Q. Dear Dr. Patel. I have just had a replacement bridge and whilst the two crowned teeth supporting the false tooth are well fitted the centre false tooth has a gap at the gum line and appears to be leaning out a little. The dentist has filed it to reduce the bulky appearance but this has not helped and it is not as good as the original bridge I had done 8 years ago by the same dentist. The dentist is trying hard to please me and has arranged to put a veneer on the FALSE tooth. Is having a veneer on a false tooth common and how durable will it be. I would be very pleased to receive a reply. Thank you very much.
A. I think that putting a veneer over the tooth may not work the bridge needs to be removed and if it can be taken off in one piece can be sent to the laboratory and fixed, if not you will need a new bridge.

Q. Hi, My 12 year old daughter is in the middle of her braces treatment. Her orthodontist noticed that she has 2 missing upper teeth (I think they are the premolars.) Now that her lower premolars are coming out, the orthodontist suggest that she has them removed. He said that number of teeth on her upper should be the same amount as her lower. I wonder if that’s true. Right now my daughter’s x-ray should that she has 2 bottom molars that have not come out yet but no sign for her other upper molars. She now has 10 teeth on the upper and 12 on her lower bridge. I’m worried that if I have the other 2 lower teeth removed she will not have enough teeth in the future! What is your opinion?
A. If she is crowded then removal of the teeth is indicated. When all the teeth come through she won’t miss the premolar teeth. Talk it through with your orthodontist before you proceed with treatment

Q. I got procelin crowns and I notice by my Gumline the Skin still didn’t mold into Place which is causing a slight space, the skin is still in the Back. Is there anything I can do to get the Skin in place? thanks
A. No, the gum line will settle into the position it needs to be in. You could consider corrective gum surgery if it wories you.

Q. I need a crown for one of my upper central front teeth, my dentist has suggested that I have the crown which is totally made of porcelain – it is the most expensive option but my main concern is will it be as strong as the porcelain crown with the metal interior?
A.
There are different types of all porcelain crowns with flexural strengths ranging from 56MPa to 900Mpa. If your teeth are not especially worn all porcelian crowns do very well – even in bruxers the zirconium crowns900
pa seem to be doing well if properly made. Porcelain bonded to metal substructure can also fail as the prcelain can chip if the bite gets in the way

Q. I am in a dilemma as to whether or not I should go for a bonded gold or precious metal crown or zirconia crown for my LL6. What are the advantages of the more expensive zirconia crown? Many thanks for your time and expert advice.
A.
The zirconium crown is metal free and may look more lifelike ( depending on who makes it). Both crowns should fit very accurately and protect the underlying tooth

Q. I have 4 crowns in the front (top) I just noticed as I was flossing today the area I thought had food seems to be more like a wearing of the side of the crowns (2). Does this happen and what do you do about it besides new crowns. I know–go to the dentist. I have had them approximately 17-20 years. Thank you
A. crowns do not wear on their side- you probably have decay and need new crowns

Q. I had my root canal retreated. I have a gold crown and the retreatment was drilled thru the top of the gold crown, can the crown be filled with a gold filling?
A. Yes it can but the cost benefit is less than replacing the crown. A simple amalgam can be used as a short term repair as an alternative.

Q. I just came back from a check-up at my dentist and I have been told that I have periodontal disease. I wanted to know from you what is about the range in price that it would cost for treament? I do have insurance, but I am trying to figure out if this office is overcharging for the treatment. I will have the treatment done but it might not be by that office. Second question I had is that my dentist wants to put 4 crowns across my front teeth and I wanted to know which kind of crowns is good not the ones that are most expensive on me? For example they gave me a price of $4,480.00 for all four crowns and I would like to know if this is a crazy price or a normal one?
A. periodontal treatment is difficult to cost but starting with the basics, ask how much time is being allocated and with who. For example 4x 30 mins with a hygeinist will have a different fee to 2 1 hour treatments with a periodontist specialist dentist. #est crowns can depend on the functional needs of the case but all ceramic crowns with a Zirconia core are good in most situations, try to shop around locally for hourly rates and zirconia crowns to compare prices.

Q. Hi Please can you help? I had a root filling done on one of my front teeth 5 years ago….the same tooth has been going black ever since. I have recently changed dentists and have discovered that when the original root filling was done a piece of cotton wool was left inside, so no matter how much i brushed my teeth it wasn’t making a difference to this tooth. This tooth is now causing the tooth next to it to discolour. My dentist has told me that because i get nhs help (full exemption) then there is nothing they can do for me? I think i need a cap or crown on it (don’t know which). The tooth is causing me real self confidence problems and i dont know what else to do. I hope you can help. Many Thanks.
A. A root treated tooth will turn dark over time, (it has nothing to do with the cotton wool) and it sounds like the adjacent tooth might be dyingas well. I would go back to your dentist and find out if its possible to have a crown on the NHS as it sounds like you are registered with this dentist and should be able to have some treatment done.

Q. Hi. Thank you for offering your advice here. I’ve had a porcelain and metal bonded crown on my main front left tooth for the last 6 years. It is fitted on my actual tooth which has been filed down to a small stub. Recently my porcelain crown broke in the middle of the night. It just fell apart at 5am. I didn’t bang it and dont hink i have ever banged it as im very carefull with it. Only the porcelain broke off. The black metal is still on my tooth with the permenant dentist glue. I have a HC2 certificate. What is the difference between an NHS crown and a private crown? Shouldn’t the quality of a NHS crown be the same as a private crown? Am I entitled to request, under my entitlement to rights from NHS dental treatment received, that i would like the NHS crown to be the same quality as a private crown? Am I entitled to an all ceramic crown under my NHS HC2 certificate rather than a porcelain/metal bonded crown? It is for my main front left tooth. Looking forward to your advice. Many many thanks.
A. Unfortunatley I am a private practitioner, therefore I am unsure about the particular forms however I think that NHS dentists are restricted to certain laboratories therefore they are unable to choose their own technician maybe thats why the quality differs.

Q. 1 tooth upper right 4 .. cost of dental implant +abutment+crown. 2 there are many types of implent systems .ie straumann, biohorizon,etc and many types of crown eg fused to metal ,fused to gold, etc. which are best implants and crowns
A.
Personally I mainly use Astra Implants which I find predictable and long lasting. I like a titanium surface which is roughened by subtraction and surface active. The Roughness should be full hieght and and micro thread at the top end to evenly load bone and maintain long term bone (and Gum ) hieght around the fixture. The internal should always be a cone fit to reduce abutment scres stress.
The best abutment and crown depends on balancing aesthetics and the loading in the UR4 region this is variable as it depends on your actual bite and the width of your smile.

Q. I am considering getting my four front teeth either crowned or have veneer on them. I would like to know the benefits and/or problems with both of these. I have had crowns before and never had a problem, but I understand that veneers are a bit more pricey and painful. Are they both comparable when it comes to wear, etc?
A. If the teeth are already crowned then it isn’t possible to have veneers on them. Veneers involve less tooth preparation than crowns but generally are not quite as strong.

Q. hi DR moore I have a crown on the upper right jaw that has become sensitive to hot food etc The pain is only mild, and seems to radiate to other teeth on that top side. There is no pain when i bite on the tooth,also there is no swelling around the tooth. what could be the problem. thanks for your time.
A. There is a chance the tooth could be dying. It would be worth getting it checked over as pain with hot is a symptom of a dead or infected tooth.

Q. hi,over the last 8-10 years my 2 upper front teeth have clearly protruded forward and it seems my teeth have become more crooked in general,i would like them staightened but there seems to be a vast array of braces to choose from. i am 39 years old and concerned whether braces work on an adult with much success,also 2 of my upper front teeth are noticeably smaller than the others,can you advise the best treatment to make these teeth the same as the others,thanks
A. Many times in these cases I will opt for invisalign to move the teeth into ideal position and treat poorly shaped teeth with veneers. As an adult the ortho using invisalign tyakes about 6mths to 1year

Q. please could you give me some advice. I started having pain in my left cheek 3 months ago, the pain then radiated in my top left molar and across to my 3 front (top) crowns, then it went into the corner of my eye down the back of my head in my neck and in my left ear and around my jaw. I had the molar taken out then a tooth next to the 3 front top crowns towards left side, and then a wisdom tooth taken out top right hand side. This was all in the space of a week. The pain after the extractions hasn’t changed. I was told by one dentist that I have grinded my teeth quite severly. I was then diagnosed as having TMJ by a prof who to be fair has been really good. She told me that when I talk I push my chin forwards and said my bite isn’t bad but not great. I think this is because my 3 front crowns don’t meet my bottom teeth anymore and they feel really uncomfortable, they haven’t done for a while. I have been given a splint only a week ago which i know takes time. The pain is still the same it seems to becoming from My 3 front crowns (had them in for 12 Years)had lots of problems with them in the past had a root canal etc, and for a while now the crowns have been petruding forwards and the pressure I feel is horrible I feel like I just want to rip them out. The pain then radiates in all my teeth. I know this is quite a lengthy question but I am reaaly tired of being in pain as it wakes me from my sleep I also find myself having to constantly bite down all the time especially in my sleep. I really need to get sorted as I hate my 3 young children and husband seeing me in pain all the time. I was really thinking of having the 3 crowns removed and replacing with dentures would this be the right move?. Any advice would be much appreciated. Thank you
A. There is a possibility that you have trigeminal neuralgia as I would have expected your pain to have at least improved from the splint. The first treatment of choice is using carbamezipine and you may suggest this to your dentist or specialist.

A. Like most things in life if you pay extra you can get abetter service or quality. Hence it is possible to pay more for a better crown, maybe one that is stronger, looks better or of a superior material. Unfortunately the NHS crown cannot be the same quality as a private crown as the lab costs aren’t covered by the NHS fee. You should discuss the options with your dentist and see what you could do.

Q. Dear Dr.Andrew I have had two implants on the lower teeth on the first and second molar. everything was ok, osseointegration, gums ect. I had an appointment 2 weeks ago for the final step, the crowns. The crown on the first molar was well fitted, but the one on the second molar kept coming out even with the seal and it was even too high( inadequate occlusion. The dentist told me that the reason is because the gum on my wisdom tooth that was extracted one week ago was still swollen so it was not permiting the crown on the second molar to fit properly.So I had to come another week later once the gum was healed. Today i went and the dentist told me that actually she mixed the abutements.SO the put the abutement the first molar on the second molar and vice versa. So she tried half’hour to remove the crown on the first molar that even if it was sealed with temporary sealing it was not coming off. then she said that since this was not coming of she was going to try to shape the wrong abutement for the next time. So i have to go another time to get it fixed. I left the dentist office very sad. I feel like now the pressure that was put on the crown is goign to hurt the implants or make it loose. I don’t understand what is goign on. Should i ask the dentist to cut the crown, change the abutements and do other new crowns? Any advice or help will be greatly appreciated. Best Regards
A. The abutment can be changed or reshaped, it won’t harm the implant. It does sound like the crowns might need replacing, ask the dentist and see what options she comes up with.

Q. I have small teeth and when I smile you can see quite a lot of gum. I would like big white teeth and heard that there was a procedure that could scale gums back. Is this true? Also, could I have this treatment in conjunction with having veneers fitted and overall whitening to create that ‘hollywood’ smile look? How much should I expect to pay and are finance packages in usually place if I am unable to pay the full amount up front?
