Due to my grinding habit i feel my bridges is coming loose any advice or suggestions

Q. Last year I had a bridge fitted. The dentist used only the tooth to the right of the gap to fit the bridge to. However, whilst it looks good i find it most uncomfortable. I have been told that I am a heavy grinder although I didnt realise I did grind my teeth until this bridge was fitted and I find I am constantly grindnig on this bridge. Whereas before I was told that I only ground my teeth at night. I feel the bridge is coming loose as I can feel it moving slightly at times and when I floss underneath it I sometimes feel like I have slightly put it out of place but it usually goes back to normally within a few minutes. I would like it replaced or maybe refitted to both teeth either side instead of just the one on the right of it. The gap is, on my right side, the 3rd from my front tooth and the bridge is fitted to the 4th tooth to my right. Any advice or suggestions would be great, but I couldnt afford an implant, so thats out of the question for me. Many thanks for your help

A. I agree that an implant would be the best option it would also last the longest so is the best value overall.  Finance options are available to help you spread costs. It sounds like your bridge is moving and because it is only supported on one side is not as secure.  It also sounds like the bridge is too high in your bite, which could be causing the grinding.  My only concern about another bridge using two teeth to support it is that you have to cut down another healthy tooth.

Ive a severe acid erosion and bruxism what can i do and can i prevent further damages

Q. Hi I am quite upset about recent findings from 2 different dentists that I have severe acid erosion and bruxism. 2 years ago I had no problems at all, although my diet/drinking habits have not changed. There are no problems with my gums or x-rays and I have never had a filling so now I am gutted about this. What I’d like to know is that I have been told different things by different dentists about the effects of swimming on teeth. I swim 3 times a week for 1 1/2 hours a time. This has been a new thing over the last 2 years. Could this be the problem and how can I check the PH levels of my local pool? My current dentist is insisting that I grind my teeth but I do not believe this to be true but at one time I was on a high dose of Citalopram and have read that this can cause grinding and I think it did in my case. I was on this for about 3-4 years. I am also a vegetarian and have been told this is the problem!!!??? so confused. What can I do and can I prevent further damage. I cannot stop swimming as this is now for me a form of anti-depressant. I am miserable. Thanks for you help.

A. From your post it would seem that the two key factors here are the medication, which may trigger grinding, and your diet
Many vegetarians have diets with high acidic fruit content eg oranges, lemons, grapefruits
The combination of teeth grinding and acid erosion can cause rapid tooth destruction.

I would recommend going through a diet analysis with your dentist and at the least a nightguard to prevent further grinding on already weakened teeth

The top surface of my two bottom teeth are starting to wear away what is the best solution

Q. The top surface of my two middle bottom teeth are starting to wear away. What is the best solution for this from a cosmetic point of view, i go regularly to the dentist and he has never said anything about it, so I dont know if it is a problem I should be concerned about, it may be due to teeth grinding? Also the last time I went to the dentist and had a clean and he said it removed some of the enamel at the bottom inside of my middle lower teeth, this area has now never felt quite right since this, leaving a rough feeling – is there anything I can get done about this.

A. I don’t think he would have removed any of the enamel although the teeth can feel a bit differnet after a throrough scale. You have 2 options for the worn tooth, you might be able to stop the wear by wearing a bite guard at night or you may need to restore the teeth with crowns or veneers

My permanent bridge is broken and my dentist says it is due to my grinding Is it correct?

Q. Dr. Bohara, I have a permanent bridge put on by a dentist in 2005. In 2009, it is broken. I paid $3000. She says that the bridge broke due to my grinding. The bridge has also damaged the bottom tooth. I consulted another dentist and he says that the bridge should be replaced at no cost to me. I went to the original dentist and she said that because I grind my teeth I broke the bridge and also have worn out my bottom tooth. She said that she would replace the bridge for 1/2 the price of $3,000. What are dental patients’ rights???? I did not like her office and I did not like her. I’ve also moved and I’m now 3hrs away. Thanks for your guidance.

A. Unfortunately dental treatment can fail and a hundred percent guarantee cannot be given. There are many reasons why treatment can fail.  If you seek treatment somewhere else, rather than her office then you would be required to pay the full price rather the quotation the dentist gave you. Sorry I cannot be of more help.

I do grin my teeth so could i use a sleepright dental gaurd comfort by sleepright is it safe?

Q. Hello I do grin my teeth in the night and was wondering whether I coul use a SleepRight Dental Guard Comfort by SleepRight. Is it save? How long can you use it for? they say it is a product that has been approved by the British Dental Health Foundation. Any advise will be vey welcome thanks :)

A. Some people find wearing an appliance at night reduces the amount of tooth grinding there are so many different devices available I am afraid it is difficult to give you advice without seeing you.  These appliances are non-invasive so you have nothing to loose by trying it.  I would recommend seeing a dentist who understands how the jaw works as 90% of tooth grinding is often due to an uneven bite.

One of my crown has worn out and my dentist said it is due to the grinding habit is it true?

Q. About three years ago I had my two lower back teeth crowned as well as two top teeth (next to each other on the upper right side). My back teeth were weak from grinding that used to happen from the stress from grad school. The upper two, one had a root canal and the other had a deep cavity. I got the porcelain crowns with gold underneath. I had nothing but problems with the crowns ever since. The back two almost completely crumbled all the way around the tooth less than a year after they were fitted. The upper tooth with the root canal had a small chip during the first year as well. The other crown has been fine. The dentist redid the back two crowns once. One has been fine. The other has a small round chip which appears to be worn from my bite. I have been back numerous times over the past fews years regarding these crowns. The dental director I spoke with last said I could have the crown redone again, and that it looked like it wasn’t fit right. They scheduled me for another appointment. When I went, the dentist who was supposed to do the work acted totally unprofessionally and told me that this “would be alot of work,” and that he “didn’t feel comfortable” about redoing my crown a second time. I ultimately told the dentist that I didn’t feel comfortable using their services and asked for my medical file. The crown that I have a problem with is regarding the wear down in a pattern which clearly appears to be consistent with my bite. If not fit right, that seems to be the reason why the crown is worn in that manner. The dentist insisted now that it was from grinding. I haven’t grinded my teeth for quite some time, and when I did, I could tell due to jaw pain. My boyfriend also said he’s never noticed it. My question is whether a crown with a worn down spot appears to be more from being improperly fit, as the director suggested during my last visit, or whether it sounds like grinding? I find it somewhat hard to accept that I am grinding, since like I said, it was very noticeable when I did it previously. It seems like the dentist just didn’t want to fix my tooth. That’s why I am switching dentists to someone who is willing to fix my teeth/resolve my dental issues and not make me feel uncomfortable. Any insight you may have will ease my mind after a very stressful dental visit. Thank you so much. Sincerely, 

A. Given that you have a history of grinding or clenching then it does not sound unreasonable to assume you would place the same stresses onto the crown work.
Placing porcelain crowns will not stop or cure your grinding but simply attempt to protect the underlying tooth structure.
I would advise when you have the crown remade that you invest in a night time guard to try and mitigate the stresses you obviously place onto your teeth either via clenching or grinding your teeth

I was wondering what could i do to help me stop the thumb sucking habit

Q. Hello I have a problem with thumb sucking and I have tried to quit countless of times and I was wondering what could I do to help me stop?

A. It really depends on how old you are and when you such your thumb.  You can have a removable appliance made which has slightly sharp points placed on the palate so when you go to suck your thumb you get a sharp shock! It sounds awful but it does work.

im considering a night guard how much do they cost and how can a dentist fix cracked tooth?

Q. Dear Dr. Bohara, I think I have pretty good oral health but I’m considering a nightguard because I occasionally grind my teeth. I was curious how much they cost? I think I sometimes clench my teeth during the day and I have a slight overbite. I think I may have slightly cracked one of my front teeth as a result of this. I’ve looked in the mirror and did not notice any crack whatsoever nor do I feel any pain to pressure or sensitivity to hot or cold but I feel there may be a crack in the back of the tooth. I’m pretty sure of it. If there is a crack, how can the dentist fix it? Would he drill it out and replace it with some resin or composite or place a veneer on it? I don’t think it’s too serious as it’s not causing me any pain but it is annoying and I can sense it with my breath when I exhale. Can you please help me by offering me some suggestions or peace of mind? If the crack is in the center of the tooth can the dentist still treat it? Thank you very much for any response. Sincerely

1. There are differenet types of nightguards available. They cost from £200 to £500
2.  The type of treatment for a crack in a tooth depends on the type of fracture. I would recommend a consultation with a dentist to diagnose this and offer options

i find my self clentching my jaw and pushing my tongue into my bottom teeth whats next step

Q. I have had a problem with my 2 front bottom teeth for just over a year. It comes on worse a times of stress, I find myself clentching my jaw and pushing my tongue into my bottom teeth on a regular basis, even weight training brings on pain. It hurts so bad i can feel the nerve pulse. My dentist has refused to give me a root canal filling or even take the teeth out even though I think both would help. What is my next step I am at my wits end with this as the pain is too much??

A. It maybe that you need a bite guard to stop grinding up against them unless there is a definite problem with the roots, in which case a root canal or extraction might be best. Maybe you could ask for a second opinion from another dentist??

My teeth are in a real mess regardless of good dental hygine will i need to have dentures

Q. I am only 33 yrs old and my teeth are in a real mess. Im ashamed when i go to the dentist. Regardless of good dental hygine over the years my filings fall out about a year after they have been done. I grind my teeth in my sleep which doesnt help. Will i need to have dentures

A. It would be best to avoid extractions if at all possible perhaps crowns might work better than fillings especially if you grind your teeth.

i had impression to my teeth but my teeth has gone more yellow is this possible?

Q. Hello today I have impressions done to my teeth and you might think I have gone crazy but I notice that my teeth hasa gone more yellow. Is this possible? moreover I have noticed that a bit of my shiny cover (enamel?) is gone from one of my teeth. Is the technique so agressive that this is likely to happen? I am quite frustrated because i did it to get a night guard to stop me grinding in the night and now I don’t know if it was such a good idea… Thanks for comments

A. Very unlikely that the impression could cause this. Often the teeth look different due to minor dehydration, this recovers in a day or 2

my mum has grinded her teeth and is now very senstive razor sharp what would you suggest

Q. Hello i am asking this question on behalf of my mum she is 53 and has grinded her teeth since she was a teenager as a result of this her teeth are now very sensitive,razor sharp and she wakes every day with headaches she has grinded through several gum shields and is now very conscious of her teeth and as a result never shows them what would you suggest would be the best way forward. thank you

A. You need to get to the cause of the grinding. Some people grind if they are particularly stressed. It is best that she continues with the gum shields for now- at least that way, the gum shields are being worn down instead of the teeth.
It is best to stress these concerns with the dentist who can referr you to a TMJ specialist ( a specialist in the field of jaw problems etc) You may need to have a ’stabilisation splint’ made, but this can all be discussed with you when you go for your consultation.

Hi many thanks for your enquiry. sometimes veneers or crowns can help improve the symmetry of the teeth, but it is also important to seek the reason for the loss of tooth tissue. if you are grinding your teeth you will equally grind or fractuce the veneer as you have ground down your own teeth. my advice would be to seek a cosmetic dentist for a consultation to be advised of your options.

I’m reluctant to lose more tooth surface than is absolutely necessary, but will an onlay/partial crown be sufficient for my very hard bite and probable night grinding?

Q. Dear Andrew; I’m 31, live in Essex and have had a string of dental disasters with my current practicioner, all concerning the same two teeth (last lower molars, I have no wisdom teeth there) following accidently biting on a stone in some food. The result is that I now require a retreatment of one root canal (which I’m having in Brentwood) and following that some kind of restoration work (which the specialist endodontist does not undertake). The endodontist suggested that because I have a large amount of healthy tooth left an onlay or a three-quarter crown would be a sufficient restoration for me. I’ve previously had inlays but these kept falling out and have become displaced around 4 times each (one on each of the molars in question) in the last two years, culminating the necessity for the current retreatment. I’m reluctant to lose more tooth surface than is absolutely necessary, but will an onlay/partial crown be sufficient for my very hard bite and probable night grinding? I travel a lot and would hate to loose the restoration on a trip, not have immediate access to a dentist and for the root canal to fail (again) as a result of this. Could you please advise me as to the course of action you would take? Best wishes

A. Hello

I normally prefer full coverage of a root treated tooth with a crown. However sometimes I will use an inlay made with CEREC if the tooth is relatively healthy.

I have venners on my teeth and I grind. Do I need night guard?

Q. I have venners on my front four teeth I grind and clinch in my sleep but I dont know how much? Do I get Nightguard for the top, Bottom, Both, or what? Should the guard be Soft or Hard The Reason I’m asking is because I already spent a great deal on my veneers and whitning and I dont want to spend $800 Dollars on a niteguard.

A. I think you need to get your dentist to check how your teeth bite together and make sure they are biting evenly.  I would recommend an upper hard niteguard.

Is there a way o stop grinding? What are the options if crowns cannot be refitted?

Q. Hi, I am in my early 20’s and had 8 porcelain crowns (costing a small fortune!) on my upper front teeth about 4 months ago. The reason for this was mainly cosmetic as I grind my teeth at night and had severely damaged them over the years. My bite is ‘out’ (so I am told) therefore could not have veneers. The problem I have now is after originally adjusting my bite, I am now continuing to grind my teeth at night. I do wear a guard for this but can still notice the edges of the crowns becoming flattened and worry that they will not last me the year! My question is what should I do now? is there a way of putting a stop to my teeth-grinding? how many times can crowns be refitted and what are the options when crowns can not be refitted…false teeth? I would be extremely grateful for your help.

A. Hi many thanks for your enquiry, firstly, unfortunately no one can stop you grinding, this is a subconscious thing which occurs while you sleep. the second thing to note is if you are grinding the porcelain on the crowns is much tougher than the opposing natural teeth so should not wear, if anything the night guard should wear down. if you are really concerned you could wear a night guard over your crowns. crowns can be replaced many times the dentist just has to be careful to not remove more tooth tissue. if there ever can a stage when the crowns could not be replaced then there is always dental implants. hope all this information is helpful.

Headaches and facial pain is due to grinding in my teeth

Q. Hi, I have recently been advised that I am suffering from headaches, and facial pain as a result of grinding my teeth at night, and clenching my jaw when anxious. My dentist advised me that I should purchase a special guard to protect my teeth-but that it would cost me £198. I am a Full time student and cannot afford to buy this at that price. I have been told that if you go via your Dr you can sometimes get this for free, is this true, or is there a cheaper alternative? Please help!

A. I do not know of doctors providing this treatment as this is usually provided by dentists.  A hard night time appliance properly made should reduce your muscle spasms.  This is a very exact appliance, hence the costs.  I obviously understand you are a student .

Wearing teeth due to grinding can i use lumineers?

Q. I have/ had straight and beautiful teeth all my life, but I am a grinder and have been consciously grinding my teeth for the last 25 or so years. I am very vain and my smile is the core of my confidence. Now my teeth are worn down, cracked and yellowing. I would like to cap all of them (to close gaps and lengthening them) and add a couple of bridges (to replace missing teeth) but I don’t like the artificial look that some veneers give. And also don’t like filing down my natural teeth. Luckily with Lumineers they are thin and transparent enough to avoid both. But are they strong enough for my griding? And is there anything I can do to stop griding because my jaw and that whole area has shifted and changes (not in a good way). Can my case be helped and how? Thank you!

A. The first thing that needs to be done is to assess your bite and find the cause of the grinding.  Veneers or crowns can look absolutely beautiful so I think you may have seen some bad examples.  It really depends on the condition and amount of natural tooth left as to what type of restorations I would make for you.  After all the treatment is done I might make you a type of night guard to sleep in to protect the new teeth.  There are lots of options for you.

Teeth grinding

Q. I am 54 with small worn down teeth from grinding. I hate them and feel they are very ageing. My dentist says there would be little point having anything cosmetic done because I would just grind them down/away/chip any work. I do believe him, but hate my smile so much that I think false teeth would look better than my own, though they are healthy, just very soft. My 2 front teeth are a filling, they were so bad.

Hi many thanks it maybe that if you had a full mouth rehabilitation this would help to build back up your teeth and improve the aesthetics, it would also place the teeth into an favourable grinding position. so that when you did grind the front teeth would protect the back and visa versa. the best option would be to seek the advice of an advanced restorative dentist to help improve your situation as i feel it maybe out of the remitt of the general dentist you see at the moment.
