Q. Hi, I got braces about 5 years ago and had them removed like 2 years ago. Before treatment i had to get 4 teeth removed, the 1st bicuspids. I guess my mouth is too small for my teeth. Since i had my braces removed my top front teeth have been shifting and my bite has changed as well. What I hate the most is the front teeth because they’re moving back to how they were before, like a V shape sicking out. (I used to suck my thumb as a child) So i decided to see another orthodontist since mine couldn’t really help me and just told me to press on my front teeth while watching t.v.! I just started treatment with the new orthodontist and he recommended invisalign for my top teeth and put in one of those glued on retainers on my bottom front teeth so they wont shift anymore. ( they’re pretty straight) I’ve only been wearing the invisalign tray for 3 days now but i notice my jaw is in pain on the right side. Is it because of the invisalign??? I don’t want the discomfort to get worse. Also, i dont know if this is an issue, when i had my impressions done for the invisalign, he also shaved between a couple of teeth in the front so that there would be more room for my teeth to move. So during the few weeks that I had to wait until i started my treatment, I felt my teeth and bite shift slightly since he shaved them and made room. So now when i’m wearing the invisalign its changing my top teeth to fit what it WAS a few weeks ago, And the bite on my bottom teeth are different since they started to shift. Is this an issue??? I hate that i’ve had to go to another orthodontist and pay alot more money for more treatment ( 6 months ). I also apparently have 5 wisdom teeth that have not been removed. This new orthodontist did not say anything about removing them. Would my wisdom teeth be causing my teeth to shift more? should i get them removed ASAP? or will i end up having to get even MORE treatment in the future once they’re out? I’m sorry for the long e-mail, i just need some advice. THANK YOU,

A. Orthodontics is a treatment for not just correcting your teeth to be straight- but it also corrects the way you bite together- into a more ideal position. I’m almost sure that the invisalign is doing what it needs to do for your case. Obviously, moving teeth creates minor discomfort that can usually be relieved with standard pain relief such as paracetamol. The filing in between the teeth is common in orthodontics. It is called interproximal reduction (IPR) it takes a very minimal amount of enamel away from the teeth to overall create enough spce. This avoids having to remove teeth unneccesarily. Invisalign is a very good orthodonic option. It may however take a little longer than your usual conventional brace treatment.I stress though, that you need to wear some form of retenion after your treatment has finished to prevent the rebound movement of teeth. This can be fixed- like you are going to have in your lower teeth- or it can also be in the form of a removable retainer.I hope l goes well.It is important to maintain a good relationship with your dentist so that you can feel that you can stress any concerns at any given time.Hope all goes well.

It happens very sporadically with months between attacks and no obvious cause beforehand. For some reason it has suddenly occurred but worse than usual, much worse. The left jaw joint is so painful if I try to open my mouth or even swallow, that I cannot eat anything requiring chewing because it is excruciatingly painful. Also, I cannot open my jaw very wide, as has happened before. What is new is the degree of pain when trying, it feels like I have suffered a blow to that area – a hard one! So far I have not sought treatment, nor ever had a dentist comment on anything being wrong with my bite or jaw alignment. What might be wrong and what’s the correct course to take? I was wondering what can I do? as my dentist (despite visiting regularly) has not sorted or even attempted to provide me with information on what is available or what work needs to be done to put these problems right. These problems are holding me back to an extent, and I believe that if they were put right I would be much more confident.,

A. Firstly, I would reccommend you have a good dental and jaw examination. There are a few different options available when it comes to straightening teeth. If you are concerned about the aesthetics of braces, there is an Invisable brace that is available – though ont everyone is suited for. You may want to see an Orthodontist so they can advise you on the best method.Justin glaisterwww.umbrellasmiles.com

I am sure he said that this is available on nhs. i am now wondering if i heard correctly does this sound correct? Thanks

Q. Dear Dr, I had two crowns put next to eachother on the lower jaw at the far back a year ago. While I was waiting with temporary ones they came out a few times and so I had to have them re-done and on the last occasion there were a couple of days with nothing on them. Ever since I always get meat trapped between the two crowns which can only be removed by flossing. This is awkward if I am out and so I have to have a vegetarian option or be careful to chew on one side only as it often feels uncomfortable with the meat stuck. My dentist says that nothing can be done and that he has the same problem. Is this correct? The original tooth was badly broken.

A. You may not need surgery. Your joint can often be treated with bite splints followed by equilibration of your bite if required. However a full examination to assess your particular problems is necessary before a proper diagnosis and treatment plan can be decided
