What can be done for lost tooth likely due to bone disease?

Monday, March 15th, 2010
Q. I had a tooth extracted last week. It detached itself from the jawbone when I bit on something fairly soft, and I understand this could indicate that the bone is diseased (I’m a 58 year old female). As I live in Germany, I’m due to see my dentist in a week. What issues should I discuss with him? Thanks, in advance Susan Lawson,
A. You should ask him to check all the other teeth and gums to check whether there are any other teeth that need treatment.

Can I get veneers if I have gum disease?

Wednesday, March 10th, 2010
Q. can you still have venners fitted if you have gum disease,
A. No you need to get the gum disease sorted out first. Once everything has settled then veneers would be a good way to improve the colour and shape of the teeth

Can I get teeth removed and replaced with bridge due to gum disease?

Thursday, March 4th, 2010
Q. i have a large over bite and my upper teeth are very lose. I tried to have my dentist put braces on them but was informed that due gum diseas it would not be a good idea to have braces because i would lose them anyway. Could I have those teeth removed a get a bridge instead,
A. For you to get a bridge in the first place you require surrounding teeth to replace the gap, which the bridge holds on to. I suggest you get on top of your periodontal condition (gum disease) by regularily seeing a hygienist or periodontist and then I would put braces on after your gums are stable. Justin Glaister

Can a mobile tooth be crowned?

Tuesday, March 2nd, 2010
Q. what does a dentist have to assess in my mouth prior to having a crown? can you crown a ‘mobile’ tooth?,
A. I would never crown a mobile tooth. A mobile tooth means there is either an infection at the base of the root or that you have gum disease (periodontal disease). Before commencing with a crown I would go and see your dentist about these conditions. Justin Glaister

Can I have all teeth removed and replaced with implants?

Wednesday, February 24th, 2010
Q. hi i really want and need all my teeth removing as they are in a terrible state due to many reasons, is this possible and will a dentist do this? also i would like in there place dental implants. do you have any idea how long this whole procedure would take, and any suggestions and ideas of the price? also i do have gun disease, and i really want to get rid of it fast but it is bad, any suggestions? any help at all would be extremelly grateful many thanks :],
A. If you are losing teeth due to your gum problem then you need to see a periodontist for many visits to get on top of your gum disease. Losing teeth is the worst thing that can happen in dentistry and no dentist likes to pull out teeth. Also if you have gum disease then the chances of having implants are pretty slim. Is wisdom tooth the culprit for moving teeth?

Thursday, February 11th, 2010
Q. Hello, I have had really straight teeth my entire life, up until the past year I have noticed my teeth dramatically moving. My right front tooth on top is twisting and turning out. The bottom two teeth are turning into each other. I am 22 years old. I have not had my wisdoms taken out. I was wondering if they are the culprit, and if that is right when I have them removed will my teeth go back in place?,
A. The wisdom teeth are not the culprit.  You need to see your dentist to have your gum health checked out in case you have severe gum disease – it sounds worrying that your teeth have suddenly moved and you should visit your dentist as soon as possible

Are dental implants a possiblity if gum disease is sorted?

Monday, January 25th, 2010
Q. hi i have suffered with bone loss through gum disease since i was in my thirties. i am now 43 and the appearance of my teeth do bother me, although i have not yet lost any teeth.why can,t bone be grafted before losing the teeth ? what , if any reasons, would prevent me from having successful implants in the future, and could i begin with my dental implants before the teeth look any worse ? lastly, how many implants are needed to replace 20 teeth, and what would they cost , including bone grafting ? i am grateful for any advice as this problem is affecting me greatly yours gratefully sandy,
A. Broadly speaking you need to have your gum disease treated before you can have implants.  The treatment can be sequenced so that you always have teeth.  The cost varies greatly so I am unable to give you costs but the options vary from dentures attached to the implants to permanently fitted bridges

Can I achieve a nice smile as I have gum disease?

Thursday, January 21st, 2010
Q. I am a 39 years old and have been so unhappy with my teeth for many years, and lack confidence with my smile. I do not have any fillings however, i do suffer from gum disease where i have receding bone on my teeths. I would love to have a georgous smile where my teeth are levelled and white. I have researched Vaneers and straying more towards Lumineers…What do you think? Is there any hope for me? IS it expensive to do? I feel I am quite elegant and love dressing up but as soon as i smile i feel weak.,
A. Lumineers and Veneers are suitable for certain cases and you would need to have a full consultation with a cosmetic dentist for me to be able to advise you on this. You are welcome to book an appointment at my clinic (020 7612 9810).Kind regardsDr Justin Glaisterwww.umbrellasmiles.com

Is gum disease root cause of my problems?

Thursday, January 21st, 2010
Q. I have just been diagnosed with advanced gum disease. I am 39. 6 years ago I had a child and subsequently was diagnosed with hypothyroidism. The gum disease has accelerated amazingly in the last 3 years and although the tests appear to show that from taking thyroxine, my thyroid function is normal, I have been arguing that I feel anything but normal. I recently paid for provate tests which suggested I also have adrenal fatigue and recent blood tests revealed the presence of thyroid antibodies. I now also have urticaria – research suggests this could be linked to auto-immune disease. I wonder if all these (and many other small issues including sinusitis) could be linked to auto-immune and whether that has a link to why the gum disease has advanced so quickly……..can you help – I have had a real problem getting the doctors to consider any link – a 5 minute appt will only deal with one issue! My dentist is going to refer me to a specialist but I am concerned that once again I will end up being treated but not dealing with the route cause. Thanks,
A. Have you seen a Periodontist for your gum condition? I would strongly reccommend you to see one. We have one at our practice www.umbrellasmiles.com.Justin Glaister

Why have I been told I have signs of gum disease?

Wednesday, January 20th, 2010
Q. Hi! I went to my dentist recently and he told me that i was “starting” with gum disease in my bottom left corner. My teeth do bleed when the dentist cleans them and a little when i floss, other than that my teeth are fine. He didn’t seem alarmed at this, but it alarmed me! It sounds really bad! I asked him if i actually had gum disease and he said no, but to take care, visit the dentist every 6 months and floss etc. I had a baby just over a year ago and my teeth started bleeding more when pregnant. Im scared now that i am going to lose my teeth and have an electric tooth brush, floss 3 times a day, brush 3 times a day, and use antibacterial mouthwash twice a day. My parents have recently had dental implants due to gum disease but they both smoked heavily all their lives and didnt take care of their teeth. Is it hereditary? Im scared to death! Also (im not sure if im being paranoid) but i think a gap between my 2 front teeth has appeared slightly and im worried this means my gums are residing? Do gaps appear for any other reason? Im going to visit my dads private dentist in February but wanted some advice now as im panicking a bit! I know gum disease takes time to get worse but im terrified of losing my teeth! Thank you very much! Oh and i dont smoke and never have! im 24! Thank you…,
A. It sounds like your doing all the right things with the cleaning. I would also recommend regular visits with a Hygienist and monitoring with the dentist. Gum disease is hereditary but is made much worse if you have poor oral hygiene or smoke. See what your new dentist says as a 2nd opinion would be good to get

Can my gum disease be treated?

Monday, January 18th, 2010
Q. I have just been told that I have gum disease. I am 21 and admittedlly I abused my teeth during my teenage years. I have had all the plaque removed but at the back of my bottom front teeth, on the gum is what could be described as a hole or a ditch. is there anyway to cure this and will this hole/dithc be recovered ? thanks,
A. the most important thing is to stabilse the gum disease so that you don’t loose any more bone support underneath the gum. Once stable you can look at cosmetic procedures.Rob

Cna anything be done to help with gum problems?

Thursday, January 14th, 2010
Q. Hi, I am 47 and first experienced gum problems in my 30s following having children. I consulted my then dentist about bleeding gums and have always had regular check ups, but all he told me to do was floss and brush. I was already brushing after each meal and flossing too, but the gums got worse. The recession also worsened, but he did not refer me to a periodontist (at that time I did not realise that you could see a gum specialist!). I think the vigorous brushing may have contributed to some of the recession. Some years later when seeking orthodontic treatment, I was referred to a periodontist and had extensive work to clean below gum line, which cured the bleeding gums. She also attempted to do a gum graft, but gave me inadequate aftercare advice and the graft failed. She subsequently carried out (from memory) 4 more grafts to the same area within a few months(very painful because tissue was taken from palate)and all failed. I am not a smoker or drinker and I am very, very careful of dental hygiene. Consequently, I now have areas of recession in my mouth which upset me greatly, as I feel they are unsightly. These are at the tops of the teeth. The pointy bits of gum between the teeth seem to be OK. I had the tooth in the area she tried to graft cosmetically reshaped, to try to make it look better, but it still looks very long (it is a canine tooth). My canine on the other side is also quite long, as is the tooth behind it. My front four teeth are ok. Oddly, I have a couple of bridges on the other side of my mouth and the recession has not occurred as much around these (I had these bridges put in because I had two teeth removed in my teens for orthodontics which were not completed). My gums seem pink and healthy. They do not bleed now. I floss and brush (with Oral B electric brush) always after eating and attend for six monthly cleaning. I think they may be still receding a little bit though – maybe through age? What I would like to know is, is there any way of making my teeth look more attractive, less long and ensuring that the recession is minimised for the future by way of cosmetic dentistry? I do not want to try another gum graft, because I feel that this is too risky and because of previous failure, I may not be a good candidate for this (also, the treatment was extremely traumatic the way it was carried out). My mother suffered from gum problems too, so I think I am just unlucky that I have inherited this predisposition from her. Sorry for the long, involved question, but I thought the background important. Can anything be done for me perhaps by way of using crowns, new bridges, veneers, etc.(assuming my gum condition is indeed stable)? Could my teeth be made to look smaller? This really does make me upset and self-conscious when smiling, so any advice would be most gratefully received. Thank you.,
A. Very difficult to give you a quick answer.  You would need to be properly assessed with Xrays etc but broadly speaking….. gum disease is caused by your body reacting to the bacteria around your teeth and causing bone loss and therefore gum recession.  It sounds as though you have the bacteria under control which is great.  However your bone loss around your teeth allows your teeth to be more wobbly.  This can increase the bone loss around your teeth if gum disease is present.  To minimize this your bite needs to be as even as possible to allow the force to be distributed amongst as many teeth as possible and down their long axis.   I have treated patients with “long” teeth but I would need to see you to assess whether this is possible- please contact my surgery for a free cosmetic consultation.  Hope this helps

Can I not get braces if I have gum disease?

Thursday, January 14th, 2010
Q. Dear Dr Caplan, I am a 23yr old female with severe gum disease. As a teen I had been initially refused orthodontic treatment on the NHS, due to the poor condition of my gums. I had lost faith in my dental care and hid away from my dental problems. As an adult I now realise how much my teeth have affected me mentally. I am so depressed about my crowded and crooked teeth, I find it hard to talk to people let alone smile incase they notice my ugly teeth. I have become a social recluse and feel so alone because of being scared to show my teeth. Since my teens I have had a patchwork of different work done by different dentists and my mouth now looks a mess. There are badly fitted miscoloured crowns on my UL2 and UR1. However, my gums have improved a lot. They are now a pinkier shade, compared with the liver red that it used to be. Also, the teeth and gums are a lot less sensitive. Do you think that there is any chance that I could get treated privately? Will private dentists/orthodontists refuse to treat me due to my gum disease? Or is that just an excuse for the NHS to refuse me due to their overstretched waiting lists? I am grateful for your reply, as I feel so lost and desperately want to sort out my teeth once and for all.,
A. You will need to have the health of your gums checked first of all as orthodontics can increase the chances of you loosing your teeth.  This is available privately – if orthodontics is still not possible ther may be other restorative options that can solve your problems

Can I have a bridge instead of implants?

Thursday, January 14th, 2010
Q. I have to have my four lower front teeth extracted due to gum disease and my dentist suggests that I have a partial denture placed directly after the extraction. Later on he suggest that I have implants to replace the extracted teeth. My question is whether a bridge could be used instead of the implants and if it is an option, which is the best option in the long run?,
A. If the adjacent teeth to the gap are strong than a bridge might be an option the teeth would have to be examined.  if you have suffered with gum disease than the bone may not be strong.   I think implants are the best long term solution.
