Q. Hi there, I suffered from bulimia when I was 18 & 19, I’m now 36 and my 2 front teeth have several fillings caused by the bulimia. I’m happy with them at the moment, although the fillings did fall off and had to have them redone but my dentist has said that I should have crowns and that Lava crowns would be the best solution. He has said that I should have them on the front top 4 teeth. Is there a problem with me leaving it as I’m happy with the fillings and think that Lava means that I will have to have a lot of my real tooth filed down and think what is the point if it looks ok and isn’t really causing me too many problems.
A. If you are happy with the fillings you have, then the best option by far is to leave them as they are. If you want to change them in the future i dont see why you cant have them changed for fillings again or even veneers as these are both much less invasive than a crown.
Kind regards

Q. Hi there, I’m e-mailing from N. Ireland. After reading extensively regarding Veneers / Lumineers ( I have 2 crowns at the side of my front teeth). Firstly, before I start searching for a cosmetic dentist, what qualifications do I need to look out for ? Also, are Lumineers as good as a porcelain veneer and do they last as long. Also, If I need root treatment, fillings etc etc after having my lumineers/veneers fitted – can I go to my regular dentist to have this done – OR do I need to see a cosmetic dentist all the time thereafter ? Thank you so much
A. You need to find a good cosmetic dentist and have a consultation, lumineers are just one type of veneer and after a consultation you will know all the options. Try to find a dentist with a good reputation or a personal recommendation.

Q. I was diagnosed with slight TMJ many years ago and it show up now and again with a lock jaw and difficulty when eating. I used to sleep with a guard on my upper teeth. Do you think it is worth having another guard made? Also do you think an old filling on the bottom molar teeth could be contributing to the lock jaw pain? I plan to be in London during the summer. Thank you very much
A.
Yes the filling could be contributing and i do think you should get some sort of appliance to help you. Please take a look at www.tmjpainreleif.co.uk. If you are in london then you are welcome to come and see myself for a consultation, alternatively if you let me know where you are living i can put you in contact with the nearest dentist practicing the same method.
Kind regards

Q. I had a white filling placed on one of my front teeth a few years back, I don’t think the dentist was comfortable with the bonding procedure, and since then the surface of that tooth is lumpy and uneven. Would it be possible to have a dentist “smooth” the tooth out by buffing or polishing it? Thank you.
A. Go back and see your dentist and explain this I am sure they will be able to polish the tooth and make it smoother.

Q. In my most recent dentist visit, my dentist had replaced several of my silver amalgam fillings with composite ones (not sure which exact type but it did involve UV light and some sort of bonding agent). One of the teeth was a Molar with pretty big cavity the filling covered most of the top of the tooth/chewing surface). Afterwards, I read online that specifically for Molars and certain cavities (not sure which ones) composite fillings are recommended less than silver (amalgam) ones due to increased problems with leakage, less longetivity (about 5 yrs less than amalgam), and if compromised higher chances of having to do a root canal to fix it. At the same time, I am pretty sure he used the most recent version of composite filling used in US – because he just recently started using it and was REALLY proud of it. So the material was the latest version, not sure about his technique though since its my first experience with this particular filling method. This made me concerned, since the cavity I had was pretty deep and another dentist (couple of years back) warned me that next time I might have to do a root canal because of the depth of the cavity. My question is was my current dentist right in replacing my old amalgam fillings for a big cavity in my molar – the cavity was at the top of the tooth (chewing surface) with composite fillings instead of using silver amalgam again?
A. I do not use amalgam in my practice, i only use composite for fillings, or if the cavity covers a large part of the tooth surface then i will use a lab made restoration called a porcelian inlay. However this restoration is a lot more costly.
Without seeing your teeth, in my opinion your dentist did the right thing by replacing your fillings with composite as amalgam is not adhesive and in some cases more of the tooth surface has to be taken away in order for the filling to stay in. For this reason i do not use amalgam, along with the obvious reason of its appearance.
Kind regards

Q. Hello I had bulimia in the past and my teeth are heavily damaged, the front teeth especially have lost enamel and left ugly yellow marks that brings down my confidence. I am considering teeth whitening. Would this make the yellow marks disappear? Thank you for your time
A. I have treated a number of bullimic patients and if all the enamel has been removed the teeth will not whiten, better option is bonded fillings or porcelain veneers or crowns.

Q. Dear Dr. Patel, At the age of around 12 I chipped my front tooth after a fall leaving less than half of my front tooth behind. I had a filling applied to this, but the tooth has gradually darkened over time. I’ve heard that this is due to internal bleeding within the tooth. Yesterday, this filling fell out and I am yet again left with half a tooth. The tooth in question is a few shades darker than my other front tooth and it looks very bad. I’m an 18 year old on a gap year, with no money at all. Would the NHS be able to help with my problem because it really looks awful? Thank you
A. You should be able to get NHS treatment you might need a root canal treatment and maybe a crown, or a bonded filling and internal tooth whitening.

Q. I am a UNITED STATES 43 male, and just had checkup. I have reflux and have ground teeth into the denton. Kanines are level w/others. molars have very wide fillings. backs of front teeth have been acid washed to denton, i guess. told i need 14 crowns immediately. front upper 6 +canines+back molars. $12K. is there some insurance I can buy to offset this? or is it too late, since I already saw the DR. I am scheduled for this friday. Please respond ASAP. URGENT to know. what is your advice? do you think this is urgent/necessary??
A. If teeth have severe acid erosion the options are onlays or crowns depending on how much tooth is present. I am a British dentist and do not have knowledge of your insurance schemes

Q. Hi. I had SEVERAL fillings done about 1.5 months ago, and on occasion, those teeth will hurt, especially when I chew gum. It seems that the sensitivity to hot and cold has diminished considerably since I had the fillings done, but most times when I chew gum, those teeth hurt. The fillings are white, not silver. Is this normal?
A. hi
this problem can occur several factors play a part the depth of the filling/closeness to the nerve.
the technique by which it was placed .
as symptoms are improving over time -give it a bit longer or contact your dentist to check everything is ok as it may settle

Q. Hi, I had two composite fillings in my two front teeth a month ago…I wanted to know, if i wanted to get braces how would that affect my teeth…aslo the tooth that had the most composite in it, feels slightly thinner (in width) than the other. It feels as if its not as durable as the other, like its more prone to breakage; would u give me some advice pleasseeee!!! Thank You
A. Please seek an opinion of an orthontist. It shouldnt affect your fillings, however this may depend on the type of brace you have.
Kind regards

Q. I am thinking of a home teeth whitening programme as my teeth are becoming more yellow with age but I have several small white fillings on my front top-bottom teeth(maybe 20-30%)what will happen to these will they whiten also or will I have to have them all re-done? Regards
A. If they are very small they can often be left. if the teeth whiten considerably and the fillings show, you may need to have them replaced.

Q. Dear Dr Moore, I had a filling done between two premolars, and both teeth now have fillings. The problem is that apart from chewing being uncomfortable, they don’t fit well with the bottom teeh and there is now a gap that gets infected and is very painful. I have been back but was not able to find the same dentist and another dentist said that the xray doesn’t show any problem and that everything is fine, but that area between the premolars stings sharply, is itchy and swells up. Looking in the mirror all I can see is that the shape of the teeth is different and they seem bigger. I am very confused and don’t know what to do. How can I get them to look and feel like they used to?
A. It sounds like they might need redoing. Maybe get a 2nd opinion from another dentist. this might help you to decide what to do

Q. When I was younger I chipped my tooth so went to my local dentist and they gave me a white filling that was fine until recently when it became discoloured. What is the procedure for fixing this problem? Will the old filling need to be removed and replaced or can this old filling be whitened? I know you will have to treat each case individually but what is the usual procedure? I am currently 16 and have had this filling for about 7 years.
A. If the staining cannot be removed by polishing replacement is normal

Q. I had a crown on a back molar which did not fit correctly and is in the process of being replaced. I have a dull ache in the tooth area to the rear of the tooth. It is not the throbbing of a regular root canal type pain. An endodontist could not see damage to the nerve or root other than the poorly fitting crown which is now gone and a temporary in place. I am getting intermittent pain but am concerned about getting a root canal as a previou s root canal on a bottom molar left permanent nerve damage. What do you suggest?
A. The fact that an endodontist could not find a problem with your root would generally indicate that the tooth can be crowned. However be aware that despite the endodontists reassurance the tooth may still need to have nerve treatment at a later date- this usually cna be done through the final crown although a filling will then need to be placed or even a new crown.

Q. Hi, I recently chipped one of my front teeth, and yesterday had it filled in (I think with a resin). Since then, I’ve noticed a sticky gunge collecting around it-similar to saliva, but thicker and sticky. Is this normal due to how recently the filling was put in, or do I need to go back to my dentist.
A. if it doesn’t resolve in few weeks I would get it checked out. Rob
