Replaced tooth is causing gum to discolour

Q. HI Dr Patel, I ‘ve broke two front teeth years ago, I replaced those now my gum is gettng black, I can’t smile anymore is horrible, what shoul I do? Should i send youa picture before I make any appointment? thanks

A. You are more than welcome to send me some pictures.  The crowns you have had may contain metal which can leave grey / black edges.  One option may be new restorations which are metal free.

Dental treatments to fix gum recession issues

Q. Dear Dr Patel Despite regularly attending my dentist and hygenist for twenty years I have now been told that gum recession on two lower front teeth is now so bad that I am not a candidate for gum grafting. As you may appreciate I am very annoyed with my dentist for not being more proactive over my recession issues given that I raised this with him from day one of attending all those years ago. On the positive side, radiographs show a substantial amount of bone remaining and I have no mobility whatsoever. My questions are these: Even with such severe gum recession would it be possible to place implants if required in the future? Are there any techniques for gum to be ‘built-up’ to accommodate implants after tooth extraction? By the way the (very) little tissue that remains is healthy and recession has been caused apparently by crowding, thin tissue-type.

At my practice we get sent alot of people for implants who have very little bone , it is something you can have assessed. I am sure you could have implants , if the bone is short it can usually be added to.

Gum recession has made me depressed and want to consider plastic surgery

Q. Andrew, I am 24 years old and i have mild gum recession at the top and bottom of my mouth, the front 2 top teeth and the bottom 4 teeth. This is making me very depressed and stressed i have considered plastic surgery to help orevent this from getting any worse, would i be able to do this? What sort of cost would this incur? I feel some funny clicky popping feeling under my gums plz advice

Q. Hello Dr, For the past couple of months (even in between Dentist visits) I have a feeling on my gums. Its a funny clicky, popping feeling as if air was getting under a layer of my gum and then requires for me to press down to pop it away. I hope I’m explaining myself right. I feel so frustrated since I do a lot of public speaking and I’m so afraid of loosing my teeth. My husband calls me paranoid, but what do you think I’m experiencing. At times the feeling last days. Sometimes its a little painful, mostly uncomfortable but mostly frustrating. One dentist ranked out gum disease. I brush three times a day and after i brush the feeling subsides for a while until I eat, chew gum the noise comes back. I even feel it behind my two front teeth’s upper gum. HELP!

A. It is very difficult to give you a diagnosis without seeing you, if you feel your dentist is not examining your problem please find a good dentist or periodontist (gum specialist) so you can have a full examination.  It sounds like the gums may have lost some of their “attachment” to the teeth.

My gums have receded to the extent would lumineers be the way forward?

Q. I have 15 year old veneers on 8 teeth and my gums have receded to the extent that the old scraped down teeth is now visible behind the veneers and is ugly and extremely sensitive. Unfortunately I had one replaced by my dentist last year and it looks even worse, he had to shave half of it away so I could close my teeth, not a great job, and I am terrified of the dentist!! Would lumineers be the way forward for me, and how expensive are they? Thank you

A. Lumineers are a type of very thin veneer more suitable for people who have never had treatment on their front teeth so I don’t know if they would be the best option for you. It sounds like you just need the veneers all replaced with good quality veneers made using an experienced dentist and technician.

Low self confidence due to gum issue causing bad teeth

Q. I am very worried about a friend of mine who is very unhappy about her teeth. She has had severe gum disease and has lost some teeth and although the gum disease has now gone her gums and teeth are not in a good way. She can not afford the cosmetic dentistry that she needs and this is now impacting on her life. She has become very self conscious and has lost a lot of her self confidence. Is there any funding available to help her?

How does diabetics influence implant surgery?

Q. Hello I am type 2 diabetic ( controlled at present with diet and excercise regime ) I have lost teeth to upper left 3no and 2no to lower right. I am considering implant replacements . Would my diabetes compromise implant surgery . Also if positive what type of implant system is the best ?

A. Uncontrolled Diabetes is a contraindication to surgery and does adversley affect the predictability of dental implants. care should be taken if you lost your teeth due to gum problems.
Most of the main systems have “Bioactive” sufaces like Osseospeed (ASTRAtech) SLA active (straumann) and Nobelbiocare, Biomet etc, I mainly use Astratech dental implants because it is proven to maintain the surrounding bone in later years.

Tooth bridges has caused lot of problems

Q. My 23-year-old daughter has a 6 tooth bridge across one incisor to the other. This was because she was missing 2 laterals. This was done instead of implants because of severe bone loss of the missing laterals and not enough space for implants. She did have braces but unfortunately had an orthodontist that didn’t know what he was doing, causing these problems. An excellent prosthodontist suggested either redo braces and gum grafting or a bridge. For monetary reasons and my daughter not wanting go through this all over again for years she chose a bridge. This is when she was 19. Is it possible for her to get implants and crowns or once a bridge, always at bridge?

A. It doesn’t sounds like anything has changed regarding the space or bone for implants and now the teeth have been trimmed (i presume) it isn’t really worth considering replacing everything. If the bridge starts to fail then the situation may change and implants may become an option then.

I have a 40 year old root canals with discoloured and fragile teeth

Q. Dr Tennet, My upper two front teeth have 40 year old root canals. What is left of the teeth themselves is a very fragile & dark brown. What is the best cosmetic choice for covering them? I currently have porcelain crowns with white (titanium?) cores. They are ok, but not super. Any other options? Also, the gum above one of the crowns protrudes a bit more than its neighbor. That developed after the crowns were replaced a few years ago. My doctor has x-rayed and it doesn’t show an infection. I’m guessing a lot can go wrong with old dental work? Any advice would be appreciated

A.

Discoloured Root filled teeth can be a problem to shade match All ceramic Zirconia cores are a good route but the level of opaque will need to be judged by trial and error it usually requires a few visits. The crown margins should be about 1mm below the gum line to hide the dark root emerging. If this is not possible then some periodontal plastic surgery  to reshape the gum using grafts  / emdogain etc might be an option.

I have been misleaded on the cost of implants

Q. Hi Due to gum disease I lost all of my teeth last year. I would like to know how much it would cost to have full upper and lower implants. i have been quoted from the ridiculous £40,000.00 to the even more ridiculous £6,000.00 (some difference) this is a procedure i will definately be having as soon as possible. Can you please let me know what the correct cost would be? Many thenks

A. Its very difficult to give you a quote without seeing you and assessing the amount of bone you have, how many implants you need and how many teeth to finish things off. As a guide, assuming you will need some bone rebuilding we would charge in the region of £12-15,000 per jaw for 12 fixed teeth held with 8 implants.
let me know if you would like me to take a look and give you a better idea of the costs involved

Could nightguard be possibly causing pain in my gum?

Q. I have been trying to wear a nightguard /splint. But it’s causing me pain especially on one side where I have a bridge. In the morning it seems like the pain is in the gum under/behind the bridge area. It’s like one side feels alright but this side isn’t. I was hoping maybe you could shed some light on why this might be happening. Your thoughts on this are much appreciated. Thank you.

A. I guess the splint is either rubbing on the gum or causing pressure on the bridge. pop back to your dentist and ask him to adjust it

Is gum contouring done along with veneers treatment?

Q. I beleive I may need 1 or 2 implants to replace 2 vineers I already have. I would also like vineers on the other 6 front teeth to perfect this. I need some gum contouring on the right side as my gum shows to much when I smile. Could the gum contouring be done before or after the vineers/implants or would the contouring have to be done in conjunction with the vineers/implants? I am also considering going to Poland to save money rather than Harley st. Whats your professional opinion on this ?

A. The contouring is normally done at the same time as the veneers. You could also consider going to a dentist outside London rather than Harley Street that tends to be quite expensive rather than travel all the way to Poland

Can veneers be fitted if one has gum disease?

Q. I suffer from severe teeth grinding and i also have bad gum disease although i am only 34 and have never had a filling. i went away travelling and have not seen a dentist for over 5 years. i would like to know if i am treatable and what my options are for a smile makeover as my teeth are now yellowing, there is some movement and the gums are tender sometimes, I have started to become very self conscious and upset about it. Can veneers be fitted if someone has gum disease, what costs am i looking at or do you think i need false teeth?

What are the factors to be considered before having dentures?

Q. Hi I would just like to know about the metal chrome cobalt denture -the factors that need to be considered before the denture is prepared. thankyou

A. This seems like a question a dentist would ask.  Broadly speaking you need to assess the patients general health.  Then local factors such as bone pathology, gum health, tooth health.  Finally design needs to be considered such as path of insertion, clasping, pretreatment tooth adjustment etc

Could you suggest any other method other than opalesense whitening and gum shields?

Q. I have been using the Opalesence whitening and have been wearing the gum shields and using the gel for over a week now every other night. However my teeth are extremely sensitive at the minute and I am considering other methods. I dont notice much difference in my teeth either.

A. Hi many thanks for your email, the whitening will work it is however a time thing. if the teeth are sensitive i would use the follow regime:

do the whitening twice daily for an hour each time for two weeks you will see a definate difference.

Can I have silver fillings to be replaced with composite materials in molar tooth?

Q. I thought of going to have my silver fillings to be replaced with composite materials in one of my molar tooths. But some of the current silver fillings stick to my gums, how does the dentist going to remove it? Will it cause my gums to bleed during the drilling? I am concern that the harmful silver particles will get into my gums.

A. Hi many thanks for your email, the silver fillings that are placed close to your gums will still be removed and can be replaced with the white filling material. the silver filling material will not harm your gums.

Need to fix receeding gum for a nice smile

A. Yes it can!  You may have gum disease but you may just need a brace to move your teeth into a more stable position – teeth keep on growing if they are not in a stable position
