Q. I would like to know if it is possible to have a tooth implant fitted under the following circumstances: My permanent lateral incisors were removed when I was a child (roughly 6yrs old) before they could grow after an accident which fractured my jaw. I have had dentures, bridges and now have one crown fitted on one side and nothing on the other. I am now 37 yrs old.

Q. hello, I have just visited my dentist to-day,I have a tooth were there is only the front part left my dentist say’s he is going to remove that front of the tooth as there is a bit of leekage in it,I don’t want to get it taken out as my smile will be ruined and im very fussy about my teeth are there any other suggestions of what i could do to get it replaced without costing a fortune,any advice would be helpful.
A. Hi
Plesae do not worry as you do have options to replace the gap when your tooth has been removed. The best options would be an implant, however this would be the most expensive option. There is also the option of a bridge, which would involve preparing the teeth on either side for crowns, and then the laboratory would create attach a false tooth to the two crowns to create a three unit bridge. Your final optin would be a partial denture, which would be removable. I hie this helps.

Q. i had bridge work done all of the upper not happy with the results, one side seen lower the teeth seem too big, did have some adjustments, still not happy, also this took almost 3 months to complete. what are the procedure to have this redone. not happy
A. Very sorry to hear that you are not happy with the result of your bridge. The only thing to be done now is to have it redone. If you came to see me I would firstly assess exactly what you are not happy with using photographs and possibly imaging. Impressions of your existing bridge would be taken and a diagnostic mock up of your teeth would be made in wax on the model. When your bridge is removed this mockup would be used to produce a beautiful temporary bridge and any adjustments would be made at this stage until you are happy. This would then be copied to make the final bridge. In this way the aesthetics are very carefully controlled. This could be completed before Xmas but you will need to contact my surgery as soon as possible as my clinical time is limited due to lecture commitments. I hope to see you soon.

Q. Q. I am due to have my bottom front tooth extracted due to a snapped root, do you have to wait six months before a bridge can be fitted as i am having mine fitted on the same day and i am feeling very unsure that i should be fitted with a denture first
A. during the first six months after an extraction the bone will reshape. A denture is easier to adapt to this change. a bridge fitted after six months is likely to easier to adapt to the stable shape. bridges fitted at extraction sometimes need replacing after six months. Rob

Q. At the moment I have a 4 tooth bridge which is at the front on my upper teeth. It is a hinge which I have been told is broken so at the moment this is only supported on the one side which is on a post. I have been told by 2 dentists that as this is a large bridge I will now have to have a denture which although not happy have accepted this is the only solution. My problem that before I had my bridge had a plate and as have a strong gag reflex didn’t not get on with this at all also when I was eating it shifted as was most uncomfortable. I have been shown a denture that is metal and doesn’t seem to cover the roof of my mouth would this be a better option than the normal partial acrylic denture for me? I have been told that this is not available on the NHS? Any suggestions greatly appreciated.
A. hi many thanks for your email, my advice is to think about dental implants which are fixed and do not require a plate. the cost of a four unit bridge on two implants are approx £5000. msny practices do offer interest free credit to help finance the treatment.

Q. Hi. I went to the dentist a few weeks ago, I had a tooth that was lose. She told me it was a baby canine. After doing an x-ray, she said the tooth on the other side was also a baby, and that both adult tooth, were still up in the gums. She talked about braces and having the adult teeth exposed. I really do not want to have braces. I’m 26. Is it possible to just have the baby teeth removed and have veneers put in instead???

1: braces, expose the adult teeth and try to bring them down.
2: bridges on both sides to replace the missing teeth. so no need for braces.

Q. Dear Dr Caplan, Could you help…. I had a bad tooth removed by my nhs dentist, the option for me was to have a cap tooth fitted and glued to the tooth next to it,,After fitting th tooth felt too big and I was told this was a normal feeling even though it also loooked too big, after only two weeks the tooth became loose and fell out, so I went to another dentist, the tooth was altered and refitted again by bonding mixture,, I was told again that the tooth would stay in for years.. only a few days later the tooth is again loose and I cannot eat antyhing on that side of my mouth in fear of it falling out again. Could you advise what I should do next Kind regards
A. The cause for stick on bridge failing is often due to the bite being incorrect. With you saying that the tooth feels too big this may well mean that it is in the way of your bite. ANother cause is that the bridge deos not fit sorectly or that the pressures from your opposing teeth are to great. You may need a new bridge or may be even an implant

Q. I am 51 years of age and had a brace fitted approx 10 years ago. All of my teeth have been pushed in too far, resulting in a narrowing and picked look to my face. The dentist in question has admitted liability to negligence, but mostly to taking too many teeth out. I now have broken teeth and 2 bridges fitted in with super glue and a bad from of gum disease, with likely hood that I will lose most of my teeth. In th
e last couple of years I have been so embarrassed by my look and the constant failure of the bridges that I have been too ashamed to work. I am desperately looking for cosmetic dentistry that will permanently improve what has happened. I am an athletic type of person so really need implants that are strong and stable and long lasting. I have been to two other dentist pratices for a programme of treatment but both have proved not satisfactory for my long term needs. I am pretty sure I will need bone grafting and would prefer to have as many teeth replaced with implants and not bridges. My bite is also a problem and is causing me problems. I used to have a wide big smile the treatment with the brace has seriously changed my face and aged me quite considerably. I am now single as a result of this, so it has had a very dramatic effect to my life. My solicitor has been hopeless in all of this and is just wanting me to choose any dentist for work, which I am not willing to do, he will also need a programme of cost and upkeep etc. Please can you help.
A. You need to find a dentist in your area that can provide complex treatment of this type. Perhaps you should look on the ADI website for a dentist with the appropriate experience.

Q. I have had to have one side of my upper teeth removed, this has caused this side of my face to collasp and my mouth had lost definition and is drooping to one side, I look like I have had a stroke. Q. Q. I have a tooth gap caused by missing two lower central incisors. then my dentist discussed with me that the solution with the problem is to enlarge the gap more enough to fit a fixed bridge so he had to put braces on my teeth. its been 10 months now, and the gap’s size now is like enough for one central incisor. but he said its enough space to place already the fixed bridge. im wondering how could 2 lower central incisors fit onto a space enough for only one? can you help me, am i on the right track here? thanks…
A. hi. many thanks for your email, if you are unsure what the final aesthetic outcome is you could ask your dentist to produce a diagnostic waxup which is a model of your mouth with the final bridge waxed up in the gap area so you can see how it will look before you commit to having the bridge placed.

Q. Hello, I’ve had surgery to expose both canines in my palette and a brace to move them forward, however the spaces aren’t large enough for them to move into the correct position. Both caninees sit in place, but behind the natural position. Is it possible to attach a false tooth (or a thick veneer?) to the front of each of the real canines rather than to have a bridge or brace? Or would it be a possibility to shave the sides of each canine and both adjacent teeth on each side to give the canines room to move forward without the removal of any upper molars and use of a brace to create space? A. hi many thanks for your email there is a procedure where you can skim of the tooth tissue on the side of the teeth called IPR which can then create enough space to move the teeth into allignment. my advice is go to see a cosmetic orthodontist and ask his views on what can be achieved. hope this is helpful.

Q. hi Dr Caplan 24 years ago i had 4 crowns fitted, my teeth were uneven before the crowns so the dentist inserted metal posts into my gums the new crowns were fab since then i have had a root canal filling due to reacurring abscess which took several hours 3 years ago one of the metal posts dropped out the dentist did some alterations to the crown and put the post in place he told me it could last one week one month or maybe years depending on how i looked after it and he would not be able to do any further repairs and my options would be a inplant or a denture the post has dropped out again it is next to sound tooth and a crown ,what are my options i cannot afford a inplant and do not want a plate thank you
A. Your only othe roption will be to have a bridge to replace the failing crown/crowns

Q. Hi, I wonder if you can help. I had an upper bridge fitted by an NHS dentist and it really is the worst thing I have ever done. The 4 teeth are massive and I feel my smile has been ruined. I now speak with a slight lisp and have a constant dry mouth. I also now have an over bite. This bridge also sticks out farther than the teeth either side of it. A. This is unlikely to be possible and you probably need a new bridge. NHS dentists are limited as far as aesthetics goes due to the fee that they recieve limiting them to use basic laboratory technicians to produce well fitting bridges but with limited aesthetics

Q. Hello Dr Solanki I had a missing tooth at the back of my upper teeth, the front two had previously been veneered some 14 years ago and one tooth next to a front tooth was a crown. I wanted the crown replaced, a bridge to be fitted to fill the missing tooth and the rest of my uppert teeth, which were healthy, to be veneered. The dentist thought it best to replace the whole upper teeth with two seper bridges, one starting from the right front and one fron the left front, a total of 11 teeth. the bridge unit starting from the left is ok , feeling quite smooth, but the other unit feels rough.What is causing this and what would be the remedy, and also, is it wise to file down perfectly healthy teeth to take a bridge? A. hi, my answer to your question is i try to avoid filing down healthy teeth to accomodate a bridge, the bridge could be rough due to excess cement being left around it, or the porcelain might not have been glazed sufficiently to give it a smooth surface. if this is the case it can be polished in the mouth with a diamond paste. hope this information is helpful.
