			<content:encoded><![CDATA[<p><strong>Q.</strong> Hello there, i have just come back from a visit to the dentist i have not gone for a number of years due to being so scared of what they would say and do!! I have come back with an apptionment in December for 2 fillings and to have a chipped toothe capped, he has put a temp cap on it, my main question is that i have slilt Gingivtis, thius has scares the living days out of me as i am only in my 20&#8217;s, he tld me to change my mouth wash, and get a soft Toothe Brush and it should get better, what toothe Paste would be the best for this??? And how long should this temp cap last for??? Many thanks</p>
<p><strong>A.</strong> The temp cap should last till your next visit but be careful with it. If you have bleeding gums you could try corsodyl mouthwash for a week or so but it can stain the teeth. Ask the dentist if you can see his Hygieinist to give them a&nbsp; good scale before your next visit.</p>

			<content:encoded><![CDATA[<p><strong>Q.</strong> Hi, I have just found out that my top right rear molar, is dying as a result of a deep filling, which is now beging to rest on the nerve of the tooth.  The dentist has shown me the x-ray and said that as the root is so bent and not easily identifiable, he does not believe a <a href="http://www.cosmeticdentistryguide.co.uk/articles/root-canal.html">root canal</a> would be sucsessful. I have also had a bad expererience of <a href="http://www.cosmeticdentistryguide.co.uk/articles/root-canal.html">root canal</a>&#8217;s, where within months I had absess, and lost the tooth anyway.  My dentist has said that my only option really is therefore too losse the tooth. My concern with this is that due to braces and overcrowding I had already had all my <a href="http://www.cosmeticdentistryguide.co.uk/articles/wisdom-teeth.html">wisdom teeth</a> removed and to loose this back tooth, I run the risk of my jaw bone disolving, there is already evidence of this on the x-ray following the wisdom extraction 3 years ago.  I asked if there was any other options and he told me I would need splints and a crown which would cost about &pound;2000.  I am only 31 and worry that if I don&#8217;t make the right choice now it will be too late, I can&#8217;t afford &pound;2000 for a tooth, but as it stands a booked for an extraction the week before xmas??!!  Reading some of your other responses to questions indicate the crown can be placed over the tooth, can this not always be done?  Any advise would be great.  Thanks</p>
<p><strong>A.</strong> hi many thanks for your enquiry, my advice would be to ask to be referred to an endodontist for a second opinion. THere are many teeth which general dentists just do not have the skill to treat. however an endodontist is highly experianced at <a href="http://www.cosmeticdentistryguide.co.uk/articles/root-canal.html">root canal</a> treatment and can give you an idea of whether they feel it can be treated and level of sucess. once the <a href="http://www.cosmeticdentistryguide.co.uk/articles/root-canal.html">root canal</a> treatment is done it is then best to crown the tooth. </p>

I suffered a lot of pain shortly after it was fitted and at an emergency appointment with a dentist it was necesary for a hole to be put in crown to relieve pressure caused by bleeding.The hole was of course filled afterwards. The crown is still sensitive from time to time &#8211; I get pain or aching or strange sensations which lasts for a few days and then it settles. Is this normal and if not what is the likely cause?</p>

<p><strong>A.</strong> the truth of the matter is you cannot be sure it will settle as not all <a href="http://www.cosmeticdentistryguide.co.uk/articles/root-canal.html">root canal</a> treatments are a success. i can only guess that it was a difficult procedure as it took so many appointments to do it.<br />root canals can give symptoms for a yr post treatment and it is their severity that determines whether you give it time or take further action</p>

<p><strong>A.</strong> Usually my treatment for such a situation is to crown the damaged teeth due to the backs of the teeth being severely damaged by acid and the fronts of teeth looking short due to acid damage to their tips.&nbsp; Sometimes composite filing material can restore the damaged surfaces followed by veneers to improve the look of the smile.&nbsp; Pleae contact my surgery if you wish for a free cosmetic consultation where I can discuss your options in more detail.</p>

			<content:encoded><![CDATA[<p><strong>Q.</strong> Hi I have a composite crown bonded to a metal post that was fitted 20 years ago when I broke my upper front incisor that I would like to replace for cosmetic reasons. Due to my gums receeding a few mm you can now see a line of grey natural tooth around the gum line. I have asked my dentist about the most natural looking replacement and she is very keen to fit another metal/porceline composite crown. I have never been happy with the one I have, it has always been a different size, shape and colour to my other main incisor so I asked about an all porceline crown and she said that her practice fits procera crowns but that because I have a metal post and not a ground down tooth to use as an anchor that an all porceline crown would be too thick and would break. I made it clear that cost was not a consideration but she insisted that a composite crown was my best bet. Apart from the crown my teeth are completely average, they are average size, shape, colour etc. Your opinion would be valued. Thanks</p>
<p><strong>A.</strong> There are number of different options for porcelain crowns that can be used for different situations to&nbsp;produce a great result.&nbsp;&nbsp;Composite&nbsp;crown sould not appear to be the obvious solution unless the prognosis for the tooth was poor &#8211; to keep&nbsp;costs down.&nbsp;&nbsp;They are&nbsp;sometimes used to reduce the bite forces on implants. For a dark post a procera or zirconium or a well made porcelian bonded to metal crown would be my choices.&nbsp;</p>

<p><strong>A.</strong> No, the gum line will settle into the position it needs to be in. You could consider corrective gum surgery if it wories you.</p>

<address>There are different types of all porcelain crowns with flexural strengths ranging from 56MPa to 900Mpa.&nbsp; If your teeth are not especially worn all porcelian crowns do very well &#8211; even in bruxers the zirconium crowns900<br />pa&nbsp;seem to be doing well if properly made.&nbsp; Porcelain bonded to metal substructure can also fail as the prcelain can chip if the bite gets in the way</address>

<p>The zirconium crown is metal free and may look more lifelike ( depending on who makes it).&nbsp;&nbsp; Both crowns should &nbsp;fit very accurately and protect the underlying tooth</p>
