Q. I had a crown on a back molar which did not fit correctly and is in the process of being replaced. I have a dull ache in the tooth area to the rear of the tooth. It is not the throbbing of a regular root canal type pain. An endodontist could not see damage to the nerve or root other than the poorly fitting crown which is now gone and a temporary in place. I am getting intermittent pain but am concerned about getting a root canal as a previou s root canal on a bottom molar left permanent nerve damage. What do you suggest?
A. The fact that an endodontist could not find a problem with your root would generally indicate that the tooth can be crowned. However be aware that despite the endodontists reassurance the tooth may still need to have nerve treatment at a later date- this usually cna be done through the final crown although a filling will then need to be placed or even a new crown.

Q. Hi there i have always had a great smile untill i staryed grinding my teeth at night a few years ago. now my bottom right molars are much thinner, and my upper right side is worn far more than the other side causing a crooked effect. can this be repaired?
A. Yes this can but often requires relatively complex treatment to reestablish your correct funtion. Once this is done you need to wear a night time splint to protect your new teeth as the muscle emgrams( muscle memory) will likely keep you grining your teeth

Q. my bottom left moler thooth hurts unbelievably. I cant sleep. the tooth was filled a long time ago. The bottom of my tooth has a grey line. my tooth started hurting about 4 days ago. cant stand the pain please help. and my wisdom tooth is growing in the back if that has anything to do with it.

Q. Hey I have a tooth that is a molar that is broken that was broken since about 8 years ago and its not infected but was wondering how my dentist would remove it as it is a baby tooth. basically I would love to hear many options and the thing is that all the sides of the tooth is still up but the middle is all gone. So I was wondering what kind of ways a dentist could remove a tooth like that?

If you go to your general dentist and they feel that they cannot take it out, they can refer you to an oral surgeon who will take it out with ease.

Q. I have relatively straight/level teeth all my life and never had braces however I’ve noticed over the last 3/4 years (I’m 27)that the 4 teeth on my lower jaw, between my ‘fangs’, have become twisted. My dentist says that’s ‘just what they do’ but I’m concerned that my 4 molars are pushing my teeth together – I have all 4 molars. Can you give me any advice/information? Thanks
A. Hi. The lower incisors often crowd together in the twenties as a result of late lower jaw growth and lip tension. The molar teeth are irrelevent in this type of crowding. You may want to catch it early and straighten the teeth with an Inman Aligner as they work very well and quickly for this type of mild crowding. Let me know if you can’t find someone in your area who does this brace. Regards An drew

Q. Hi, One of my top molar is senative if run my nail along the gum line on the tooth, i have just been to the dentist and had the filling replaced with a white one and the dentist said there was nothing else wrong with the tooth so why is it still senative if run my nail along it? do i need further treatment or how can i cure this i have the same thing on the same tooth on the other side as well. Thanks
A. You are just touching the root where the gum has shrunk away. this part is always a bit sensitive I don’t think you need any treatment for it but if it gets worse ask your dentist to check it. I would just stop sticking your nail into it!!

Q. I had a root canal treatment on my molar few years ago but now the crown over it has came off… how much does it cost to paste it back in…..thanks
A. If it will go back in we would charge £25 to glue it back in or £450 for a new crown

Q. 2-3 years ago, my last molar on the left side had a big cavity and reached the nerve. my dentist advised me to do a root canal but i decided not to and asked her to do a filling instead. i did a composite one, and another time when it wore down, and switched to amalgam the third time when it had to be replaced (just this year). now, part of my amalgam filling has chipped off. i am not sure how big the chip is in comparison to the cavity but i don’t feel any pain. i think my dentist will want to replace it again but i am concerned about the negative effects of drilling away the amalgam (it releases harmful mercury vapour). what should i do? should i still get the filling replaced? can the chipped part just be filled up with another filling?
A.
You need to ideally discuss your concerns with your dentist. The filling can be re-done or patched up. This is entirely your call. To patch it up, you can expect it to chip off again- and having it re-done may be the best option- but still not guaranteed to NOT chip again. Make sense? If you have concerns regarding the mercury vapour, then discuss this with the dentist. If you have a new amalagma filling, then not much of the amalgam has to be removed in order for more to be added. In order to do a composite filling, then ALL the amalgam has to be removed. It can also be patched up with a materila called glass ionomer that will repair the filling. If the tooth is heavily filled, then it is always advised to have a crown for protection. Hope this has helped.

Q. Three of my molars (both upper and lower) have cracked so much fillings would be useless. One has splintered to the point it’s almost buried in my gums. This affects my jaw alignment and chewing – what’s a longterm solution for restoring them, please?
A. It sounds as though the teeth are probably unrestorable- I cannot be sure though, as I haven’t assessed them. Perhaps the best option would be to have them extracted and have dental implants placed as a long term solution. I would definately advise though, to see your regular dentist to see what options are available to you. Hope this has helped

Q. I have had intermittent pain in my upper teeth for some years.My Dentist and I could never clearly identify which tooth the pain came,whether it be from either a heavily filled molar tooth or a crown with a root filling. Last week the pain became constant,pulsting and throbbing.I visited a emergency dentist who removed the root of the heavily filled molar and put a temporay filling in.However four days post this ,the pain is as bad as ever.The emergency dentist took xrays that didn’t show any infection. Could the pain come from my crowned molar with the root filling in? The pain is getting me down but I am keen to hang on to my teeth.To compound the issue I am holiday at the moment so can not get to my regular dentist Any ideas on the cause.Thanks
A. unfortunately it is impossible to say what the cause could be without an examination of the mouth. I would recommend you see a dentist immediately.

Q. My gums around my upper right molar tooth have been swollen for a while, but the area is firm. They become more swollen when I do not get much sleep, but I noticed today that the size difference between my right and left is significant. The area is a lot larger. I’ve already gotten my wisdom teeth taken out. Could it be an infection in my gums or another molar tooth growing out?
A. Its likely to be an infection, get your dentist to check it to see what it is

Q. After crown preparation on a back molar I have slight pain when I bite and it is sensitive to cold. My question is should I ask for a root canal before the crown is placed next week I did have an xray and was told I did not need a root canal. The crown is needed due to a large amalgam filling that showed cracks.
A. if its only slight pain then these symptoms are normal, as you probably have minor leakage of the temporary, in which case, root canal is not needed.

A. 1. it is possible to have an impalnt, but an examination would be required to dermine this. 2. good oral hygiene and diet are required. Also regular visits to the hygienist 3. depends on the circumstances

Q. Hi I would really appreciate your help. This afternoon I was having a cold drink when I experienced a sharp shooting pain in a molar I had filled a few months ago. I also experienced some sensitivity in another filled molar but not to the same extent. All this was accompained by a general ache in my teeth. This morning I had no pain and drank cold drinks with no problem, do you have any idea why the pain suddenly came on just a few hours later? I chewed a lot of gum today which I haven’t done for a year or so. Could I have chipped the filling? I checked in the mirror and there doesn’t appear to be any holes but maybe I missed them? I don’t really have the time or money to go to the dentist this week but I am desperate to sort this problem as it is my birthday this weekend. Any help would be really appreciated. Thank you
A. Hi there,
Thanks for the enquiry.
Firstly, it sounds as though you have sensitive teeth, as the pain that you described is often associated with this complaint. (sharp, shooting pain that comes and goes)
Although the enamel on your teeth is hard, it is not uncommon for it to be worn away through grinding, friction, clenching, injury, acidic foods and drinks etc. The enamel is worn away and exposes a layer of dentine that sits underneath. The dentine consists of of minute tubules that react to hot, cold, sweet that causes the twinges. Over time, the dentine forms a secondary layer that will protect the nerve and the remaining tooth structure- almost like a self-healing mechanism.
I would however, go and see your dentist to check that it is nothing too serious. You may need a filling- or have a filling replaced if there are any leaking margins around it.
In the meantime, I would advise that you use a toothpaste specifically designed for sensitivity- ie sensodyne or colgate sensitive.

Q. Hi, 2 months ago i had a root canal treatment. The tooth, molar (front) was also in decay. A large filling in the centre of the tooth was put in place after the RCT. Yesterday, whilst eating, a half section of the filling came off and one side of the tooth(outside) where the RCT was done felt wobbly. Today it is more wobbly. The Inner side also feels wobbly. The RCT treatment cost nearly £300. I guess the next option is either take it out completly or fit a crown. More expense I cannot afford. what would you recommend
A. If the tooth has fractured a long way under the gum, then extraction would be the best option, it maybe that the tooth has broken level with the gum or just under, in which case it could be refilled or crowned. Ask you dentist for his advice.
