Q. Hi Dr Caplan, My front left tooth has gone very crooked and exposed a large part of the root and is extremely loose, the 3 teeth to the left of that are missing and i wear a bridge which has contributed to the twisting, the next 3 teeth are not in good condition and the gum has receeded. on the right side my front tooth is chipped, the next 2 teeth are slightly longer and the gum is receeding, the molars on the right are not bad but there are just gaps and 1 is missing. would it be better to have most of the teeth pulled and a denture fitted as there are only about 8 teeth on my upper jaw and most are not up to the job. could you give me advice and a rough estimate of cost. I live in North London, not too far from St Albans. Many Thanks
A. Thank you for your questions. Apologise for not getting back sooner but I have been away lecturing. In your situation I have often used a precision retained denture. In America they are called removable bridges because of the accuracy of their fit. There may be other solutions but ti sounds like this may be a likely treatment for your problem. Q. I need to renew some bridgework and have a smile makeover. With so many dentists offering what seems to be a great service how do I go about find the best & most experienced dentist to carry out the work? In the pastI have had a poor experience with a bridge fitted and do not want to compromise on quality and results again. Kind regards

Thanks for your questions, I think that the most successful way of finding the best dentist to carry out your smile makeover, would be to listen to recommendation from friends or relatives who have had similar work carried out. If this avenue is not fruitful, then I would suggest researching different cosmetic dentists on the internet, and maybe consider attending a few practices who offer free initial consultations. This would allow you to meet the dentist, discuss the options that they recommend and make an informed choice based on your experiences without any financial loss or commitment to treatment. A. You could try a dental cement from Boots but I doubt you will be able to do anything without a visit to the dentist.

Q. Hi Dr. Moore. I have two maryland bridges replacing my eye teeth. They were recently replaced as the ones that I’ve had since I was 14 (I’m now 30) were chipped etc. That being said, the new ones keep popping out. My dentist has used numerous types of cement but it keeps happening. As my roots are too close together for impants (I was born without the teeth) are there any other options for me? Thank you
A. If the 1st bridges lasted a long time it might be worth trying new bridges perhaps looking more closely at the bite as this is often the reason the bridges come out.

Q. ihad 3 front teeth taken out 3 days ago and partial dentures fitted. Unfortunately they very uncomfortable and all i seem to be biting on is plastic. Will they settle down? Please advise.
A. Yes they normally do but let your dentsit have another look if they aren’t settled in in 2 weeks, you could also consider alternatives such as implants or bridges.

Q. Dear Dr Glaister, I consulted your site in 2006 and was delighted with your speedy and informative response, therefore, now that I have saved enough money to pay for the dental work that I need, I hope you will be able to help me once again. I decided to send my qustions to yourself as i understand you have great experience with cosmetic surgery and feel my question slighlty steps over the line from required treatment to cosmic treatment. From the start I should say that during childhood and my early teenage years I neglected my teeth insofar as I didn’t brush them at all; I didn’t even go to the dentist until I was seventeen. Four years on from my first visit and I have finally found a dentist who actually seems like he wants to help rather than inform me that I should have looked after them when I was younger in a patronising yet cheerful tone as my previous dentist often did. I have been going there for around six months now and as such my check up is due. When I visit at the end of this month there are a few things I will be asking the dentist, but thought I would get your opinions first: One thing I noticed since I first started going to the dentist is that after I had received all the necessary treatment, a few weeks later I would need to go back to the dentist after experiencing tooth pain, only to find I need another filling, and also by the time my next check up was due, I needed a whole host of work. Even though I brush my teeth twice a day with Colgate total and use Cordosyl daily (as recommended by my dentist) my teeth still seem to be going from worse to worse every few months, or in some cases weeks, and have been for the last few years. To the stage where I am sure the next time I go I will be told I have gum disease as lately my breath has an unpleasant odour and one of my teeth has started to hurt; a pain that I can only describe as being inside the tooth, yet not part of the tooth but rather up inside the gum. This is the main factor that I am holding back from spending my life savings on private treatment, i.e. crowns/bridges. Insofar as I do not want to spend every penny I have if the teeth the crowns are fixed to will still be rotting away underneath and thus the crowns fall out and I have wasted my money and am left with small stumps for teeth and no money to fix the situation. So could you please advise me as to whether it is possible for the teeth to still rot away underneath the crowns? Or if, once treated the gum disease is likely to cause problems for the crowns/bridges? I understand that you obviously cannot comment on me specifically, and even if you could, would need to examine my mouth before answering, but I am asking, based on the information provided, i.e. teeth keep decaying and worsening even with proper care and attention, is it possible and if it is how likely, that the teeth and will rot away underneath and cause the crown to fall out. Could you also please advise; on an estimated timeline from the start to the finish of treatment, detailing what procedures are involved. I am looking to have sixteen teeth fixed and would greatly appreciate your advise, because as you can imagine with my current smile of autumnal colours I have no confidence and do not even want to go to job interviews or out with friends as my teeth become the focal point for everyone, and there is only so much abuse and embarrassment you can take. Even going to the dentist themselves has become a struggle as I feel as though everyone must be talking about me, as I am told I have the worst teeth he has seen in someone my age. Plus it has now become a practical decision, in that even eating is now becoming more of a challenge as hot and cold foods and drinks seem to aggravate different teeth on different days as and when they desire. If you think having crowns would be a bad idea as it is the likelihood that the teeth will indeed continue to decay underneath, is their anything you could advise as an alternative as I don’t really know what to do. I have a budget of £8000 and obviously cannot afford to have implants. Also, it is the upper and lower front four teeth on the left and the right that I want the work to be carried out on. Q. I’ve had a fixed dental bridge about 15 years ago. Somehow they became sort of grayish in color. I can not afford to have then redone, so is there a way to whiten them?
A. You can not whiten porcelain so the only option would be to replace the bridge.

Q. I had an apisectomy roughly ten years ago and have 2 caps on my front teeth. However I have been told that the root is still infected and that I require another operation but there is not a high success rate. What other options are available to me? Should I go for implants or a bridge? I would like something that will last me for a longer time and look natural as my caps are very discoloured. Many thanks

I recommend you book an appointment with an implantologist to see if this would be an option for you. I then would ask the dentist that recommended the further treatment for this tooth and ask him the prognosis and make an informed decision from there.

Q. hallo dr my lateral frontal tooth was broken under the gum line my Dr. said that the root canal is good and he put a screw inside using x ray it appear at t6he upper part of it then he make crown id like to ask is this will be stable life time or the screw will broken the root canal or fallen as i heared frm my friends thank u
A. Post crowns are the last chance to try to save the tooth as you know there is very little good tooth left. Long term you may need an implant or bridge.

Q. hi my teeth are really startin to decay and im scared to go to the dentist i dont want to have dentures is there anythin else i can have done on the nhs like crowns or bridge work or is that only for 1 or 2 teeth as i only have a couple of teeth that are actually ok and wot wld b the best option for me appart from the dentures?

Q. one of my bottom teeth has become loose the whole of the length of the root is exposed and my gum has receded either side of it and if i rock the tooth the root pivots out of the gum there is no recession inside the mouth this is under the bottom lip can you give me advice on what to do i have to see my dentist on thursday but not sureif hes familar with things like this Thanks
A. many thanks for your email, from the sounds of it the tooth has been subject to attack by periodontal disease. it needs to be taken out. the options after this are to 1: replace with a denture, 2: replace with a bridge 3: if possible replace with a dental implant.

Q. Hi, I only have 1 back molar tooth on the bottom rear (the 1 behind it & ahead of it had to be extracted). Are there any problems down the road if I don’t do anything about it – or should I be getting a bridge? I’m told a bridge will run around $3,000.00 but that’s a lot of money! Please let me know if there would be any problems leaving it as is? Thank you!
A. Its always best to replace missing teeth despite the cost as not replacing teeth causes added stress on the other teeth

Q. I am 61 years old and as seems to be with many older people I have considerable movement of my lower teeth resulting in them bunching in the front. My upper teeth now also appear to be moving and I would like to have them corrected. Is this possible? I should say I enjoy good health both orally and physically and participate in regular excercise and have never smoked. I visit the dentist for check ups regularly but have been told I am too old to bother with this. Your opinion would be appreciated.

Q. Hi Dr Caplan, I sent a question about 3 weeks ago and haven’t seen a reply yet so I’m not sure if you received e-mail – apologies if you have received it twice. I am 48 years old and I lost one of my front teeth when I was about 13 years old, I had a fall and the dentist removed the broken tooth and fitted a false one on a pallet which I hated. About 17 years ago I had a bridge fitted which now needs attention as it is starting to wear badly. However my problem is that I have protruding front teeth that I would dearly love to have straightened (they are not so bad and they are not crooked in any way – I think the problem was caused with m
e having a dummy as a child) Am I right in thinking I cannot have an invisible brace fitted when I have a bridge? If not, what do you think my best option would be where I could have an invisible brace fitted? Can I have the bridge removed and have an implant fitted and a brace fitted then? I do hope you can help me – despite my age, my confidence is rock bottom because of this problem and I don’t want to waste anyone’s time by making an appointment. Many thanks in advance. Many thanks
A. Apologies for the late reply. for the Your problem is definitely complicated by the presence of the bridge. I have treated cases like yours where I widen the smile using veneers further back to balance the appearance of the front teeth. The treatment usually involves about 10 teeth including a new bridge. This allows me to create a beautiful smile. Invisalign may be possible to use to help achieve this but usually this is not required. A. It sounds like your teeth are not savable. Your options are now to have new dentures, dentures supported by implants or bridges supported by bridges. You would need to find a dentist to discuss the fees for these procedures
