Old crowns need replacing but I am worried about possible results

Q. hi rob. i have 2 23 year old crowns that the gum has receeded back and looks terrible, i visited one dentist who recomended a 5 tooth bridge conected to two healthy teeth…..i had a second oppinion who said to wait for the crowns to fall out and then replace the crowns due to weak roots…….now im frightened to do anything……cannot smile and miserable…please advise,

Should I get two implant or just have bridge done?

Q. I am a female, 21 years of age. I have a hereditary problem where I have missing permanent teeth under 6 of my deciduous teeth. Before my orthodontist put braces on me, he pulled out 6 of my deciduous teeth as through as X ray, he could tell that they were weak and the roots were not really firm/stable. 1 deciduous tooth was pulled out in the top right part of my mouth, and another 1 at the bottom right part of my mouth. Both of them are at the same location if you count from the mid line of my mouth. 2 other teeth were pulled out from the top left part of my mouth and another 2 right below it, on the lower jaw. My doctor told me that I have to get 2 implants put in on the upper and lower left side of the jaw (where 4 teeth were pulled out) as the gap cannot be closed by just braces. Well, technically it is possible but the midline would shift, causing my smile to be ugly. In conclusion, I want to know if it is worth investing so much money in the 2 implants. Should I just have a bridge done? My doctor says it will be a waste as the rest of my teeth are so perfect. What should I do? My braces will be off in July and I do not want to have the 2 gaps empty for long. Alternatively, would it be ok if I asked my doctor to close up the gap in the lower jaw and have an implant in the upper jaw? Would my teeth look weird? Please advice. Thanks a million.,
A. I just need to clarify that having a bridge is not the best long term option- as the lifespan of them range from 10-20 yrs. You also need to be aware that the teeth on either side will need to be drilled down to house the bridge teeth. This may compromise the health of the teeth- and these may need to be Root treated in the future. This will then weaken the teeth and their lifespan is reduced further. Dental implants are by far the best long term option in regaining fixed teeth. They are teeth in their own right and can last you a lifetime if they are looked after. I would recommend implants over bridges in your case as you have many yrs ahead of you. This would be an investment in the long run. As for closing the gaps with braces, this can- as you quite rightly said, create an unsymmetrical smile with the wrong size/shaped teeth in the wrong place. Hope this has helped.

Has crown been fitted correctly?

Q. Hello, I had a 4 teeth bridge fitted last week on the lower left side of my mouth. I have found there is a large gap between the bottom of the bridge and my gum, I can also feel with my tongue the supporting anchor teeth under the crowns as there is a gap there also. I was under the impression that a crown covers the whole tooth back and front. Does this sound like it’s been incorrectly fitted? Thank you for your time.,

Would a bridge or implant be best for replacing missing front tooth?

Q. I had to have my top right second from front tooth removed yesterday due to an injury and my front right top tooth will need a crown. I have a temporary denture for the time being and my dentist has suggested a bridge for my missing tooth. Is this the best option or should I consider an implant. I am 37 and the rest of my teeth and gums are healthy. What would you recommend. Kind regards,
A. Hi It is very difficult to comment without carrying out a full examination. An implant would be an ideal option, because the adajacent teeth would not be touched, unlike a bridge.

Unfinished implant treatment is causing me great trouble

Q. Hi i WOULD LIKE TO ASK YOU IF IS ANYTHING TO BE DONE IN MY SITUATION.i START A FULL MOUTH RESTAURATION LAST YEAR,i MANAGE TO HAVE ALL THE EXTRACTION DONE AND SIX IMPLANTS ON MY UPPER JAW.NOW MY CIRCUMSTANCES CHANGE I M ON INCAPACITY BENEFIT,I HAVE A HC2 CERTIFICATE WICH NOT EEMS TO HELP ME MUCH AND MY DENTIST IS NOT PAYING ME ATENTION ANYMORE.I M STUCK IN THE MIDLE WITH A TEMPORARY BRIDGE WICH IS NOT DONE AND FIT IT PROPERLY AND MY DENTIST EVEN SUGEST TO TAKE OUT MY IMPLANTS AND HAVE A DENTURE INSTEAD WICH I THINK IS RIDICULOS.I OFFERED TO PAID THE DIFERENCE FOR THE ABUTMENTS,DIFERENCE ON CROWNS PRICE TO HAVE FIX ATLEAST MY UPPER JAW BUT i CAN’T FIND ANYWHERE SOMEONE TO HELP ME.i BEEN REFUSED BY DENTAL HSPITALS BECAUSE i M NOT AN EMERGENCY BUT THINGS GO WORST WITH EVERY DAY PASSING,CAN’T EAT PROPERLY,MY STOMAC START TO HURT,LOSE WEIGHTS ,GET INFECTION IS LIKE A KNOCK EFECT.ANY HOPE?,
A. Unfortunately there are no other options availabe. It is either having a denture of finding a way to finance the rest of the treatment.

Does cost of treatment seem reasonable to you?

Q. Dear Dr Caplin, I would be very grateful to recieve your view and informal advice. I am a British ex-pat living in Bahrain and following a visit to the dentist today, I would like to get a second view. The dental practice is well regarded but in Bahrain I am cautious of local health care. I have had some discomfort in my upper left middle molar (the one next to the back molar). FYI I have had my wisdom teeth removed some 15 years ago (I am 35). The discomfort is in the form of a sharpe pain with some cold foods, and recently sometimes when chewing. The pain does not last beyond a split second or so. The dentist x-rayed the area and did not mention any problems or decay. He said I simply needed to floss 3 times a day. He said (whcih I saw on the x-ray) that there is a gap between this tooth and the back molar of about 1mm or so, and that this is the cause. Meat gets lodged in the gap and apparantly causes decay/acid which is causing the pain. Simple flossing will stop the pain. He also recommened that I consider a bridge between the two teeth to block the gap and prevent food lodging. My first question is: Is this the only problem that is likley to be occuring? It seems a little straight forward. Is it possible that regular flossing will stop the pain and sensitivity? Or could there be something more significant going on? My concern then arose when he spotted my two front crowns. These are in need of replacement as they are over 15 years old and I have some gum disfiguration (very minor) from the few occassions I have knocked the caps out in my teens. He then suggested that if I had a budget I should get the front caps sorted to give me a better smile and use floss for the molar. My concerns are not cosmetic, but are with the pain in the molar so I want to ensure this is sorted as a priority. He quoted me GBP 1,200 for the molar bridge and GBP 1,400 for replacement of the two front caps. Having not had major dental work done in the UK for some time I would be grateful if you could advise if these costs seem reasonable. I’m sure they are not the cheapest in comparison to some UK dentists however, I would prefer to pay an appropriate amount (i.e. not the cheapest) to recieve good quality treatment. A. It sounds as though the appropriate tests have been carried out – the fees also seem reasonable. I would advise you not to have the bridge before your pain has subsided from flossing the area in case there are other issues with your tooth

Can I get teeth removed and replaced with bridge due to gum disease?

Q. i have a large over bite and my upper teeth are very lose. I tried to have my dentist put braces on them but was informed that due gum diseas it would not be a good idea to have braces because i would lose them anyway. Could I have those teeth removed a get a bridge instead,
A. For you to get a bridge in the first place you require surrounding teeth to replace the gap, which the bridge holds on to. I suggest you get on top of your periodontal condition (gum disease) by regularily seeing a hygienist or periodontist and then I would put braces on after your gums are stable. Justin Glaister

What tooth replacement options are there other than dentures?

Q. I am a 48 year old female with loose front teeth caused by receding gums and being hit in the mouth due to horse riding accidents. I am very self conscious of my smile. Please could you tell me if I have any options other than Dentures and what sort of cost would be involved. I am desperate to have something done and soon because I am terrified they might fall out when on a date. Thank you very much.,
A. Your other options are a bridge or implants. A bridge would normally entail drilling down adjacent teeth where an implant is independent from the adjacent teeth and stimulates the bone. It sounds like you would need a thorough examination and scan to decide whether you have enough bone for implants, if not you may need some bone rebuilding prior to the procedure. Regards Andrew

Does a bridge help to prevent bone loss?

Q. Implants seem like a great solution to missing teeth but they are expensive and take a long time especially as I need bone grafts too. Would a bridge, say for my two front teeth slow done bone loss as much as implants?,
A. A bridge won’t stop bone loss as the bridge teeth sit on the gum and do not provide any stimulus to the bone. This stimulus the implant provides stops the bone shrinking back

What would it cost to replace loose teeth?

Q. I need my three front teeth replacing. They were bridged several years ago but now they keep comming loose, my NHS dentist says he cannot do anymore and I should have a plate fitted. I would prefer to keep my teeth without a plate. How much would it cost, and how long does the treatment take.,

What options are there to fill tooth gap?

Q. I’ve had an extraction next to my wisdom tooth, what options are there available to fill the gap. also could you give me some indication of cost and time and visits needed for the procedure. thanks,
A. Hi The options available to fill the gap would be a denture, a bridge or an implant. The process to make a denture would take a series of weekly/fortnightly appointments, in order to ensure the best fit and aesthetics for the final result. The cost for a denture would start from around £250. In respect of a bridge, this would involve a preperation appointment, where either one or both of teeth either side of the gap would be prepared for crowns. An impression would then be taken and sent to the laboratory for a bridge to be made. This would consist of either one or two crowns with a false tooth attached which would fill the gap. The cost for a bridge would start from around £200 per unit. The final option would be an implant, which involves a titanium implant be placed into your jaw, and once this has integrated a crown would be made which would attach to the implant to fill the gap. Is it possible for me to have bridge instead of implant?

Q. Due to an abscess I lost a tooth, the second one from the back upper right side, I was told I couldn’t have a bridge as I had virgin teeth either side and to opt for an implant, is it possible to have a bridge and is it the cheaper option? Although you can’t see the gap when I speak, I do have a big mouth and wide smile and it makes me feel very awkward now in company and I tend to smile lopsidedly. Many thanks,
A. The most conservative treatment would be to have either an implant or a single tooth denture, as this would eliminate the need to prepare your healthy teeth. However, it should be possible to have a bridge, if you decide that you are willing to have the teeth either side of the gap prepared and fitted with crowns. The gap will then be filled by a false tooth which would be attached to the two crowns to form the bridge. I hope this will help. Many Thanks. Dr Solanki

Is it possible to have crowns fitted over my bridges?

A. You could have porcelain venners placed over the bridge but they are going to be quite weak and could break off. You need to find a cosmetic dentist so some alternative options can be found such as dental implants.

Is there a temporary fix for missing tooth?

Q. I am missing a tooth at the bottom near the front of my mouth. I think it is number 13. I am a soldier in the army and i am about to deploy to Iraq. What kind of temporary fix can a dentist do to fill this gap until i get back? It is so unbecoming.,
A. It should be possible for you to get a denture made, quite quickly which would fill the gap. Alternatively, it may be possible to have a bridge made. This would depend on the amount of time you have before your deployment, and would involve the preperation of the teeth either side of the gap at an intial appointment. Can veners of lumineers help with absent front tooth?

Q. i’m missing 1 of my froth teeth can lumineers and veneers help,
A. Unfortuantely, once you have lost a tooth the only way to replace the space is with either a denture, bridge or Implant. Lumineers and veneers are bonded onto existing teeth which would not work in your case.

All teeth are broken so what treatment is best?

Q. I have Broken ALL my teeth and have realy bad tooth pains evry month. I would like to know what your recomendations are and what treatments are available because I think all my teeth are beyond repair and I am quite woried because I do not have alot of money and would like to know the cost of treatment.,
A. I can’t really tell how bad they are but you might be looking at either fillings and crowns or extraction of the bad teeth and replacement with dentures, bridges or implants. You need to see a dentist as soon as possible you may also qualify for NHS treatment if money is tight.
