What are pros and cons of teeth filling?

Q. I had braces when I was 17. At the time there was an issue with overcrowding on the bottom teeth. There was discussion of pulling a tooth, but it was not done. I’m now 33 and have overcrowding of my lower front 6 teeth. My orthodontist suggested pulling a tooth or filing the teeth down. What are the pros and cons of filing the teeth down? Does this ruin your tooth enamel?

A. Slight adjustments using a techniwque known as interdental stripping helps to create the space in order to align the teeth. The loss of a tooth often causes an asymmetrical final result but might be your only option if the teeth are very crowded.

Need best treatment for no calcium teeth and its cost

My enamel is disintegrating on my incisors could you advice

Q. Dear Dr. Solanki, Having always looked after my teeth, I am very dissapointed to see that the enamel is visibly disintergrating on my inscisors. Twenty years ago the same problem occured and I was given crowns on my two front teeth, which I know feel look too large for my mouth. I am unsure what to do. I have a problem visiting the dentist (though I always do),having had a few unhappy experiences. I would be very grateful for your advice

A. Thanks for your questions. If you are considering having crowns fitted on your incisors, I would recommend that you visit your dentist to discuss this with them. I would also recommend that you ask your dentist to have a wax up created by the laboratory that would be making your crowns. This would involve the dentist taking impressions of your teeth as they are, so that the laboratory would be able to create a realistic impression of how they would look with crowns. To create the perfect look, you may also wish to consider having your front crowns replaced at the same time, but I would recommend that you explain to your dentist prior to any work, exactly what you dislike about your existing crowns. You may also wish to consider gaining a second opinion, by visiting a cosmetic dentist and having them create a wax up as well. This should give you a good idea of what it would be possible to acheive. Many cosmetic dental practices will offer a free initial consultation, which would enable you to gain their opinion without financial loss. What would be the cost of treatment crown whitening cleaning

Q. I have been a sufferer of bulimia nervosa for about 8 years now and have suffered very severe enamel erosion as a result of it – I haven’t been to see the dentist in a long time because I know my mouth is a disaster area and until I have managed to recover I don’t want any dental work done that I might later wreck again. I imagine my mouth is full of cavities etc so that will have to be treated and I was also wondering what the cost of cosmetic treatment would be? At the very least I would guess I need crowns on upper front teeth (front 6 at least) because they are transparant at the edges – every tooth in my mouth has a greyish discolouring where my teeth are turning transparant at the tips and are mostly yellow everywhere else so I will most likely need cleaning and whitening too. Could you estimate the cost of this treatment? Is treatment even possible do you recon? Thank you for your time.

A. I have experience treating patients with severe loss of enamel due to various types of acid damage.  Often crowns are required due to enamel damage on the inside surfaces of the teeth.  Due to the tongue position when you are being sick the damage is often limited to the upper teeth.  Treatment in the past has cost from £6000 depending on how many teeth need to be treated. I also make a mouth guard that you can wear to protect your teeth during your illness. Please visit me for a free consultation if you wish to discuss this further.

What can be done to prevent erosion?

Q. Hello Dr., I read about enamel erosion on front teeth and I believe that this is my case, i have a few yellowish/transparent spots on my 2 front teeth. I read from one of your replies that I can’t restore the color with diet or habit, however what can I do to prevent more erosion and to at least keep it as is. thank you

A. Check your diet for acidic foods particularly fizzy drinks diet or otherwise. This is the cause of most erosion so reducing this will help prevent further enamel loss

I ve discoloured teeth can I still ve teeth whitened?

Q. Hiya, my teeth developed without the enamel being there when i was younger and i’ve been left with ugly discoloured teeth. Am i still able to have my teeth whitened?

A.

If you have amelogenesis imperfecta it is unlikely that bleaching will help.  You will need some type of porcelain restoration to replace your missing enamel

Q. If there is enough enamel under a dental crown can it be bonded using composite bonding?

A. yes this is called a dentine bonded crown!! these should anly be done with at least one wall of tooth tissue is still in enamel.

What would be the treatment for having invisible braces?

Q. Hi dr rob…. I am now 27 and had a number of removable braces from the age of 14-16 than a fixed brace from 16 till I was nearly 19. I was really happy with the results, but shortly after having this removed my bottom teeth all crossed over again. I did visit my orthodontist about this but told ne at the time all they could do would be to file each tooth down then put a fixed brace on again. I was concerned about the health of my teeth filing them so did not go ahead. Since I have remained unhappy with my teeth but thought there was nothing that could be done, until I heard recently about invisable braces. I would like to look into this but feel that as the job wasn’t done properly in the first place that to pay for this now would be unfair. My top teeth have now also moved since I have a wisdom tooth coming through, and now I am feeling unhappy with these also. I was also told I would never get any wisdom teeth!! What would be your suggestion for how I go treatment from here?? Kind regards

My first impression is that you are unlikely to get funding for invisible braces. Also all braces need space to move teeth around and so even Clearstep or Invisalign may need “interproximal stripping” ( removing enamel between teeth) to create space. After treatment you will need to wear lifetime retainers.

I had bulimia Will the yellow marks disappear by teeth whitening?

A. I have treated a number of bullimic patients and if all the enamel has been removed the teeth will not whiten,  better option is bonded fillings or porcelain veneers or crowns.

Are porcelain veneers a better option for complete makeover?

Q. hi i was wondering if theres a procedure to replace enamal on my teeth i have coeliac disease and my teeth are kind of transparent in places, due to a gap in the middle of my top teeth and a damaged top tooth from bad root canal treatment i would love a complete makeover but cant unfortunatly afford that right now, would porcelain veneers be the better option? many thanks ps if ever you need a model for anything in exchange for discounted dental work please contact me i would be more than gratefull and willing!

A.
You could have composite veneers placed which are mush cheaper than Porcelain Veneers, but they also have a shorter life span (around 5 yrs) as composite can discolour and chip and is not a long term solution.

Alternatively you could contact the dental teaching hospitals or the cosmetic courses run for dentists to see if any dentists need a patient to take with them.

Will drilling away enamel cause any problems in future?

Q. Hello Dr Caplan. I am due to have my teeth straightened with Invisalign braces. As I don’t have enough room in my mouth my orthodontist is going to drill a tiny amount of enamel away between each tooth. I was worried about this as I always thought damaging the enamel in any way was a bad thing – but he has advised me this won’t cause me any sentitivity and the drilled areas will harden again over time, and actually become harder than before the treatment. I’m very unsure about this – can you please let me know what your opinion is? Thank you.

A. The maximum amount that is removed during invisalign treatment is 0.25mm off any surface.  This will not cause long term problems if carried out carefully and is the normal protocol for invisalign treatment

Will fizzy drinks erode enamel and effect the Whitening process?

Q. I have been given a kit called ‘day white excel 3’ from my dentist in Australia. I used the kit last week for 9 days straight and was meant to leave it on for half an hour, but sometimes left it for over an hour. I had very sensitive teeth afterwards and felt that diet coke or anything acidic afterwards was attacking my teeth. It looks like some enamel may have been eroded the root s of some of my teeth, is this possible or could the whitening highlight a problem that may already be there? In addition do fizzy drinks and sparkling water erode the enamel on teeth over time? Thanks in advance.

A. During bleaching and for 2 weeks afterwards you should stay away from fizzy drinks and staining foods/drinks.  Home bleaching kits will not cause erosion of your teeth provided they are prescribed by a dentist and are carbamide peroxide based.

A gel for hardening gums

Q. Hi Can you tell me please where I can buy the gel for gums that hardens to protect my gums and lips when I use my home kit please ?

A. That gel is only for in-surgery bleraching when 40% hydogen peroxide is used.  For homw bleaching the gel is usually 16% carbamide peroxide – this breaks down to about 5% hydrogen peroxide which is safe to use without gum protection

My enamel are badly eroded can ive porcelain veneers on the nhs

Q. As a recovered bulimia sufferer, the enamel on my teeth is badly eroded. I am a new mother too and wondered if it is possible to get porcelain veneers on the NHS.

A. It sounds as though you MAY be a good candidate for veneers on the NHS. It all depends on the extent of the damage. Veneers will only be done if it is totally necessary. The only thing I can advise, is that you go and see what can be done. If youhav elost a lot of enamel, I’m sure you’ll be expoeriencing some sensitivity and will probably need some form of treatment doing. Hope this has helped.

Q. Hi, when I had my braces removed my authodontist took off half my tooth and quite a bit of the enamel on some others. How can I get it repaired and how much would it usually cost?

A. Usually the orthodontist will skim off the bonding resin that held the brackets in place, if any tooth has been lost then cosmetic tooth bonding can replace this usually at a cost of around £100

Ive notice that the enamel on my front teeth has worn away is there a way to built it up

Q. I have just had my braces taken off and now only have a retainer to wear at night. I hadn’t noticed before but the enamel on my front two teeth and a few of the bottom front teeth seems to have been worn away at the tips. Is there any way to build it back? Also, some of my teeth have wavy lines at the bottom instead of a straight edge. Why could this be? Is there any way of fixing this? In addition to this, one of my canine teeth is particularly large and more jagged than the rest of my teeth. Can this be re-shaped or filed down? Thanks.

A. Re4contouring of your teeth is possible to reshape and restyle your smile.  The wavy lines are called mammelons and are perfectly natural – just means you have had very little wear

Ive been bulimic on and off and ive have lost enamel on my teeth is this caused by vomiting?

Q. Hi I have been bulimic on and off for a few years now. I have a huge fear of dentists but went last week. I was worried the dentist would be able to tell that I’m bulimic but he didn’t say anything, so does that mean he probably couldn’t tell? Also, I don’t think I have lost enamel on my teeth but my top, front teeth appear ’see through’ at the bottom and have really bad brown stains on the back of them. Is this caused by vomiting or is it just inproper dental hygiene? I wanted nto ask my dentist if I was doing any damage (as I also need several fillings and a root canal) but I was too embarrassed. Thanks for your help.

A. Hi

I would recommend you ask your dentist. This is the reason you are seeing him/her….to look after your mouth. So dont be worried , as they will have your best interest in mind. It is very difficult to give you advice in this situation without looking at the mouth.

My front teeth are transparent what are the treatment possibility other than veneers

Q. Hi im 19 and just recently i noticed that my front four teeth are transparent and i was wondering what exactly is this? I’ve been reading for quite awhile now about this issue and it seems to be common, but no one gives a straight answer on what it is. I’ve read that it is loss of enamel and need to be looked at cause they are sensitive, or it is not enough calcium. Well my teeth are not sensitive to hot or cold and rarely bother me (other than appearance) and ive been taking calcium pills for some months now and still have seen no signs of change (other than white spots that appear on my teeth but then after a few times of brushing they fade away). I have looked into treatments and toothpaste and have been using toothpaste with liquid calcium (Arm & Hammer Age Defying / Pro Enamel) in it that claims to rebuild enamel, but this is my 3rd tube and again i’ve yet to see change. I did contact my dentist and asked about my problem and when i mentioned it they were hung over on lost enamel, but the more i told them i felt no pain or sensitivity, that i don’t drink soft drinks (have drank only water for about 2 years now), and rarely eat fruit plain or by it’s self, they became confused. Then i asked about the treatments i’d found (ex. synthetic enamel, and liquid calcium) and again there was a silence on the phone and they told me they had never heard of those and said that i should get veneers right away. (i feel they just wanted my money) I was wondering if these treatments work, or if there is another way to fix this problem other than cosmetics? For could it be a mineral / vitamin i am lacking in my diet?

A. Once the teeth have eroded or worn it isn’t possible to add enamel using a tooth paste or calcium supplement. The teeth can only be strengthened to any degree with regular application of fluoride, this won’t change their apperance. I suggest you see your dentist and ask him to monitor the situation or suggest a restorative option.
