Q. please could you give me some advice. I started having pain in my left cheek 3 months ago, the pain then radiated in my top left molar and across to my 3 front (top) crowns, then it went into the corner of my eye down the back of my head in my neck and in my left ear and around my jaw. I had the molar taken out then a tooth next to the 3 front top crowns towards left side, and then a wisdom tooth taken out top right hand side. This was all in the space of a week. The pain after the extractions hasn’t changed. I was told by one dentist that I have grinded my teeth quite severly. I was then diagnosed as having TMJ by a prof who to be fair has been really good. She told me that when I talk I push my chin forwards and said my bite isn’t bad but not great. I think this is because my 3 front crowns don’t meet my bottom teeth anymore and they feel really uncomfortable, they haven’t done for a while. I have been given a splint only a week ago which i know takes time. The pain is still the same it seems to becoming from My 3 front crowns (had them in for 12 Years)had lots of problems with them in the past had a root canal etc, and for a while now the crowns have been petruding forwards and the pressure I feel is horrible I feel like I just want to rip them out. The pain then radiates in all my teeth. I know this is quite a lengthy question but I am reaaly tired of being in pain as it wakes me from my sleep I also find myself having to constantly bite down all the time especially in my sleep. I really need to get sorted as I hate my 3 young children and husband seeing me in pain all the time. I was really thinking of having the 3 crowns removed and replacing with dentures would this be the right move?. Any advice would be much appreciated. Thank you
A. There is a possibility that you have trigeminal neuralgia as I would have expected your pain to have at least improved from the splint. The first treatment of choice is using carbamezipine and you may suggest this to your dentist or specialist.

Q. Hi there Andrew. Thank you for offering your advice here. I’ve had a porcelain and metal bonded crown on my main front left tooth for the last 6 years. It is fitted on a small stub (my actual tooth) Recently my porcelain crown broke in the middle of the night. It just fell apart at 5am. I didn’t bang it and dont hink i have ever banged it as im very carefull with it. Only the porcelain broke off… the black metal is still on my tooth with the permenant dentist glue. I have a HC2 certificate. What is the difference, in detail, between an NHS crown and a private crown? Shouldn’t the quality of a NHS crown be the same as a private crown? Am I entitled to request, under my entitlement to rights from NHS dental treatment received, that i would like the NHS crown to be the same quality as a private crown? Am I entitled to an all ceramic crown under my NHS HC2 certificate rather than a porcelain/metal bonded crown? It is for my main front left tooth. Looking forward to your advice. Many many thanks.
A. Like most things in life if you pay extra you can get abetter service or quality. Hence it is possible to pay more for a better crown, maybe one that is stronger, looks better or of a superior material. Unfortunately the NHS crown cannot be the same quality as a private crown as the lab costs aren’t covered by the NHS fee. You should discuss the options with your dentist and see what you could do.

Q. Hi i had new crowns fitted at the front , top and bottom,the right hand side below and the left hand side top. Everything was fine until i had the lefthand bottom done, i ended up with an infection but it cleared up i had a root canal and crowns went back on some of the crowns are also a bridge. From then on which is over 10 months ago i cannot eat anything on that side at all cannot even press down with my finger as it hurst too much. I went back to the dentist twice and one of the answers he gave me was my body could get use to the pain or use sensitive toothpast. I went to see another dentist about 3 weeks ago and he said my bite was wrong, i went back to my dentist today (different dentist as mine had left from that surgery) and he said they were a good job and one of my own teeth at the top may need filing down, never had any problems before crowns. My jaw has now started cracking on occasions but he said that sometimes happens. My tongue on the opposite side of this now has a white patch on it where my tooth is catching on it due to being only able to eat all the time on that side. So what do you think should be my next step i have never been pain free since i had these done. Thanking you.
A. Assessment of your bite would seem the first treatment of choice after Xrays have been taken to assess for abscesses present

Q. I have small teeth and when I smile you can see quite a lot of gum. I would like big white teeth and heard that there was a procedure that could scale gums back. Is this true? Also, could I have this treatment in conjunction with having veneers fitted and overall whitening to create that ‘hollywood’ smile look? How much should I expect to pay and are finance packages in usually place if I am unable to pay the full amount up front?

Q. I had 2 caps done that I wasn’t really sure about the way they did it. That is, they joined the two teeth together, rather than making them individual caps.
A. Usually 2 crowns that are joined together is called a bridge. Tis is done when a false tooth replaces a gap in the mouth. I am unsure of your case as I haven’t seen you in person, but I would strongly advise that you see your dentsit and discuss any concerns that you may have. Having seperate crowns is the best way- as cleaning and flossing around it is much better and that way may help prevent food packing and gum disease.

Q. I had 2 caps done that I wasn’t really sure about the way they did it. That is, they joined the two teeth together, rather than making them individual caps. The first go around they were individual caps but alot of food was getting stuck in between and so they join them…I’m not sure I like that idea…two caps joined together, what do you think?
A. It works well if the teeth tend to drift apart causing food packing. However, you must see your dentist every 6 months to check the 2 teeth and to give you advice on their care to avoid gum problems and decay underneath.

Q. as i am getting older i am getting a gap in between my front teeth ( i am 52) which i hate. i dont want to be without a front tooth whilst treatment is carried out so what would you suggest, an implant or a crown?
A. It is possible to replced teeth immediately however it might be a case of filling the gap using a simpler treatment such as veneers or fillings

Q. hiya im looking for some advice i went to the dentist a few weeks back as my crown had fallen out the dentist that put it back in again didnt bother to clean my mouth or the crown so ten days later i swallowed it went to a different dentist and she said i couldnt get another crown fitted as i had an infection but she hasnt given me any antibotics also she wants me to get a false tooth fitted will this be sore getting the root out and how will she be able to fit it on i have to go back on friday and im scared
A. I guess the false tooth will be fitted on a plate when the tooth is removed, this might act as a good temporary replacement until you decide whther to use a more permanet solution such as a bridge or implant.
The root normally comes out fairly easily and with a good local anaesthetic you shouldn’t feel any pain.

Q. I had a crown put on a tooth of mine. When my crown came in I was called yo the office to have it attached. It was to long so she remove it and took a drill and shorter so my bite could be even. While doing this I heard her say something to her assittant. When she put it back on it didn’t shap in place but she used it anyway!! then she whet about bonding it better be doing something like filling a cavity. Is this the right thing to do? Now it has broken off and she has moved to a different office. and she told me at the time that a cap is not garinteed. Did I get taken adventage of her? I do not have insureance for Dental. Did I get robed. and should I get it recaped. Thank You
A. You need to take thi up with your dentist, I guess you are from the US so I’m not sure about the time span of guarantees offered by practices in the States

Q. I THINK I MAY HAVE CRACKED MY CROWN IM NOT FOR SURE ALL I KNOW IS I WAS EATING SOMETHING AND I FELT LIKE I MAY HAVE OVER BIT MY FOOD AND PUT TO MUCH PRESSER ON MY CROWN IS IT POSSIBLE IF I CRACKED MY CROWN IS IT POSSIBLE TO GET IT FILLED THAT SAME DAY I GO TO THE DENTIST.WHAT DO THEY DO IF YOU CRACKED YOUR CROWN?I CAN’T SEE IT BUT MY TOOTH JUST DOESN’T FEEL THE SAME AND IM PRETTY SURE IT’S COMING FROM THE CROWN. Q. I was just wondering how much a single tooth replacement would cost. I know an implant is £2000 per tooth wich is fitted later with a porcelin crown but how much is a implant retianed denture tooth.

Q. Hi i have 2 porcelain crowns on front top 2 teeth(my own teeth in part are underneath), 1 has become a little lose, my dentist says it’s wobbly but not wobbly enough to fall off(he pulled the crown before he siad this) however when i grip it i can feel movement(slight rock backwards & forwards and a clicking noise) i am about to go on holiday so i am very worried. Please can youy advise if there is such a thing as wbbly but not enough to fall out! p.s how lon will tempory cement hold a crown for? Your reply would be greatly appreciated. Many thanks
A. there is very small risk that even a solid tooth might fall out. the risk of a wobbly tooth is a bit higher. however if it not willing to come out with moderate pressure I would suggest it is better to risk it falling out than risk damage by using too much pressure to remove and recement Rob

Q. in a few days i will have a root canal done but i can not at this time afford the crowns should i still have it done
A. You should get the root canal finished in case an infection occurs. Maybe a strong core filling would suffice until you can afford to get the crown sorted out

Q. i had a crown fitted a couple of years ago but it has always felt tight and iam constantly chewing on it and at times want to pull it out. Iam considering asking the dentist to take it out but is that my only option.
A. It might be infected underneath, causing pressure. Perhaps he could x-ray it 1st then decide if there is a possibility of root canal intsead of removal of the tooth.

Q. hi, i am living in korea and visited the dentist here in order to ask about my tooth, its one of my top lateral teeth that has moved forward and towards my front teeth. The dentist said it was due to my wisdom teeth coming in, although i have all my wisdom teeth now. ihe advised me that i could either get a crwon put on that one tooth to make it straight or get my wisdom teeth out!! would a crown look obvious that it was fake? or would my tooth maybe straighten back to where it was if my wisdom tooth was taken out? im not sure which to go for and the language barrier is not helping as we discuss it all through a translator who ddoes not know much english either!!! thank you
A. Placing a crown is an option but I feel it is very agressive, once that tooth is cut down it will always need more treatment in the future. I would much rather you had orthodontics to straighten your natural teeth as it would have much better long term success and look better. You can email me photos of your teeth if you need more help. If your current tooth sticks out, a lot of it would have to be removed to make it look straight and the new tooth would appear much thinner which will not not look as nice.

Q. Hi, I have recently had a crown and a veneer on my two front teeth, the crown came loose after a week and so when i went back my dentist put a harder cement in my gum, hoever when she has put the crown back in it has not been positioned correct and is no further forward than my other teeth. I have been back to the dentist and she has told me if they take the crown out and put it back in there is the chance the root could crack or the crown could crack. Do you know what i can do as i am really unhappy with the out come? Thanks
A. If you’re really unhappy with the way iy looks it might be bets to remove the crown and start again unless a minor adjustment would suffice
