give you fixed teeth which would not require removing, the cost of which would be approx £12,000 per jaw.
Two implants per jaw fixed to a denture would be removable but when in very secure. the cost per jaw would be £5000
The alternative would be dentures which is what you already have, but with a better fitting surface and new teeth, these are not ideal, but cost wise may range from £1000 – £4000
There are many practices that offer interest free credit to help finance the treatment. hope this is helpful.

Q. I have had to have one side of my upper teeth removed, this has caused this side of my face to collasp and my mouth had lost definition and is drooping to one side, I look like I have had a stroke. Can you offer me any adice is this normal can this be corrected in any way so that both sides of my face are semetrical again.
A. hi many thanks for your email, when you lose teeth this can have a dramatic effect on the facial features on that area, unfortunately the only way to restore your face to some kind of symmetry would be to replace the missing teeth with either dental implants, bridge work if possible or a denture. hope this is helpful.

Q. Hi. I was womdering if you could help me. I have recently had all my teeth removed but wasnt offered immediate dentures so have gone without teeth for quite a while. I have had impressions made and have had the wax moulds done. How long does it take from that stage to actually having teeth? and are the wax moulds bigger than the dentures i will have because i couldnt close my mouth with the wax ones and they felt very thick. A. Wax molds or bite blocks are used to register how your jaws are related to eachother. The final dentures will be smaller. Dentres normally takes about 5 weeks to make.

Q. hi Dr Caplan 24 years ago i had 4 crowns fitted, my teeth were uneven before the crowns so the dentist inserted metal posts into my gums the new crowns were fab since then i have had a root canal filling due to reacurring abscess which took several hours 3 years ago one of the metal posts dropped out the dentist did some alterations to the crown and put the post in place he told me it could last one week one month or maybe years depending on how i looked after it and he would not be able to do any further repairs and my options would be a inplant or a denture the post has dropped out again it is next to sound tooth and a crown ,what are my options i cannot afford a inplant and do not want a plate thank you

Q. recently had four inplants, prungs, on the front bottom. I am getting ulsers or sores on back side. How can I get rid of the ulsers to see if it the dentures I am wearing. You see, I had to wear my old dentures a week after the inplants. And could not take them out. The drilled holes in them to take the four prungs. I recently had new dentures. They told me try them for awhile. After the ulsers heal, we can see if the dentures work or fit. How can I get rid of these ulsers. I take them out at home so air hits the ulsers/ Rince my mouth out with warm salt water aand even rinced with peroxide… yuk Please advise. I had the dental work at aaffordable dentures.
A. You may need to get the dentures eased away from the protrusions a little more as they are probably rubbing.

Q. Q. Hi Dr Caplan, My front left tooth has gone very crooked and exposed a large part of the root and is extremely loose, the 3 teeth to the left of that are missing and i wear a bridge which has contributed to the twisting, the next 3 teeth are not in good condition and the gum has receeded. on the right side my front tooth is chipped, the next 2 teeth are slightly longer and the gum is receeding, the molars on the right are not bad but there are just gaps and 1 is missing. would it be better to have most of the teeth pulled and a denture fitted as there are only about 8 teeth on my upper jaw and most are not up to the job. could you give me advice and a rough estimate of cost. In America they are called removable bridges because of the accuracy of their fit. There may be other solutions but ti sounds like this may be a likely treatment for your problem. A. It is likely tha tyou will need 4 implants to retain and stabilise your denture. The usual cost for this type of treatment is in the region of £7000.

Q. I have had a severe fear of the dentist after having a terrible experience around 10 years ago. Unfortunately in this time my teeth have gotten in to very bad condition, which I am really embarrassed about. After my front crown got caught at a dancing competition and broke, I decided I needed to take action and so I visited the local dentist. He devised a plan for me and I am currently on the waiting list to have the necessary teeth removed at the hospital and then all the fillings and treatment I need completing. I was however horrified with his advice to have my two front crowns replaced with dentures. I am 25 years old and devastated with this suggestion! There are crowns on my two front teeth at the moment(granted one is cracked) and I am in no pain. If I pay privately can I choose to have the crowns replaced? I understand that ultimately the dentists advice may be correct, however, I at least want to try! I would really appreciate your help, as I am so upset! Thank you in advance!
A. Thanks for your e-mail, I am sorry to hear that you have had bad dental experiences in the past. I feel that for your own peace of mind, you need to attend a General Dental Practice to gain a second opinion with regards to the feasability of saving your tooth. The amount of healthy tooth tissue which remains will determine whether or not the crown can be replaced, and without a consultation I would be unable to give a definite answer. It may be that there is not enough tooth tissue left, and that is why your current dentist is advising you to have the extraction. However I feel that if you do not gain an independent second opinion, you will always wonder. There are private practices which you can attend who will offer a free consultation with no obligation to have traetment. If you require any further advise, please do not hesitate to contact me.
