Q. In early March this year, finally my “baby tooth” cut through – in the roof of my mouth, just behind my eye/front tooth at a pointing outward angle. Nothing special really, except that I’m 43 years old. I needed dental surgery, as it was pushing my front tooth forward and making loose my 3 tooth bridge. I had the surgery at Guy’s Hosipital under the care of a lovely team. It was pretty drastic I must admitt, drilling through the front of my upper gum line to the left of eye tooth and incision/hole drilled in the roof of my mouth to extract said tooth. Nine months later however, I’m left with a widening gap of the eye/front tooth and my 3 tooth bridge is loose, and lower than my gum line. My whole gum line has changed above the bridge so that now you can actully see the metal structure of the bridge itself. The bridge used to fit snug and I never had a gap either at the top of this structure or as is now a gap between my front and eye tooth. I don’t think my upper jaw bone structure would support implants following my surgery. What would you suggest? At this point I am seriously considering a denture of some form, one that does not involve invasive surgery, but then what about the gap between front tooth and eye tooth?
A. Hi it sound like you’ve been through a lot so a denture would seem like an easy option. It might be a good short term option however I guess in the long run you would probably be more comfortable with fixed teeth. This could probably be acheived using an implant supported bridge although it sounds like the bone would need rebuilding firts. Perhaps you could go back to Guys and ask them for their opinion as they have very experienced surgeons who might be able to help you.

Q. Hi, I visited an orthodontist in Cambridge who was very helpful. He said my teeth have an 8mm gap from top to bottom, they are also quite crowded at the top.So i would have to have 2 teeth extracted. The answer to straightening my teeth was take 2 teeth out one each side at the top (premolars)and then have a fixed brace for 18 months. I am quite happy with his answers to my questions.The cost was £2800 which i thought was reasonable. When i got home i looked into it more on the internet and found out about 6 month smiles. Would that be more suitable for me. The orthodonist i saw does’nt fit 6 month smiles.I guess he knows about the procedure and probably thought it wasn’t suitable for me, but would it be worth me making an appointment with yourself or someone nearer to my town to see if that treatment would be better.
A. Thanks for your questions, it sounds like you could be a candidate for the six month smiles treatment. However it would be impossible to say for certain without a consultation. I would recommend that you search for a certified six month smiles provider and visit them for a consultation. If you decide the treatment is not for you, you can still proceed with the Orthodontists recommendation, so will have lost nothing. I hope this helps.

Q. ok i 13 i have a gap in between my wide two front teeth and ppl say its fine and it well close and i have all my adult teeth and i have a missing adult tooth in the back and i say i need braces or a retaner but with do u think i should do?
A. Thanks for your questions, I suggest that you attend a dental practice to obtain a professional opinion on whether you would benefit from braces or not. Once a dentist has seen you, they will be able to refer you on to an Orthodontist, if necessary, where you will be able to receive treatment. I wish you well.

Q. hi Mr caplan i hope you can help me I am 36 years old and i have gaps in the first four front teeth at the top not massive gaps but enough to be obvious and they are overlap the bottom teeth quite a bit this affects my confidence and i dont want to smile how much would it generally cost to cure and to bleach my teeth also as they are not completley yellow but are not white either!! I need to have two teeth out also that have decayed the teeth behind the fang tooth on both sides these obviously need to come out first. Please can you give me some advise as i would really like to resolve me confidence issue with my smile

Q. I have had braces and yet i have a small gap between my two front teeth, my orthadontist said i should go and see my dentist for bonding or venners as there is nothing else she can do. i was just wondering which technique is better and longer lasting and if they are stain resistant. Also i may consider home teeth whitening kits, and was wondering if they the veneers or the bonding will whiten also. morevover can you please tell me the NHS prices fro these and if there is a significant reduction in prices if you are a university student.
A. Thanks for your enquiry, firstly these procedures would be classed as cosmetic, and so would probably not be available on the NHS. If you are considering having your teeth whitened, then I would encourage you to do this before you have any restorations placed on your teeth, as whitening only works on your natural tooth. The material which is then used would be matched to the whitened shade of your teeth. There are benefits to both tooth bonding and to veneers, as tooth bonding is a less invasive treatment however veneers would lead to a more aesthetically pleasing result in my opinion. Q. I have 4 incisors instead of 6 (upper teeth) – the 2 incisors adjacent to the front 2 teeths are missing from childhood. I have gaps between all theh 4 incisor teeths. I would like to know if there is a way to get rid of the gap. Also, what could be an estimate expense in terms of money and pain.
A. Thanks for your questions, it may be possible to have Orthodontic treatment to close the gaps between the teeth or failing this veneers may be an option. Although the price for Orthodontic treatments will vary between systems and practices, the costs currently start from £999 at my practices. For veeners, you should expect to pay between £450 and £650 per veneer. I hope this helps.

Q. My teeth are spaced out on the top(only 6 of hem) and on the bottom my teeth are a little crooked.(only 4 of them) I know it might not seem like a big issue but i was just wondering if there is a way i can get braces for about 6-8 months without it costing me too much?
A. Thanks for your questions, there are teeth straightening systems available now, which can work in as little as six weeks. I would recommend that you attend a consultation with a practitioner who offers systems such as the Inman Aligner and Six Month Smiles. there are many practices now that will offer a free consultation to enable you to discuss the options that would be suitable for your specific case and the prices, without any financial commitment from yourself. Many of these practices will also offer finance options such as interest free credit. Although the prices for these treatment will vary between practices, I can tell you that the cost of the Inman Aligner at my practice starts from £999. Q. hi, my teeth are quite small for my age. i’m 16 and only noticed this 1 year ago after an orthodontist pointed it out to me. My molars are maybe 2-3mm out of my gums and my other teeth are less than an inch out of my gums. is this strange or common, furthermore, my to front teeth overlapped as i grew up. is this related?
A. Size of teeth and crowding/spacing are related. The size of your teeth oftens relates to your general body size.

Q. I recently(3 months ago) had a crown done. When my dentist put the final crown on, the back side could not be flossed. After 30 minutes of trying he ended up drilling between the two teeth to make a gap. I now have what feels like a huge gap between my two back teeth. I can’t eat on that side because the teeth now are to sensitive. They weren’t that way with the temp crown. I went back to him and he told me I have pulpitis. What I want to know is this common to drill between two teeth?
A. I wold ask for a new crown to be made if this is what has happened as food will impact in the gap and cause problems in the future

Q. Dear Dr.Caplan, I’m 24 years and My teeth have had moved a bit forward and my dentist suggested me expanders and bracers after that. So im wearing expanders now (upper jaw) and suppose to remove it next week. But my problem is, now my front teeth have come EVEN MORE FORWARD and looks bad than earlier.(and of course the jaw has been expanded as well and created gaps between teeth.). so my question is, other than creating gaps between teeth, does the expanders causes teeth to come forward like this ?can this be corrected? please answer. Thank you.
A. The expander does not usually cause the front teeth to move forward but if they have this can be treated with the next appliance

Q. hi. im 26 and at the moment im having some real problems, that is really making me very depressed. last year i was asked to be referred so i could have fixed braces as i was having alot of pressure in my mouth which i was told was due to a over crowd. i was fitted up braces but im a full time student at university and have two young children, i cant afford to have these braces as much as i want to. i have noti
ced these past few month that my teeth are pushing forward which is causing me a lot of pain, my bite has changed and im now having to hold my jaw at an angle due to this. is there any way i could get any help with having these braces fitted? or would i be able to have them on the nhs? it is really ruining my life at the moment and affecting everything that i do. i have constantly got aches and pains in my mouth, my teeth feel so tight has they are pushing with no where to go, and it also causes headaches and pain at the top of my neck. im at my wits end with it. if u could help me it would be great.
A. I doubt you will get braces as an adult, on the NHS. I also doubt whther they will help your symptoms. i would consider a bite guard worn at night to try to stop you clenching or grinding your teeth and to realign your bite as this is probably the cause of your facial pain. If money is a problem you might try a referral to a local dental teaching hospital?

Q. Dear Dr., im wearing expanders (upper jaw) and suppose to remove it next week.then il be getting my bracers to push my teeth back which are abit forward earlier.. but my front teeth have come EVEN MORE FORWARD now and looks bad than earlier. so my quetion is, other than creating gaps between teeth, does the expanders causes teeth to come forward?can this be corrected?
A. hi many thanks for your email, the expander will expand the whole jaw so yes initially this will cause all the teeth to move outwards. the brace will then be placed to then bring the teeth all back into allignment and close the gaps, this should then solve the problem. hope this is helpful.

Q. My daughter is 19 and has 2 pre-molar (baby teeth) still in place. There are no second teeth to replace them. Her own dentist suggests extracting these 2 teeth as she says they are growing into the gum. Do they need to be extracted or do you think they should stay. If they have to go what will she get to replace the wide gaps. thanks
A. hi many thanks for your email, the problem is if there remain in the mouth there will continue to grow into the gum and be functionally unless, the alternative is to remove the teeth and replace the gap with fixed teeth via dental implants which should hopefully last a very long time.

Q. I was diagnosed with gum disease over a year ago and i’ve now been told apart from my loose teeth my gums are well improved. my problem is i have prominent gaps between my front 2 teeth top an bottom and i don’t know what can be done to help. my dentist has told me eventually they will fall out. i hate smiling and i hate my teeth.

Q. the tooth next to my upper front tooth has over the years pushed right forward. My dentist says that if the tooth was taken out then the gap would be to narrow and any replacement would not look natural. is there a treatment to cure my problem. Thanks

Q. Hey Dr. Solanki I have two big buck teeth that really bother me they are way big and have a gap in the middle but the rest of my teeth are pretty much straight just those two and my teeth also arent the whitest what would you recommend to fix my buck teeth and whiten my teeth?
A. hi many thanks for your email, the two options to fix your buck teeth are braces or veneers, the braces are more conservative but the treatment may take longer, the veneers are a much quicker fix but hav the draw back of the teeth needing to be filed down and also the veneers will need replacing every 10-15 years. the whitening treatments i would recommend is zoom2 whitening or enlighten.

Q. Hi. Many thanks in advance for taking time out to read, and hopefully answer my question. Over 20 years ago I had a dental brace, but for the last few years, my teeth have started to move. It appears that because of the gaps in my teeth I have developed a “rasp or lisp” in my speech, which is very distinctive. My question is this. I realise and accept to have a dental brace will cost me money. I am prepared to pay this. However, in view of the fact the gaps in my teeth are causing me to speak with a lisp/rasp, could the gaps in my teeth (one of which must be close to 1cm) be repaired under the NHS, or if not, would it cost of a dental brace have to be funded soley by myself? I am intent on having a brace, and this question is the first step in progressing. Many thanks
A. Thanks for your message, there are very few, if any, circumstances in which the NHS will subsidise orthodontic (teeth straightening) treatment for adults. As a result of this, I feel that you will be looking at private treatment. The prices for orthodontic treatment will vary between practices and between different systems, however I can tell you that at my practice, our prices currently start from £999.
