There are black spots on my teeth should I be concerned?

Q. Hi, It was a few years ago i noticed black dots on my teeth, i’ve since been to the dentist every 6 months apart from in the last year. She has always said there is nothing to worry about, but now i havent been to see the dentist in a while i have noticed they are gradually getting larger and on one of my teeth there is a second smaller black dot(the black dots are situated on the back two teeth on the bottom on either side, 4 in total)I have a phobia of any dental surgery. I had 2 teeth removed when i was 8/9yo under gas because of it and have was wondering is this decay also if it is are there any treatment which i would be put tosleep for the procedure?

A. It is very likely that the black dot is just stain in the pits an fissures of the teeth. Ask your dentist to x-ray them that way you will be able to tell if the dots are decay or just stain. You could have them filled if needed with sedation although the fillings will probably be very small.

Need treatment for my discoloured teeth for reasonable cost

Q. Hi i got a single yellow upper front teeth. it looks very ugly . iwant to get rid of this problem .if i go for surgery how much it cost for me pls help

A. Thanks for your question, you may wish to consider having a veneer placed on the tooth. This is a thin layer of porcelain which can be used to alter the size shape and colour of your tooth. The shade of your veneer could be chosen to match your other teeth, hence creating a more uniform look to your smile. Although the costs will vary between practices, you should expect to pay between £400 – £600 for a veneer. Many cosmetic practices who offer these services, will also have the facility to offer payment options such as interest free credit to help spread the cost. I hope this will be of assistance to you.

Pain post tooth filling

Q. I’ve just had a filling – there was decay under an existing filling (spotted via X-Ray) and some of the original filling had to be removed to remove the decay underneath and then place new filling on top. It has been 18 hours since the filling and I am still very sore. Is this normal? It’s 4am in the morning and I cannot sleep because of the pain/soreness. I could not brush over the recently filled area before going to bed either as it felt very sore/sensitive. I have had fillings before and cannot remember the pain post-filling lasting this long. Thank you

A. Thanks for your questions, I am sorry to hear that you are suffering. I would advise that you return to your dentist at your earliest convenience to have the filling checked, as it is keeping you awake. It is possible that the filling may just need some slight adjustments, but I would encourage you to have it checked.

My discolouring teeth needs to be whitened

Dear Dr Caplan, have the fillings in my front front teeth an the look horrible i have had them since a child an now i am 23, however on my left hand side im missing a canine so i have a latral.An on my right i have a canine which has been made to look like a latral. both side have centrals. my right side has a root cannl which has started looking darker is there anything i can do ie whitening all my teeth or have crowns an these but i want my other teeth to look whiter to please help

Need extensive makeover to my teeth

Q. I’m 22 and my teeth are awful! I’m embarassed to smile, laugh and often get told to ‘cheer up’ because of my regular closed mouth expression. My teeth are discoloured, sensitive, and crooked. This is due to lack of dental care when I was a child. I regularly use whitening products, and sensitive toothpastes, I visit the dentist but just seem to have filling upon filling. I’m so scared of one of my front teeth completely falling out. I honestly feel like I would be happier with false teeth, which is a bit of a daunting prospect at the age of 22. Being a student, I cant afford cosmetic dentisty and even the financial plas seem to want £100+ a month, which just isnt an option for me. I don’t seem to have a local dentistry school, and even ones far away seem to have full waiting lists. I’ve considered using a student loan to go abroad to have ‘cheap’ dentistry work done but hav been advised this could make things even worse. What should I do?

A. Its very difficult to have extensive treatment on a budget. You might be able to get some done on the NHS which is usually cheaper than paying privately. You always take a risk when going abroad as often the cost isn’t that much different, especially as the pound is quite weak against the Euro at the moment. You might also have complications that require further visits abroad. I guess the best option would be to save up as much as possible and to start the treatment in stages perhaps using a finance plan to help spread the costs?

What are pros and cons of teeth filling?

Q. I had braces when I was 17. At the time there was an issue with overcrowding on the bottom teeth. There was discussion of pulling a tooth, but it was not done. I’m now 33 and have overcrowding of my lower front 6 teeth. My orthodontist suggested pulling a tooth or filing the teeth down. What are the pros and cons of filing the teeth down? Does this ruin your tooth enamel?

A. Slight adjustments using a techniwque known as interdental stripping helps to create the space in order to align the teeth. The loss of a tooth often causes an asymmetrical final result but might be your only option if the teeth are very crowded.

Tooth filling is causing pain while chewing

Q. Hi doc, I am 25 year olf female. I had a filling done at the end of the jaw a month ago and did tooth cleaning. But apparently after the filling in last teeth upperjaw 2 teeth adjacent to it have become sensitive and cause pain when i chew. There should not be cavity coz i recently had a dental checkup. Only 1 teeth was treated that i already mentioned Is it coz i did cleaning and that has hurt my teeth ?

A. The area may need to settle or you may have to have the bite changed

Can a chip in porcelain bridge repairable?

Q. I just had a 6 unit full porcelain bridge placed in December of 2009, I just noticed a very tiny chip in between my two front teeth. Is this repairable?

Need solution from veneer teeth grinding

Q. I have had 4 porcelain veneers done in Sept 2009 on my front teeth and wish I hadn’t now as have had problems in that after the permanent ones were placed on one came off after 3 days as it appears I grind my teeth at night. I know the veneers are slightly longer that original teeth which i didn’t agree too and so now everytime i talk, they ‘catch’ with the bottom teeth. I am concerened this catching will lead to other falling off prematurely so want them altered so they are same length as original teeth. Will there be damage to underlying teeth if these veneers are taken off to be replaced?

A. They can be replaced without causing problems but the first thing to do is go back to your dentist and see if the bite can be adjusted.  You may benefit from a nightguard that can reduce grinding.

My wife suffering with lot of pain after filling Is this normal?

Q. Dr Moore, My wife attended the dentist last month for a routine check-up. The dentist told her that she needed a filling and made another appointment for her to attend for this. later on, after having the filling done, she was havig a lot of pain. She went back to the dentist and he told her that he would have to remove the nerve from this tooth. My wife was still having a lot of pain. within the last week, her mouth & face has swollen causing her extreme pain. She went back to the dentist again and he removed another nerve from a tooth at the front of her mouth. I am very concerned that my wife has been suffering with pain for several weeks now as a result of attending the dentist for a routine check-up. Is this normal?

A. Complications such as these can occur after a filling, however it is important that they are dealt with correctly. Perhaps she should go back and ask him to explain what has happened in her case

Is its correct to place cap on cracked tooth?

Q. I have pain in one of my back teeth which had an existing filling. My dentist took xrays and advised of a crack. He has now cut away half of the tooth and is intending to place a Prcelin cap on it in two weeks time costing £650. There is still a large crack running down the back of the tooth that is now visible. It it correct to place cap on this now? Thanks

A. They are trying to b ond the tooth together and prevent the crack spreading.  If this works your tooth should be fine for many years

My filling causing pain What shall I do?

Q. I was told by my dentist I had a leaking filling. He has re-filled but my tooth is lose and I am still in alot of pain. I am unable to eat or drink as the hot or cold causes me pain. What shall I do

Hi many thanks for your email, if the tooth in question which was filled is loose then it is safe to say there is bone loss around the tooth and the tooth is infected probably from the bone loss and gum disease around the tooth. my advice will probably consider extraction of the tooth as i think root canal treatment may not work.

What would be the cost to replace silver fillings with white?

The cost of replacing each amalgam filling for a white filling will vary between practices, I can only advise that at my practice the cost of white fillings starts from around £80. The base of the cavity is bit dark after having molar tooth removed Is its normal?

What would be the cost for implants?

My two back teeth on the top left both have root fillings ,one has a gold crown being held in by a metal pin ! went to see my dentist today to be told that both teeth I will come out eventualy but because my jaw is out off line and clicks ,I need to replace them with i’ver plate or inplants .both teeth are giving me truble , I don’t like the sound of a plate as I am only 37yrs and really upset ! Is its necessary to hv my fillings redone?

Q. Hi there, I suffered from bulimia when I was 18 & 19, I’m now 36 and my 2 front teeth have several fillings caused by the bulimia. I’m happy with them at the moment, although the fillings did fall off and had to have them redone but my dentist has said that I should have crowns and that Lava crowns would be the best solution. He has said that I should have them on the front top 4 teeth. Is there a problem with me leaving it as I’m happy with the fillings and think that Lava means that I will have to have a lot of my real tooth filed down and think what is the point if it looks ok and isn’t really causing me too many problems.

A. If you are happy with the fillings you have, then the best option by far is to leave them as they are. If you want to change them in the future i dont see why you cant have them changed for fillings again or even veneers as these are both much less invasive than a crown.

What would be the reason for lock jaw pain Plz advice

Q. I was diagnosed with slight TMJ many years ago and it show up now and again with a lock jaw and difficulty when eating. I used to sleep with a guard on my upper teeth. Do you think it is worth having another guard made? Also do you think an old filling on the bottom molar teeth could be contributing to the lock jaw pain? I plan to be in London during the summer. Is its possible to smooth the tooth by polishing or buffing it?

Q. I had a white filling placed on one of my front teeth a few years back, I don’t think the dentist was comfortable with the bonding procedure, and since then the surface of that tooth is lumpy and uneven. Would it be possible to have a dentist “smooth” the tooth out by buffing or polishing it? Thank you.

Is its possible to replace amalgam fillings?

Q. Hi, I am 15 and have suffered from amelogenesis imperfecta my whole life. At the moment, all of my molars have stainless steel caps on them and most of my teeth are heavily restored with amalgam fillings. Apparently, these restorations were stronger when I was younger, but now I am getting bullied at school because it looks like I am chewing tin foil all the time! Is it possible to get these replaced with tooth coloured restorations and can this be done on the NHS?

A. It might be possible to get porcelain crowns on these teeth, ask your dentist to see what he might be able to do on the NHS for you.
