what type of implant and precision attachments aid in denture retention

Q. i would be grateful if you could help me. i would like to know what type of implant and precision attachments aid in denture retention?

A.

Hi

There are many different options. Of course the options depend on -  is it for the upper or lower, amount of bone you have, type of bite you have, etc.

The possible options are
1. locators
2. magnets
3. studs
4. bars.

the following web page provides different videos which explain the different options in more detail - http://www.dental-suite.co.uk/dental_implants.htm
(just click on the video link on this page)

I personally tend to use locators or bars to provide denture retention.

Ive had yellowish teeth since i was young due to antibiotic medicines i took

Q. Hi, I’m having yellowish teeth since i was young due to antibiotic medicines i took. I tried whitening toothpaste & treatment but seems can;t work. I’ve read about veneer treatment. is it the only option i have? how about laser treatment? what is the pros & cons?

A. Tetracycline staining is all the way through the tooth.  Long term bleaching can substantially improve things.  Bleaching can be a combination of home bleaching and “laser” bleaching.  In my practice we only carry out home tray bleaching with a surgery visit of slightly stronger bleach placed in the tray - I have my doubts as to whether laser bleaching is really effective.

However veneers will provide you with a good end result immediately.  The down side for veneers is that a surface layer of tooth needs to be removed ( 0.5 to 0.7 mm especially if the staining is dark) . If the teeth are in good alignment there usually are minimal problems with this technique - years down the line you will need them replaced with more veneers.  If the teeth are in poor alignment there is a chance that root treatment may be required if the neves of the teeth are damaged.  In this instance I would advise some form of brace work prior to veneers to minimise the damage done to the teeth.

I have treated many people this way very successfully but the important thing is that you realise that once your teeth are veneered there is no going back.

My front four top teeth are in need of crowns

Q. Hello there, I’ve seen your contributions to dentist’s blog at : http://www.cosmeticdentistryguide.co.uk/blog/hi-i-was-looking-for-some-advice/ My front four top teeth are in need of crowns, I think. Currently they are heavily filled, discoloured and I hate them. Could you please advise how do I go about choosing a good dentist? Thank you in advance for your response.

A. The best thing to do is to look for cosmetic and restorative dentists on the internet and then go and have consultations with them. You need to ask about their training and experience. They should hae training in Ocllusion which is a study of how the teeth, jaws and muscles function together. Definately look at their portfolio. If they don’t have one then don’t go to that dentist. You will be able to judge yourself by the end results of treatment that they show you

my Dentist told me that I am loosing one of my front tooth

Q. I am 42 yrs old and I recently went to have get cleaning done and my Dentist told me that I am loosing one of my front tooth, he said it cannot be saved it will eventually just fall out. I also have a little abrasion on the inside which feels very funny but is does not hurt.I keep putting my tongue on it. I am scared as I need to know what can be done at this point in time.

A. Well, unfortunately situations where teeth become mobile do occur.  Eventually they are lost and a replacement option is required.

The three options for replacing a missing a tooth are 1. a denture, 2. a bridge 3. a dental implant.
The option for replacement depends on the cost and the surrounding teeth.

You should speak to your dentist and go through the options so that you have a plan in place for when the tooth is lost.

I am 26 and my mouth is a mess

Q. I am 26 and my mouth is a mess. I was a meth addict for 10 years and it destroyed my teeth. Over half of my teeth are broken and down to the gum line exposing the root. There’s not one tooth in my mouth intact. I am in so much pain everyday and have been for 2-3 years. I am so ashamed i don’t have any confidence and am constantly reminding myself when around people even my husband to turn my head away or cover my mouth so they can’t see my teeth. After the birth of my 4th child i was diagnosed with CHF i have to take 5 different meds daily for life. My dr said me being so young the meds could strengthen my heart but unless i get these teeth out of my mouth i can’t get the full benefit of my meds. I also believe once a dentist sees my mouth he won’t want to bother because it’s going to be so much work to remove them i need to get this done ASAP my husbands insurance doesn’t kick in for 30 days Do u think an oral surgeon will go ahead and start on my mouth and accept payments or do i need to wait until insurance kicks in

A. A general dentist should be able to manage the dental aspects of your treatment. You may be exempt from dental charges try NHS direct for links. At this stage it might be better to clear out the hopeless teeth do some simple fillings on the NHS. IF you find out that you are not exempt the fees would be £198 at an NHS dentist. As you are in pain I recommend seeing a dentist now and find out what treatment you need / estimated costs ( if any)

my problem is overcrowed bottom teeth

Q. Hi, my problem is overcrowed bottom teeth, slight overbite from top teeth(sucking thumb child), uneven bite (left side teeth just about touch compared to right), narrow(ish) bite, 3 venners and crown (4 front top teeth due to broken teeth). The thing is I’m 30 in a sales role, any chance Invisalign will work for me? Its as much for comestic reasons (fear it will look worse as i age) as it is for hygiene as my teeth never feel really clean even after flossing.

A. i think you may need a number of different treatments.  Invisalign could straighten and re align your teeth and improve your bite, after this you may need some cosmetic to replace old restorations and create a new smile.

I have triangular gaps between my teeth owing to gum recession

Q. I have triangular gaps between my teeth owing to gum recession. My gums are stable and I have no loose teeth. Can I have composite bonding to fill the gaps and if so, could you recommend a highly skilled dentist who I could go to for this procedure? Many thanks

A. Hi

It is possible, but you would require an examination to see if could be done.

To recommend you a dentist, i would need to know the area you are looking for one.

I am 55 years of age and have just had a bridge removed

Q. I am 55 years of age and have just had a bridge removed and have been fitted with a plate with three false dentures at the front and four at the side. I am intererested in implants, and am wondering at the cost of implants and whether it is feasable for me?

A. Implants can be placed in many situations.  The key is to the amount of jaw bone available to place them in .  If insufficient grafting techniques can be used to increase the volume of bone available.  You would need a proper assessment before any costscan be given to you.

I had a route canal but did not get the crown yet

Q. I had a route canal but did not get the crown yet. The filling just came out. What can I do until I get into the dentist? The office is closed until Jan 5, 09.

A. You can go to a pharmacy and get some temporary dental materials to cover the tooth until you can see your dentist.  Emergency dentists are available.

i have recently had crowns caps not sure which over my teeth

Q. i have recently had crowns/caps (not sure which) over my teeth. i still have little “stubs” underneath the crowns but not much as they were filed down. unfortunately my new front teeth stick out slightly and was wondering if braces were an option and how much would they cost on the nhs? thanks

A. Their are parameters that decide whether a patient has enough of a problem to warrent braces on the NHS.  You would need to be assessed by an NHS dentist.

my mothers dentures are eroding her lower left molar region

Q. Dear Dr moore, My mother is edentulous and has a full set of dentures. She is 66 and for last two months she is the dentures are eroding her lower rLeft molar region. The dentures are about 5 years old. We got the dentures fixed by a dentist. But that didnot help. My queries are ; does she need restorative surgery, how frequently should the dentures be replaced and what type of dentures would be the best for her. Best Regards

A. Hi
The dentures will probably need either replacing or relining after 5 years or so. If the gum i ulcerating around the molar region I would get it checked out by your mother’s dentist. The denture may need a temporary reline before the new dentures are made to allow the gum to heal.

I really really hate my teeth

Q. Hello there:) I’m a 16 year old girly, and I really really hate my teeth! Everyone tells me there’s nothing wrong with them and to be honest there isn’t any major problems! I just want them to look perfect! For me! Here’s an idea of how they look:- / < My top teeth slightly stick out \ < & this is the bottom row. I just want both rows pushed back, straight, but the thing is i really don’t want any normal braces! My hearts set on lingual. As my teeth stick out having normal braces would look flaming rediculous! Could i have lingual braces? All my teeth are together with no gaps apart from the front two, so would having teeth removed be an issue? Im honestly not fussed about the price! but i really wan’t lingual and no other, not even invisalign it’l just look silly. The un-noticable the better! As i said i’m 16 but do i have to pay the whole amount straight away, could i not pay a fee over a period of time as of my circumstances? Sorry if i blabber on, i just really wan’t my teeth straight and my private dentist said no when i was 15. So now i pay the price! Please do get in touch if you have time! I’d love to hear your verdict on my sad dilema! Much appreciated:) Ps: I have no idea if you even specialise in lingual braces, total guess!

A. Hi

It is possible you could have lingual orthodontics, but impossible to say without a clinical examination.

If your teeth are crowded then you may require some teeth to be removed to create space.
Many practices now offer financial palns to allow you to spread the cost from 1 to 5years.

I do not specialise in lingual orthodontics, therefore i would recommend searching for a good orthodontist who carries out this procedure routinely.

Best of luck!

My teeth have been bleached and are whiter than B1

Q. Hi My teeth have been bleached and are whiter than B1 - they match the first new ‘bleached shade’. I have two teeth that have been cosmetically bonded with B1 resin but these now look ‘yellow’ compared to the others. My dentist says he cannot find a supplier of the composite resin to match the bleached shades. I know cosmetic dentists can obtain bleached shade resin - can you tell me where my dentist can obtain this from. Many thanks

I have 4 teeth that are heavily filled

Q. Hi, I have a question. I have 4 teeth that are heavily filled also on 2 of them I dont have that much tooth left. My nhs dentist has told me that they cannot fit crowns on the 2 teeth because there is not that much tooth left. Now, I dont really want to go for Implants now, because they are basically surviving on a large filling. Can i request crowns or should my dentist advise me what I should do. I basically feel like she is leaving it too late and leaving it till i dont have much tooth left and then saying i cannot put crowns on there. I am only 28 and only eat on one side of my mouth, so this is the reason why most of my teeth are filled on one side.. My question is can I request crowns or should she advise me. ..Pls can you get back to me. Thanks

A. Your dentist probably has his / her reasons for not wanting to place crowns on these teeth I would recommend you talk to him/ her and discuss alternative options.  If you are still unhappy you can always go to get a second opinion.  If the teeth are not very strong they may not be suitable for crowns.

I have recently got 4 front veneers

Q. I have recently got 4 front veneers, and i have now found that my breath is a bit smelly…is this normal and if so how do i combat it (other than brushing and using mouthwash day and night? It is REALLY getting me down! Thanks

A. Bad breath is not caused by veneers.  Bad breath is caused primarily by sulphur producing bacteria.  The key is to remove these completely with brushing, and especially flossing.

I have 2 posted crown’s as my two front upper teeth

Q. Hello, I have 2 posted crown’s as my two front upper teeth. They have been crowned for 20 years now and i think its time i had them removed and implants to replace them. Would the removal and the titatium impant be done on the same day and also would the temporary teeth be fitted also. I would also want to have veneers fotted to the remaining upper teeth to compliment. It would seem you’re quite the expert when it comes to implants but as i live i yorkshire could you advise on my travelling after the proceedure? As in would i be ok to drive……ok to catch the train or would i need a overnight? Many thanks

A.

You are fit to drive after a reasonable r

est if you are relaxed about dentistry. An anxious patient probably should relax a little before driving long distances bringing someone with you who can drive is probably the best option.

You can fit implants on the same day that teeth are removed but it is a little less favourable ( in my opinion ) than waiting 6-8weeks. I personally wouldnt “load” the newly placed implants with crowns for 3-6 months. The temporary we iuse most during extraction healing and implant healing is an adhesive bridge.

I would be happy to do a free initial assesment/discussion on the ways to manage your case. If you wish to proceed to a full case written treatment plan with photos, study models and xrays we ask for £100 to contribute to our costs.

Hi there i am wanting my teeth sorting out

Q. Hi there, at the present moment im an nhs patient and im wanting my teeth sorting out i wont smile when people look or talk to me, when i chew i cant use my front teeth as they dont meet up with my lower ones, they go over the top, ive askd my dentist what i can do and she just said leave them alone as your gums are not 100% perfect but ive had terrible gums all my life and nobody has said anything about in the dentistry, she also said ive got too many fillings for anybody to even assess me, how can she judge a person by this, my old dentist said the reason why ive had 2 have these fillings which were my 1st ever at the age of 26 was because of my pregnancies the babies took all my calcium from me and put veneers on my upper left 2nd n 3rd and my upper right 2nd n 3rd teeth, im 39yrs old now and my husband goes mad at me when we have family get togethers as im always behind the camera, we didnt even have wedding pictures done beacuse i wont have my photos taken or anything, please can you give me some advice as to what i could do, i dont like locally to your surgery but if i have some advice then i may be able to do something as im getting to be a recluse because of this and cant even find a job… please can you email me back thanks

A. the first thing you have to do is get your gums treated. It sounds like you have gum disease and it is nothing to do with the calcium being removed from the bone during pregnancy. If you have always had gum problems then chances are you will have had the disease or at least gingivitis before you got pregnant. Hormones released during pregnancy cause the gums to further inflame and the bacteria associated with gum disease causes bone loss and mobility of teeth. It would be wrong for any dentist to carry out restorative treatment whilst your gum disease is left untreated. You will not gat adequate treatment from the NHS. you need to seek private dental treatment from a dentist that has a hygienist and address this issue. Only then will you be eligible for restorative work. Restorative work can be carried out but again this will have to be private. It is very unlikely you will be able to have this carried out on the NHS
