A. It is perfectly fine to have orthodontic treatment at your age. As with any treatment there are always s
Side effects to orthodontic treatment are very occasionally a nerve can die or a root can begin to dissolve. These occur extremely rarely and it should deter you from having the treatment done

Q. HI BEEN GIVING A PRICE FOR 22 CROWNS OF £9,000 IS THAT ABOUT THE GOING RATE? ITS THE BEST CROWNS CERMNIC.

A. hi many thanks for your enquiry, the cost of a crown varies from £300 to £1500 dependant on who does the treatment. if you have been quoted £9000 for a 22 unit case this means the average cost of each crown is £400. The more important question is to ask how many 22 unit cases has the dentist in question done, this enters into the realms of a full mouth rehabilitation and is more complexed than just preparing the teeth and fitting crowns. if the occlusion(bite) is not taken into account this can cause alot of jaw joint problems and pain which is then difficult to sort out.

hope this is helpful.

Q. I chipped my two front teet about 8 years ago and I have had them capped but because I grind my teeth a lot the caps are continuously being worn and chipped. I have a night guard but they are still really noticable when I smile. How can I fix this?

A.

You need to go to a dentist that has a knowledge of occlusion. This is the study of how teeth bite together and function in relation to the muscles and the joint. You may not actually be a grinder. If you were there would be wear facets on your posterior teeth as well or your bite guard would have marks characteristic of grinding on the surface.
It could be that the crowns were placed in a position that interfere with your chewing cylces and the chipping and wear is happening for this reason. It is very difficult to diagnose the problem without examing you. If you live within traveling distance of my pravtice I would be happy to see you for a consultation but if not then please follow my advice in finding an appropriate dentist
Kind regards

Q. Do you know of a treatment for when the front teeth top and bottom are set too far back and from the profile view the chin and nose protrude. I wore a brace when I was 13/14 and think that the teeth were pulled back too far. I have seen a local surgeon who really made me feel uneasy and wouldn’t speak to me privately and since then i’ve been really depressed. At school kids used to goad me about it. I am not confident when I smile and really would like to restore that confidence.

A. There are possibilities for treatment. You may be eligible for brace treatment again but you may also need a combination of brace treatment and veneers to give you the perfect smile. I the problem is skeletal, orthognathic surgery may be suggested. This is very invasive and involves reposition the jaws so that you have a better profile. If you have a consultation with a cosmetic/ restorative dentist they will also be able to refer for an orthodontic opinion. If you are unsure who to go then then any of the dentists on this website would be able to give you all of your options
Kind regards

Q. My father-in-law recently had an operation for cancer, which involved removal of some of the roof of his mouth, as a result of which he now wears an obturator. Following radiotherapy, his obturator now fits so poorly that he is almost unable to eat or drink, as everything comes straight down his nose. It also makes his speech difficult to understand. He has gone from 12 stone down to about 9, and is still losing weight – to the extent that everyone involved in his care is concerned about him, but his reconstructive dentist is too busy to anything about it. Is it possible to have an obturator made privately? What would the likely cost be? How long would it take? It seems absurd to cure a man of cancer only to condemn him to starve to death! I look forward to hearing from you.

A. hi many thanks for your enquiry. i do not have alot of experiance with this kind of device but i am more than happy to have a look at your father in law and look at the best treatment options that would be suitable for him

Q. Hi there, I chipped my front tooth some years ago and after various crowns falling off the ’stump’ that was left, the stump rotted completely and was removed. It was replaced with a metal rod to which a crown veneer (?) is now attached. I am happy with the appearance of the tooth but the gum above the tooth has got progressively more discoloured over the years. I am concerned about this as I haven’t heard that this is normal and it is understandably bothering me as it is my front tooth and so visible to others. My NHS dentist informed me that the grey was in fact the top of the metal peg showing but I’m not sure if I believe this as the gum actually looks grey. Furthermore, i’m sure that it has changed colour- going from a pale grey to a darker grey over the past few years. My dentist also informed me that I was being too aggressve with my teeth brushing and that the reason you could see grey was because I had brushed the gum back and that I should stick to a more gentle, circular cleaning motion in order to reshape the gum. I tried this over a period of several months- in fact I am still following his advice, but I can see no change. I want to know whether this is a problem you have encountered before? And if there would be any cosmetic dentistry service available to remedy it?

A. Thank you for your question. I am sorry to say this but you need to find your self a good private dentist who knows about cosmetic procedures and occlusion, ( the science of jaw movements and how teeth fit together).

From your history it sounds like you have occlusal problems. If your tooth chipped while you were eating something it was probably interfering with the lower teeth on chewing and too much pressure was being put on the tooth. I am assuming you have good oral hygiene in which case the failure of your subsequent crowns can also be attributed to this. The constant added pressure on the tooth will have caused it to weaken over the years and this is why you are now left with a root and a metal peg. To check whether your existing crown is getting too much pressure you need to place you index finger on the front face of the crown and tap your back teeth together. If you feel any movement of the tooth that is crowned is interfering with your bite. This moving sensation is called ‘ fremitus’. If this is the case then this crown will also fail and eventually the root will fracture and you will be faced with needing an implant.

I have a feeling that the crown is getting too much pressure. My reasons are as follows;

1) Your gum is receding. This has absolutely nothing whatsoever to do with tooth brushing techniques. When too much or the wrong types of pressure are put on teeth the gums can migrate upwards and recede because of the trauma. The gums are attached to the teeth and the extra pressure is thought to cause the tooth to flex around the junction of the enamel and root. It is good to use circular motions when tooth brushing but it will never grow back or reshape favourably.

2) The tooth seems to be getting darker. This can happen to teeth when continually traumatised. Having a metal peg in place doesn’t help situation.

3) You have experienced continual failure of that tooth and any restoration that has been placed on it.

You can definately get this problem resolved by finding a privatedentist that knows what he/ she is doing. White pegs can be used now and the new crown can be made of porcelain only and fabricated so that it is the correct position in your jaw
