Q. Hello. Five years ago, my orthodontist informed me that I am missing both of my adult upper canines. I am currently 20 and, luckily, I am still in possession of my ‘baby-teeth’ but I fear that I may loose them within the next year. I’ve been told that I will need dental implants, and that they’re very expensive. Being a student, I’m already facing a large amount of debt. Is is possible to get this treatment on the NHS? My dentist has informed me that it may be possible to reduce the cost by having dentistry students perform the surgery. How much risk is involved if I choose to go to a teaching hospital, etc?

Q. I have had a leaking abcess above my front left crowned tooth … some dentists say it is not a problem as the toxins can get out, but I have noticed that both Front teeth are now loose , is the infection eating into my bone ? I am considering a bridge as I cant afford implantology is that likely to be a problem .
A. hi many thanks for your email, if you have an abcess then the chances are the infection will be eating the bone. my advice is to get it sorted as soon as possible. a bridge is a good option to replace a missing tooth as long as the teeth holding the bridge are strong and healthy.

Q. When I was a child I fell on my bike, cracked my front tooth (tooth #10), had a root canal done, then a composite attached to reshape the tooth from the crack. 9 years ago, the composite fell off and I went to the dentist to have a new one done and she said a crown would have to be done as soon as possible. She said the best one was a porcylain fused to gold crown and assured me that no black lines would show because it is not metal, it is gold. (I was young and uneducated in this department…) She immediately drilled down the tooth, took impressions, put a hidious bulky temporary crown on it and I left with a black and blue and extremely raised gum line. I returned 2 weeks later to have another hidious bulky permanent crown put on. The crown was longer than all the teeth in my mouth because she drilled my tooth so high up the gum line that my #10 tooth had a “new” gum line. The gum line is not only still much higher than all the other margins, but the “gold” from inside the hideous crown has darkened my gum line and the bulky crown does not slide under my gum line at all. The gum around the crown not only is dark at the margin, but almost appears to be dark farther up the gum than the actual margin, if that makes any sense. I don’t know why this is. I tried having this dentist resolve these issues, and it was an endless battle, so I decided to give up for now. I did not have the money at that time to persue the situation, or to have the problem fixed by another dentist. Now, 9 years later and financially fit, I have braces that are almost finished. I went to my local dentist to start preparing for the crown to be replaced (after braces come off) who said he could put a new crown on and it would look normal again. I asked him “what about the gum line, how are you planning on making the receeding gum look normal again and come down over the new crown and in line with all the other gums” and he then decided to add that I should see a perio before a new crown is done. I immediatly went to a local perio, he examined the area and told me that he would not be able to graft gum because the tooth was drilled as high as the gum line sits, so there would be nothing to graft to. I asked him if it would be wise for me to have my othodontist bring the #10 tooth down and hope that bone will come with it (and maybe even the gum) and if that would resolve my problem. A lightbulb lit over his head and he said that would be an excellent idea. I went to my ortho the next day, explained what the dentist and perio told me, and he said it is a 50/50 chance that the bone and gum would actually come with the tooth if he brought the tooth down. After all this controversy, I decided to go to a dentist out of the area and in a larger city that was referred to me by a friend. He is a cosmetic dentist (or so he says). He examined my tooth and felt with his instrument that there was a crack on the tooth just above the margin of the crown. He took a close up xray, and there in fact is a horizontal crack on my tooth between the crown margin and the gum line. He said he needs to cut the crown off soon so he can see what we are looking at. He also said a new post would need to be inserted to replace the existing post, as it is too short and not really supporting the tooth anymore. He said if the tooth can be saved (which he says is very very likely) he would put a temporary crown on, have my gum examined by his perio, have my ortho bring the tooth down with the temp crown in place, and return to have a Lava crown put on when all is ready. My questions are: All these professionals are giving me different advise on my gum line situation, I can’t be the only person out there with this problem. Will the gum most likely come down with my tooth and temp crown and avoid the need for perio work altogether? Is it normal for a gum to appear slightly discolored from a gold fused crown all the way up, and not just at the margin? Is a horizontal crack at my margin a cause for concern? (The last thing I want is an implant…) Is a Lava crown the best choice for a 100% natural look? I’m sorry this is so long, but after speaking to the dentist out of the area (that noticed the crack) he seemed to need to know everything from the beginning to be able to better help me. My lack of trust is explainable based on all these professionals telling me different things, and after my initial experience with a horrible crown. I would just like some other professional opinions, if possible. Thank you…
A. hi many thanks for your email, ok to try to explain everything i will use bullet form:
1: when a tooth is root canal treated the tooth can discolour all the way to the top which is the probably reason for the discolouration past the margin of the crown.
2:the horizontal crack is not a problem as long as its superficial and does not run through the whole crown, the crack can be caused by the post.
3:there is unfortunately more than one way to crack a nut and the same can be said to fix a tooth. there are always risks and benefits with all treatments.
4:the gum coming down is a possibility but unfortunately it is an unpredictable science, the orthodontic extrusion is the option i would try doing it very slowly.
5:make sure there is at least 3mm of tooth tissue above the gum line around the new post to ensure the crown will be sucessful.
6:if a new metal post is to be used to support the crown the best all porcelain crown is lava to replace the crown as it will not allow the metal post to shine through but does not contain metal so is aesthetic.
finally the option of an implant if done properely maybe better than the other alternatives.

Q. hi Dr Caplan 24 years ago i had 4 crowns fitted, my teeth were uneven before the crowns so the dentist inserted metal posts into my gums the new crowns were fab since then i have had a root canal filling due to reacurring abscess which took several hours 3 years ago one of the metal posts dropped out the dentist did some alterations to the crown and put the post in place he told me it could last one week one month or maybe years depending on how i looked after it and he would not be able to do any further repairs and my options would be a inplant or a denture the post has dropped out again it is next to sound tooth and a crown ,what are my options i cannot afford a inplant and do not want a plate thank you
A. Your only othe roption will be to have a bridge to replace the failing crown/crowns

Q. I have been advised that one of my bottom molars need to come out due to problems with the roots. I have also been looking into the option of braces for my top teeth as overcrowded and have been advised that I need to have one on my bottom teeth for the bite (also my bottom teeth are overcrowded also). Can you please advise whether it is possible to have an implant to eliminate the space of the missing molar and then have invisalign braces fitted or whether it is impossible to have a brace fitted to an implant? Regards

Q. recently had four inplants, prungs, on the front bottom. I am getting ulsers or sores on back side. How can I get rid of the ulsers to see if it the dentures I am wearing. You see, I had to wear my old dentures a week after the inplants. And could not take them out. The drilled holes in them to take the four prungs. I recently had new dentures. They told me try them for awhile. After the ulsers heal, we can see if the dentures work or fit. How can I get rid of these ulsers. I take them out at home so air hits the ulsers/ Rince my mouth out with warm salt water aand even rinced with peroxide… yuk Please advise. I had the dental work at aaffordable dentures.
A. You may need to get the dentures eased away from the protrusions a little more as they are probably rubbing.

Q. My daughter is 19 and has 2 pre-molar (baby teeth) still in place. There are no second teeth to replace them. Her own dentist suggests extracting these 2 teeth as she says they are growing into the gum. Do they need to be extracted or do you think they should stay. If they have to go what will she get to replace the wide gaps. thanks
A. hi many thanks for your email, the problem is if there remain in the mouth there will continue to grow into the gum and be functionally unless, the alternative is to remove the teeth and replace the gap with fixed teeth via dental implants which should hopefully last a very long time.

Q. I was diagnosed with gum disease over a year ago and i’ve now been told apart from my loose teeth my gums are well improved. my problem is i have prominent gaps between my front 2 teeth top an bottom and i don’t know what can be done to help. my dentist has told me eventually they will fall out. i hate smiling and i hate my teeth.

Q. hello, i have a full upper denture which i cant stand. it doesnt fit and is loose. was wondering about mini impants. have been told i ll need about 4 2 hold denture. i dont have a lot of money but maybe could get a loan?? just wonderd how much it will be. plz dont say come for an appointment as i dont have the £60 or wot ever u charge just to see u.
A. It is likely tha tyou will need 4 implants to retain and stabilise your denture. The usual cost for this type of treatment is in the region of £7000.

Q. I had cosmetic dentistry done many years ago, which included replacing my middle four front teeth (top) with crowns. One was knocked loose and subsquently I have had it removed as it was apparently so small that there was nothing for crown to hold on to. I am now using a temp denture, however a couple of things I would appreciate your advice on: 1) is there any alternative to temp denture, only I find it very painful after a while, and it looks very ugly with a thick pink plastic rim around the tooth, that does not match my gum and looks very prominent. 2) I have been diagnosed with gum disease around these crowns, and am desperately trying to work on this, but is teh denture now causing irritation to gums, as they not getting any better (p.s. gum disease appears to be around teh crowns only) I am very depressed about this situation and would give anything to be able to smile again, Many Thanks
A. Hi
Thanks for your message, you mention that you are desperately trying to work on the gum disease. Forgive me if you are already doing any of the things I suggest, but I would recommend that you attend your dentist for their advise on your gum disease. Following on from this, as well as advsie on how you can try to improve things yourself, I would recommend that you attend regular appointments with your dental hygienist.
There are other options available instead of your temporary denture, firstly there is the option of having a permanent denture made. When you have a permanent denture made, it will take 4 – 5 weeks, and the end result should be a vast improvement in both appearance and comfort from the temporary one. If this is not something you wish to consider then you may be interested in a bridge. This would involve preparing the teeth on either side and fitting a bridge ( this would have attachments for the teeth either side and a false tooth for the gap) and fitting the brdige to them. Finally you would have the option of an implant. This would not involve any preperation on your other teeth, as the implant is placed into your bone, and after a healing period, a crown that then be placed.

Q. Hi Dr. Moore. I have two maryland bridges replacing my eye teeth. They were recently replaced as the ones that I’ve had since I was 14 (I’m now 30) were chipped etc. That being said, the new ones keep popping out. My dentist has used numerous types of cement but it keeps happening. As my roots are too close together for impants (I was born without the teeth) are there any other options for me? Thank you
A. If the 1st bridges lasted a long time it might be worth trying new bridges perhaps looking more closely at the bite as this is often the reason the bridges come out.

Q. If slight showing of Titanium after 6 years, is this of any concern to my implant?
A. This can happen., just like gums may recede on natural teeth. I would recommend you see a dentist regularly to keep it under review

Q. ihad 3 front teeth taken out 3 days ago and partial dentures fitted. Unfortunately they very uncomfortable and all i seem to be biting on is plastic. Will they settle down? Please advise.
A. Yes they normally do but let your dentsit have another look if they aren’t settled in in 2 weeks, you could also consider alternatives such as implants or bridges.

Q. My two back teeth on the top left both have root fillings ,one has a gold crown being held in by a metal pin ! went to see my dentist today to be told that both teeth I will come out eventualy but because my jaw is out off line and clicks ,I need to replace them with i’ver plate or inplants .both teeth are giving me truble , I don’t like the sound of a plate as I am only 37yrs and really upset ! I know inplants aren’t cheep but could tell me how much I would be looking to pay and where I could get it done? I live in bromley kent , thankyou for your time .

Q. Good afternoon. I am a 46 year old female and am having trouble adjusting to my upper denture. I have had a full upper denture for approximately 2 1/2 years. My original dentist removed my remaining 6 front teeth, and made my denture. After 6 months, he made another upper due to the shrinkage of my gums. The first set did not fit well, even with adjustments, so I ended up gluing them in until he made my second plate. The second plate did not fit well or look good at all. After repeated fittings and adjustments, I kept both upper plates and returned to wearing the original plate made. About 1 year after these plates were made, I went to affordable dentures and had a new upper plate made which was much better appearance wise, and fit alot better than the others. This has been a litter over a year ago, and I wore them for about 4 months before I started having to use adhesive to hold them in. I am considering mini dental implants for my upper plate as I have to use the adhevise several times a day to keep my teeth tight. I have spent what I consider a considerable sum over the years (approximately $3,800.00) for this work. How many posts would be required to hold an upper plate in position and would the implants do what I need them to do? Thanks so much for your time.
A. The specifics of the volume and health of the bone combined with the bite force and direction will determine your answer. You will need a clinical assesment for this.

Q. Dear Sir,I have had a pallet fitted on the top,and one at the bottom,but i can not wear them.It has cost me nearly £2000.The dentist has said to me that he his going to refer me to a specialist, and that i could get them free on the NHS.He said that i am a gagga or something like that. I have got just three teeth in the top and just three in the bottom at the back. It is making me ill becaus i am jovial chap,and i can not smile any more.So please will you answer this straight away,i know that you are busy.I am 62yrs old.Thank you for the reply.

I agree with your dentist that you should get a second opinion with someone who spealises in dentures. They can either adjust your existing pair to make them more comfortable or make you a more suitable pair. Alterntaively you could consider implants, but these are much more costly and start form £2500 per tooth.

Q. Hi, due to a medical condition, i have regular MRI scans of my head and spine, as they won’t do these if you have a chance of metal fragments in your body, is it safe to have a dental implant which i was planning? Many thanks

please consult an implantologist and you do not need an MRI scan to have an implant placed however you medical history will need to be be assessed before proceding with any treatment.
