My tooth keeps getting infected please look into my issue and suggest.

Thursday, November 26th, 2009
Q. I have had a root filling and a gold cap put over the tooth. The tooth keeps getting infected and it may be better to have it out. I have a 5 tooth bridge one tooth away form from the cap and if nothing is replaced it could undermine the bridge. Ive been told that a good option is to have an implant but the cost is 2,000 which i cannot afford can it be done chaeper or have you got any suggestions?

A. That is a reasonable cost for an implant.  The only other option would be a denture if there is not a tooth either side of the gap

My second root canal has gone horribly wrong

Monday, November 9th, 2009
Q. I had a root canal done on a mandibular 2nd molar 5 days ago, was already being treated with antibiotic as a precaution. After the novacaine wore off I was in a great deal of pain. Took vicodin and antibiotic as prescribed, called the dentist the following morning. He switched my antibiotic to a stonger one and said he would see me next week. I was awake all night tat night again in pain, called his emergency number, now accompanied by swelling an an inability to eat due to pain. He returned my call suggesting that I make an appt after the wknd was over to come in to have the tooth extracted. I am 44 years old, female, in good health. This is my second root canal and experienced nothing like this with my last one. Even the exterior of my face is painful when touched. Any suggestions on what has gone wrong or what I should do?

A. Sometimes an infection is so severe it won’t clear wth root canal. In these cases sometimes removal of the tooth is the only option. You could also ask for a referral to a specialist in root canal , an endondontist.

Infected front tooth is drilled and treatment is causing lot of pain

Friday, November 6th, 2009
Q. I have been suffering wit a dead front tooth (top) for over a month. The dentist has drilled a hole in the back of the tooth, and initially left the tooth open in order for the bacteria to be killed off by the oxygen. This alleviated the pain initially. The tooth was then padded with soaked disinfectant on cotton wool. Following this after a week or so the tooth was repacked but left open. After the following week the tooth was packed with cotton wool and then sealed with a temporary filling. This felt ok for a while and then the pain was too great and so I removed the temporary filling and the padding and left the tooth open. This alleviated the pain once more. The tooth has now been packed again and sealed over with a temp filling and the pain at this point is quite bad with pain now at the gum and painful to pressure. Should I leave the filling in place or remove it and let the air in again? Is there a better method, this all seems a little ludicrous for this day and age!!!

A. You may need to consider removing the tooth if the infection doesn’t clear soon. Alternatively  you could ask your dentist to refer you to a specialist endodontist, they might have more success in saving the tooth and completing the root canal

Can infected implant be treated?

Tuesday, October 27th, 2009
Q. My question is this, I had an inplant put in next to a 3part bridge. Unfortunately it did’t tak. A few months later we tried agasin, and again it didn’t take. As the second implant was being put in, I could feel my brain rattling and that is no exageration. I left the office and my whole right side of my face swelled up, to the point that I stayed in my bedroom for 3-4 days in the dark. I was on antibiotics and pain meds, but nothing helped. My doctor came in on a sun to see me and I didn’t have parmacy coverage so he gave me a left over antibiotic from his sons. Now my bride was damaged and cracked, and I had to have my teeth pulled due to such infection under bridge. Also my implant on left side is abcessed. What should I do?

A. You need to see your dentist and discuss your way forward

When does the gum become tender and white?

Tuesday, October 27th, 2009
Q. I have had one place on my lower front tooth that has been receded for 10+ years. When I first noticed it I started using the Sonicare toothbrush and have used one twice a day since that time. I have obviously purchased new ones over the years, and do not let the head of toothbrush get old and frayed. My overall dental health is excellent, my gums do not bleed when brushing or flossing. Recently (approximately two weeks ago) I noticed the gum area below the tooth was very tender and when I looked, I saw that it was extremely white all around the base of the tooth. I had been using one of those peroxide whitening mouth rinses once a day for about one year. I thought it may have been related so I stopped using it. Since then the area is still very white, with no pain (unless I mess with it.) The gumline below the white area is somewhat more red in color that the rest of my gumline, but again no tenderness or pain. I do not smoke, have never chewed (I am a female)I drink about 3-4 cups of coffee per week. I take a blood pressure med once a day and am 51 yrs. old. Should I be concerned and see a dentist or am I being an alarmist?

A. Go and see your dentist for them to assess the situation

What can be done to prevent erosion?

Thursday, October 22nd, 2009
Q. Hello Dr., I read about enamel erosion on front teeth and I believe that this is my case, i have a few yellowish/transparent spots on my 2 front teeth. I read from one of your replies that I can’t restore the color with diet or habit, however what can I do to prevent more erosion and to at least keep it as is. thank you

A. Check your diet for acidic foods particularly fizzy drinks diet or otherwise. This is the cause of most erosion so reducing this will help prevent further enamel loss

Could you suggest some options to prevent my teeth going yellowish?

Tuesday, October 20th, 2009
Q. HELLO DR MOORE, I AM A 40 YR OLD FEMALE I DO NOT SMOKE, DONT DRINK RED WINE OR COFFE. BRUSH MT TEETH TWICE A DAY AND FLOSS BUT MY TEETH HAVE STARTED TO GO OFF COLOUR (YELLOWISH). IS THIS PURELY AN AGE THING. I AM SCARED OF BLEACHING AS I DONT WANT MY TEETH TO GET SENSITIVE. I HAVE HAD SO MANY PROBS THIS YEAR WITH MY BACK MOLAR (TOOTH WAS DYING). I VALUE YOUR THOUGHT.

A. Bleaching can cause some transient sensitivity but on the whole is a very good way to improve the colour of the teeth with a minimum of fuss and expense

My front teeth hv started to ache Could this be Sinus?

Monday, October 19th, 2009
Q. HELLO DR PATEL. I HAD A ROOT CANAL ON A LARGE UPPER MOLAR. IT HAS NOW STOPED HURTING. I CAN EAT PAIN FREE ON IT, TOUCH IT PAIN FREE, TAP IT PAIN FREE. HOWEVER IT FEEL WEIRD LIKE THERE A SMALL AMOUNT OF PRESSURE IN IT . MY FRONT TEETH HAVE STARTED TO ACHE SINCE THE ROOT CANAL. MY JAW ALSO ACHES NEAR MY EARS. I HAVE HAD THE BITE ADJUSTED AND ITS PERFECT. COULD THIS BE SINUS? KIND REGDS.

A. If there was infection it could take a while to settle it sounds like the symptoms are reducing so I would wait until everything is pain free.  It could be a sinus infection or the fact that the bite was too proud.  Leave everything for at least 10 days unless symptoms get worse.

What is the best way for root canal treatment?

Monday, October 19th, 2009
Q. Two years ago I went to the dentist for my regular scaling and check up. My dentist told me I had a cavity and thus would need a filling. I got my scaling and my filling done that day. He then told me to come back if I feel discomfort in my just-filled tooth (which I thought was weird because fillings aren’t associated with post-procedure pain). My tooth did hurt a bit, but I thought I’d survive, so I didn’t go back to my dentist for about a year and a half. Now (it’s been 2 years since I got the filling) I find myself getting a root canal. Can I please know exactly why my tooth got so sensitive, when, before the filling and discovery of the cavity, my tooth was absolutely fine (feeling wise). I feel as if the dentist I got my filling from did something wrong. That tooth was sensitive to hot and cold things from time to time, but I could survive. Was is a good choice to have got a root canal anyway? I am regretting having done this, and now I’m wondering if I really needed it. Thank you, Jessica

A. Teeth that have decay in them can often be symptom free.  That is why dentists take Xrays to confirm that there is no hidden decay.  When the decay is removed it can sometimes be close to the nerve of the tooth. When a filling is placed in this situation the nerve inside the tooth can still die and give you pain.  It does not sound as though your dentist has done anything but the correct treatment for you and it is unfortunate tha the decay and bacteria had infected the nerve of your tooth

How can I get rid of ulsers Plz advice

Monday, October 19th, 2009
Q. Recently had four inplants, prungs, on the front bottom. I am getting ulsers or sores on back side. How can I get rid of the ulsers to see if it the dentures I am wearing. You see, I had to wear my old dentures a week after the inplants. And could not take them out. The drilled holes in them to take the four prungs. I recently had new dentures. They told me try them for awhile. After the ulsers heal, we can see if the dentures work or fit. How can I get rid of these ulsers. I take them out at home so air hits the ulsers/ Rince my mouth out with warm salt water aand even rinced with peroxide… yuk Please advise. I had the dental work at aaffordable dentures.

A. You may need to get the dentures eased away from the protrusions a little more as they are probably rubbing.

I hv fixed braces and its causing pain Plz advice

Monday, October 19th, 2009
Q. Hi. im 26 and at the moment im having some real problems, that is really making me very depressed. last year i was asked to be referred so i could have fixed braces as i was having alot of pressure in my mouth which i was told was due to a over crowd. i was fitted up braces but im a full time student at university and have two young children, i cant afford to have these braces as much as i want to. i have noticed these past few month that my teeth are pushing forward which is causing me a lot of pain, my bite has changed and im now having to hold my jaw at an angle due to this. is there any way i could get any help with having these braces fitted? or would i be able to have them on the nhs? it is really ruining my life at the moment and affecting everything that i do. i have constantly got aches and pains in my mouth, my teeth feel so tight has they are pushing with no where to go, and it also causes headaches and pain at the top of my neck. im at my wits end with it. if u could help me it would be great.

A. I doubt you will get braces as an adult, on the NHS. I also doubt whther they will help your symptoms. i would consider a bite guard worn at night to try to stop you clenching or grinding your teeth and to realign your bite as this is probably the cause of your facial pain. If money is a problem you might try a referral to a local dental teaching hospital?

Why to hv full root canal procedure when there is no infection?

Monday, October 19th, 2009
Q. #14 molar had cold and sweet sensetivity. USC Dental school removed decay close to pulp, pulled nerves, filled cavities, put temp crown. told there is no sign of infection. If nerves are already pulled why go to the proceedure and cost of full root canal , Why not return for full crown only, Cavity was deep soft dentin close to the pulp i believe was the cause of pain

A. Nerve removal is root canal treatment and is often carried out over a number of visits.  If the infection has penetrated the nerve it can a while to to diagnose it completely.
