Why is that my crowns and veneers are not whiter anymore?

Q. PORCELAIN TEETH CHANGING COLOR I had a full restoration in 2000-01 using empress crowns and veneers on the front lower teeth. The crown and veneer color were the second whitest you could get. I wanted the whitest but my dentist talked me out of it. My teeth looked fantastic for several years but now have turned off white. I was told that the porcelain would never change color…that is why I got the work done. My smile was very white for the first 5 years but has now changed to off-white..not very pretty. I have my teeth cleaned every 3 months and brush and floss 3 times a day. What is going on? I have pictures I can send. I am so upset because I wanted a really white smile and paid $30,000 to get one.

A. I would go back to your dentist to find out whats happening. I have found this myself with Empress veneers and have had to replace several veneers. It may also be the teeth underneath changing colour so get your dentist to check them out.

Is the tooth crowing treatment irreversible?

Q. Hello. I am a 31 years old male pharmacist who needs 4 upper front dental crowns. I consulted with three reputable dentists, and they all say I do not qualify for veneers because I have large fillings, and not enough bone structure for veneers. I did a research on the internet, and it seems to be that multiple dental crowns are rare in people of my age, 31 years old. I feel somewhat embarrassed, and it makes me feel old and falling apart. It is a psychological issue and my dentist does not completely understand it, I think. Is it rare for a patient of my age, 31, to require 4 front crowns? Throughout my life, my teeth were always more vulnerable to decay. As a result, dentists did a lot of drilling. Unfortunately, I have large front fillings some with recurrent decay around the them. I understand that I need this procedure to save my teeth in the long run. But I feel so much uneasy about having this procedure done. Because almost all of the bone will be filed away during preparation, it seems to me that this procedure is really a tooth loss. The tooth that is receiving a crown will not longer be functional. Technically there is no tooth loss because the root is saved, but the crowned tooth can’t exist without the cap anymore. What scares me is that this procedure is irreversible. I am seeing a reputable cosmetic dentist and I do not question his quality of work. But still, just like others, he will have to take away most of the bone during crown preparation. A. He will not take your bone away – to prepare a crown enamel and dentine are removed and if you have large fillings already in place this is probably the only option for you.  If done well they should last for many years.

Are the crowns the only solution for my strange bite

Q. Dear Dr. Caplan I have a strange bite where my teeth only touch at the very back. I have had this since childhood and have learnt to compensate accordingly (no problems eating etc.). I am now 34 and my new dentist advised there are too choices, surgery – which I would rather avoid especially since the reults are not a given and secondly to crown the teeth and create a new bite. This would mean crowning 10 teeth on the top and ten on the bottom. My dentist says it would change the way I speak (get rid of my slight lisp) and correct my bite and prevent it worsening over time and over-crowding of the teeth. Are crowns the right solution? 20 crowns seems very extensive and no doubt expensive! What problems could I forsee? Will I develop problems in the future if I do not do this? How long with crowns last? Thank-you!

A. A lot depends on how severe your open bite is. In these situations I often get a second opinion by an orthodontist.  If possible it is always best to move the teeth into the correct position rather than cut them for veneers or crowns.  If teeth are heavily prepared – as in the case of crowns – there is always a possibility that the nerve may be affected and root treatment necessary.  Having said that I have done many cases where the case was very straight forward and no root treatments were needed.  If orthodontics is not possible then I would opt for onlays on the back teeth and veneers on the front teeth – minimizing the amount of tooth that would need to be removed

What are the pros and cons of having posts added to root canals?

Q. What are the advantages and disadvantages of having posts added to root canals? If it is not absolutely necessary, would it be wiser to wait? At what age do dentists start advising people to add posts? I have had a few front crowns for a few decades now. Thanks!!

A. Posts are required to support a crown when a tooth is badly broken.  All posterior teeth and premolar teeth should have full coverage restorations made to prevent them from splitting.  Anterior teeth only need to be crowned if they are very heavily filled or for aesthetic reasons

What can I do to reduce the size of my central incisors?

Q. Hi, My central incisors are rather large compared to my other teeth, I saw on the internet that the average size of the crownisabout 6mm and mine are well over 10mm. When I was younger I had a brace to bring down my canine teeth, this brace re-aligned my teeth nicely, however my central teeth still look(ed) large in comparison to the others. I have always been really consious about my smile and would love to have a smile that I am comfortable with. So, I was wondering what action I could take to reduce the size of my central incisors and a rough estimate of how much this would cost. Thanks in advance.

Capped teeth seems to stick to my lip a bit advice

Q. Re:Capped Teeth I recently had my 2 top front teeth capped,1 is fine the other is much thicker causy my old tooth was knocked back years ago,so having trouble actually biting into food,have to break food or eat soft stuff.Also I seem to be lisping a bit.Caps seems to stick to my lip a bit,dry like.Will things get better in time or no? Had them done about 4 weeks ago. Thank you for your time.

A. Often an adjustment to the incisal third can solve these problems – this stops the lower lip catching on them , will make the edges feel thinner and the overall look improves

I have particles getting stuck between two crowns, what can be done?

Q. Dear Dr, I had two crowns put next to eachother on the lower jaw at the far back a year ago.While I was waiting with temporary ones they came out a few times and so I had to have them re-done and on the last occasion there were a couple of days with nothing on them. Ever since I always get meat trapped between the two crowns which can only be removed by flossing. This is awkward if I am out and so I have to have a vegetarian option or be careful to chew on one side only as it often feels uncomfortable with the meat stuck. My dentist says that nothing can be done and that he has the same problem. Is this correct? The original tooth was badly broken.

A. If there is a gap between the crowns then meat will always get stuck, it might be worth getting it checked with a view to replacing one of the crowns to create a tighter contact

After root canal tooth has broken partially can it be fixed?

Can bonding cover up visible silver caps?

Q. My 5 year old daughter had to get 2 silver caps on her lower back teeth they are visible and she is very upset. The kids are starting to tease her at school and they are the only option for treatment. I was wondering if Bonding would stick to the tooth to cover the silver. Looking forward to your response.

A. This is not possible – she must have had very decayed teeth and the silver caps should reduce the chances of her loosing space for her adult teeth in the future.

I have smelling tooth with my crown coming off, what should I do?

A. You need a dental exam and proper evaluation of what is left as if it is smelling there is also likely decay

Would there be any problem in fixing refitted crown which is sticking out?

Hi, I hope you can help me, i have recently had acrown and a veneer fitted on my two front teeth, my crown came out after only a week. my dentist has put it back in with a stronger cement however has pushed it froward when putting it in. now it sticks out further than my other teeth. my dentist has said she can take it out but will have to numb it and there is the chance the root might crack or the crown. do you have any advise on what to do. Need to know positive and negative of root canal treatment.

Q. Hi I have been diagnosed with a cracked LL7 tooth, and have had a crown put on to stabilise it. We can’t tell if the crack has affected the root or pulp. 4 weeks on after the crown was fitted, I am still suffering grinding, aggressive pain all day in the tooth (though I can get some sleep at night). It’s more acute with hot and cold and sweet foods. I can just about use the tooth, but not to eat anything hard or crunchy, as this is uncomfortable too (though the crown has helped to make it less painful). I am taking painkillers every few hours and am not coping with being in pain all day. My dentist says it can take a few months for nerve pain to diminish after it’s been stirred up, but I can’t stand this kind of pain for much longer. It’s interfering with my quality of life. Would it be best to ask for a root canal to be performed on this tooth so that the pain situation is resolved? Presumably if this is done the tooth cannot hurt anymore? Does having root canal cause any longterm problems for a tooth?

A. If the nerve damage becomes irreversible, root canal would be an obvious choice to consider. The are some drawbacks but the alternative is extraction. Rob

Can old age person have dental implants and teeth whitening?

Q. Hello, my mother who is 76 wants to have her front teeth made less prominent.she has been told by her dentist that dental in plants would help and also to have teeth whitening. I was woundering if porcelain cosmetic crownds would be just as good, also cheaper.

A. This may be the case if the teeth underneath are healthy and the gum health is good.  A diagnostic waxup to assess the position of teeth will be necessary before proceeding – hope this helps

How much cost am I looking to replace my metal crown for porcelain ones?

Q. I had 10 metal bonded front upper crowns 19 yrs ago.They still look ok ,however I woudn’t mind replaceing them with more natural looking porcelain ones .Is that easy to do? Also due to acid erosion(eating disorder) my back teeth now need crowned ,can I have it all matched up.How much cost am I looking at?

A. You can have this done.  Whether it is easy depends on how you cope with dentistry. Presumably pretty well if you already had plenty of dentistry before.  The cost per arch for a full set of crowns is in the region of £12000 to £15000

Root has lost small piece can it still support post crown?

Q. I have post crowns on my two front teeth one i had replaced less than a year ago because it kept coming out it has come out three times since being replaced, and to be honest, although i was told a longer post would solve the problem the post does not look any longer. The root has lost a small piece and i am worried i will end up with no root to put it in to.

A. There comes a time when the root is unable to support a post crown, IF the root is no longer predictable , consider extraction aand replacement with bridges dentures or implants.Rob

Veneers have broken should I consider rebonding a veneer or crown?

Q. Hi Rob, I have several (5) Veneers . They have lasted be for around 15 years. Recently one has broken ( cracked and 1/3 of it fell off). My dentist would like to replace it with a crown as he is concerned about rebonding a veneer. Personally i would prefer another veneer. is there an expert opinion on the pros and cons of this repair

Please compare different cosmetic treatment for my dental issues.

Q. Hi, I am desperate! I had to have my top teeth removed about 16 months ago due to absess’s and severe infection. Previously had very healthy teeth. I had some gold crown at the bottom which sheared off so a few bottom teeth were removed prior to the top. My dentist used a sedative on me to remove my teeth but with awful consequences, I am allergic. Was ill with low BP for 3 weeks after. He made me a new set of dentures 6 months after removal. They did not fot. He told me to glue them in! He then made me a new set last April. The bottom set did not line up so he threw them straight in the bin and just gave me the top set. They do not fit they rock and fall out if I yawn or anything. He told me to glue them in. I only have the bottom 6 teeth nearest the front of my own. My face in now older looking, I can not eat only soggy or mashed food. I have not eaten meat now for 16 months. I am very depressed with it. I am only 54 years old. That dentist has now left the practice and a young new one has taken his place. She said she would make me a new set. but then questioned when I last had a set and accused ME of binning them. I am not a wealthy person so had to have a NHS set last time. I have an awfull gag reflux so taking impressions is horredouse to me. I am on disability benefits due to medical problems, e.g. Anaphylatic, svere allergys, Asthma to name but a few. Now she says I have severe oral thrush so she can not make any dentures, and I should make an appointment some time in the future. I am really sad and do not know what I should do. What would it cost me if I tried to save up to get it sorted properly? What can I do ?

A. The costs vary private dentures range from £600-1200 for a full set, implant retained dentures about £5000 per jaw ( very very roughly) There is a potential issue regarding the stability of your jawbone and hence the fit of dentures if your weight / general health is fluctuating.
