Q. Unfortunately I had a fall in August and knocked my front teeth. The tooth to my right of my front teeth is a crown fitted to a post and my dentist says I have fractured a nerve here. Consequently I have had an abcess and severe pain since early September. The tooth needs to be extracted. My dentist has referred me to an implant specialist but I am a dental phobic - and because I am already in pain I am frightened of the drilling involved with the implant. The alternatives ie. bridges, dentures etc, although cheaper, all seem to have their problems. Can you advise me? As the tooth is so important to my smile etc (I am 53 years old)

£200 is actually very cheap for what you want.  I would not advise a full crown as this is very damaging to your tooth.  A silver veneer may be possible and would give you the silver look!

I have a gap between my front incisor

Hi, I have a gap between my front incisor and the next tooth (can’t think what that one is called) i would like this gap closed, but have been told that i would have to have all of my teeth shaved down from canine to canine just to close this gap! A. Hi, cost is an issue dentists have to deal with. Recommending payment plans comes under “financial advice” and that means you have to be a formally accredited financial adviser.
We dont offer payments plans from our practice at the moment because normal loans are more competive. However if the credit crunch continues we may go down this route.
It is worth looking into phased treatment or alternatives to veneers such as direct bonding of resin filling to the teeth. It can look great and is about 1/3 cost, however it has a shorter lifespan and is not great in smokers / tea & coffee addicts due to staining over time

I am 21 years old i have three gaps in the upper jaw

Q. Dear Expert, I am 21 years old. Since I was a child I had been unhappy about my teeth. I have gaps between them - 3 in the upper jaw and 5 in the lower jaw, all in the front. In addition to this, my teeth in the lower jaw are very small. My parents or dentists I had visited in the past were never concerned as my teeth were always very healthy, and I was also told that braces would not help as my teeth are either too small or moving them would cause more serious problems. After many years of ignoring the problem, I feel that I should no longer be afraid to smile widely and would like to do something about this. In addition to filling the gaps, I would like to whiten my teeth. Could you please let me know what I should start from, what kind of procedure may be involved and what are the potential (approximate) costs?

A. Treatment for gaps is often either closing the gaps  by orthodontics or filling the spaces between the teeth by cosmetic bonding or veneers.
You and your new dentist  need to get some understanding of why moving the teeth might have caused a problem. It might effect the treatment route.
If teeth have very short roots with a poor long term future, dental implants and bridges might be an answer.
The costs of these treatments varies greatly  from £1000 at the cosmetic bonding end to around £8,000 for implants and these are very rough guide prices.
I recommend starting with whitening and gettng a couple of assesments to determine your best option within your price range.
Our practice does not charge for an initial assesment with a dentist if you mention the cosmetic guide.

When your new teeth are planned I would ensure your bite is much better than it is now sometimes this reduces the grinding naturally.  A night guard is advisable to prevent your new teeth from breaking.  But getting your new teeth to be the correct shape and bite for your jaw are key to having a successful new smile.

A. hi many thanks for your enquiry, you have what we call a class three malloclusion. unfortunately due to the size of the overbite your only option would be to have surgery to correct this. hope this is helpful.

Having had 4 metal based porcelain caps fitted to my four bottom teeth

A. Hi , you can have the bonding replaced but don’t have it done with your dentist. He may not be able to do it very well and the fact that he suggested Lava crowns for your anterior teeth means he doesn’t have a grasp of the different types of restorations that are suitable for anterior teeth. Lava crowns require 2.5mm reduction around the whole tooth which is a lot considering veneers can be done in 0.2 mm thickness. Lava is a zirconia restoration which is very opaque and many cosmetic dentists do not use this material in the anterior region due to this fact. Have an opinion from a restorative/cosmetic dentist to find out your options for the two upper front teeth.

I was recently obliged to have a bottom far rear molar tooth canal treated

Q. ello, I was recently obliged to have a bottom far rear molar tooth root canal treated, but when I went to an NHS dentist afterwards for a crown he told me that wouldn’t be necessary for six months. Now I read that uncrowned root canal teeth are at risk of fracturing especially with a heavy bite like mine. What is your opinion given that I would be hard pushed to finance private crown treatment having already laid out, what for me is a very large sum of money (£560) for the canal work. Also, after four weeks the treated tooth is still sensitive to the touch (though no actual toothache as such). Is this a cause for concern? Thanks for your advice

A. The consensus recommendation from Europe & USA is to procceed to a crown sooner rather than later. This view was adopted over 10 years ago, prior to that a delay of 12 months was suggested to confirm evidence on radiographs of healing. I would be concerned if the tenderness was getting worse or not resolving

I had a serious car accident whilst on holiday in turkey and had jaw surgery

Q. Hello I had a serious car accident whilst on holiday in Turkey. In brief, I had jaw surgery and was wired for more than 4 weeks. The wires have been removed now. Its been like 6 weeks now since i had jaw surgery. I also had broken/chipped teeth because of the accident. My questions are: 1. How long should I wait to get my teeth sorted - to get veneers? 2. Should my travel insurance be responsible for my broken teeth as well? If yes, how do deal with insurance payments? Hope to hear from you soon

The teeth affected should be checked that the nerves inside are alive ( vitality test).  If they are are not the teeth should have the nerves treated and then all the damaged teeth restored with either white fillings, veneers or crowns depending on how severe the damage is to each tooth.  I cannot help you with the insurance issue - best advice is to contact them directly to find out if your policy covers you.

I am now looking to have a dental bridge put in place as i cant stand having the plastic in anymore. I cant smile and even some words make the denture fall out. My dentist has said implants prob wont work now because the gum in my mouth has shrunk, so a bridge is the best option. A. Implants can still be done you may just need an extra procedure to re grow the bone and gum.  implants are a much better longer lasting option than a bridge.

I was told that I require five fillings

Q. HI! I was at then dentist today and was told that I require five fillings, four amalgam at the back of my mouth and a composite filling at the front, I already have alot of Amalgam fillings and I have heard storys of the mercury from them leaking into your blood stream, are they harmful?? would I be better looking to get white fillings?? and get the amalgam ones i already got replaced?. A. hi, many thanks for your enquiry, firstly yes mercury is contained in amalgam but there is no evidence to say amalgam fillings are harmful. secondly amalgam is a good material in certain circumstances but for large fillings at the back of the mouth is not one of them circumstances. my advice would be to seek out a cosmetic dentist in your area and have a consultation where he can advise of better long term options for your back teeth, ie porcelain inlays/onlay and white fillings and crowns if needed. hope this is helpful,

A. hi many thanks for your enquiry, yes it is possible to have the crowns removed and replaced with better looking crowns. the key is to design the new crowns first on models of your teeth and then copy this design into your mouth, once you like the new design and are happy with the size, shape colour we can then copy this design into the final crowns so hopefully ensure you are happy with the final result. it is possibly to use the six month smiles with the crowns however i would be first best to see what level of movement is required. i would recommend you seek out a cosmetic dentist to help improve your smile. many thanks
