Q. After having braces about 5 years ago I noticed when they were took off slight gum recession on my front lower two teeth. This has got progressively worse, I am now 20 and my front two teeth have very little gum left and bleed when I touch them, they are slightly wobbly. I keep going back to my dentist who has told me different things every time, last time I went he told me there was still gingivitis in my gums but that my gums had grown back, when in actual fact my gums have got a lot worse. I am brushing twice daily with an electric toothbrush, brushing upwards on my gums, flossing, and I have gengigel which I apply before going to bed. I have recently started using cordosyl mouthwash. I realise I need to go back to the dentist but I am not confident at all in his abilities anymore. What is the best course of action for my gums and is it possible to save my teeth as they are only very slightly wobbly. I have heard of gum grafts, could this be an option, and also is there anything else you think I should do to help my gums and teeth? I realise I will have to pay for gum grafts, can I get this on the Nhs or will I have to go private and could you tell me an estimation of the cost. I am 20 and in full time education (college) but receive no benefits. Thanks in advance,
A.
You need further assement and some Xrays to diagnose why this has happened. The correct treatment to first stabilise and then (if possible) repair the damage can be planned.
You can ask your dentist for an NHS referral to a dental university teaching hospital.
The StJohns Dental practice in the heart of Birmingham City Centre offers a free consulation that will manage the basic questions, two Xrays cost around £14. Then you can get an idea of appropriate treatment and cost.

Q. Good day Sirs, On Wednesday evening I felt a mild toothache. In the morning on the next day my right chick started swelling a bit. I made an appointment to see a dentist on Friday. The doctor treated the tooth canals, said that there’s nothing bad about the tooth, no pus or any purulent substance just a slight inflammation. She left the tooth open and prescribed me to wash it with salt, soda and iodine water thoroughly throughout the day. The next appointment is on Monday. The pain is gone, but the chick is not getting smaller nor swelling any longer. I’m worried that the tooth was left open, could you please tell me if it’s ok? Thanks.
A. I would prefer not to leave teeth on open drainage for more than 24hrs. ( in line with the protocols adopted by the btish & european endondontic societies )

Q. My gums started bleeding recently when i brushed my teeth, so i started using a mouthwash specially for gingivitus and thankfully it stopped. However when i looked closly at my bottom teeth in the mirror earlier i noticed they have black lines randomly across the inside. They look like lots of little thunderbugs. Theyre only on my molar and bicuspid teeth (think thats what theyre called) There arent any marks on the front or back of the teeth, just the top. What is this??
A. I guess this is just stain in the pits and fissures of the back teeth, if you used corsodyl mouthwash, this can cause tooth staining. I would check things with your dentist and see the Hygienist to give them a good clean

Q. I am 23 years old and at my wits end with my teeth. My front four top and bottom are the only ones which are okay. Every other tooth is filled beyond belief, infected or causing me problems. I have developed a SEVERE phobia of dentists now and have to be sedated to have any treatment. It took me 2 and a half years to build up the courage to have a decayed molar that was ground down to the root removed in March. the gum became infected thereafter and I had dry socket…. now, June 2009 and the surrounding teeth are causing me problems. I am now on the waiting list to be sedated to get these teeth sorted. My question is: Is there ANY dentist… is it even heard of that a dentist will remove MOST (not all) of my teeth… under sedation of course? I am honestly at my wits end with them now I cannot cope with anymore dental problems. I would give anything to have all of my back teeth and side teeth removed leaving me with only my front four on the top and bottom. I would happily live toothless for the rest of my life if it meant no more toothache. Thanks for your help.
A. I couldnt promise for another dentist, i would hope that after following the first principle of not doing you any harm someone give you honest unbiased advice on your choices. Some of those choices should be suitable for a severe phobic. Try some of the dental phobic websites for ideas.

Q. Hi I am from Dublin but living in turkey at the minute. I have genetically crooked teeth at the front of my mouth with fangs and canines. My mouth is very small so hence why the front are all together. One of my fangs is now black after lots of fillings on it i would like to get venners. The denist told me that to change i would have to change the front 7 teeth and she will use something called ymoxin i think thats what she said she said got no metal properties in it. she will decide what she can do. Can to tell me in your opion what is best.
A. I am afraid it really is difficult to advise you without seeing you, I do not know what ymoxin? is. I think your better option is to find someone who can straighten your teeth first using braces such as Damon Braces as they will reduce the crowding generally without the need to remove teeth and then see what can be done cosmetically. Please feel free to contact me again if I can be of further help.

Q. I am in need of crowns fitting to my upper front four teeth and my dentist has quoted me 1400 pounds for the job, which as a student is a terrifying prospect. He said it will have to be done privatley but didn’t explain why. Is my dentist just trying to make money out of me or is there a limit to how much treatment you are allowed through the NHS?
A. There is a limit on how much you are allowed to do, however he may be suggesting the front crowns be made privately so you can get a better result for such important teeth

Q. Hi, In the event that root canal treatment was required on one or more teeth if teeth were too irritated following veneer preps, and the pain became apparent AFTER the veneers were placed, would the veneers have to be taken off and changed for crowns? And would any further treatment be at the expense of the patient? Many thanks
A. Hi The veneers are usually placed on the front of the teeth so the root canal could be accessed through the back without disturbing the new veneer. The cost will depend on the arrangement you have with your dentist.

Q. How big does the space need to be for an implant? Is any space to small?
A. Yes you need at least a 5-6 mm space between the teeth and 4mm of bone width. the width can be increased but the space between the adjacent teeth can only be altered with trimming or orthodontics

Q. I have a 24 year old bridge that snapped and dangled in my mouth at the Isle of wight Festival. A good emergency Polish dentist after 3 injection took the bridge out on the island. Before going to the Festival I had taken anti biotics for pain over one post.My dentist xrayed it and said it was root caneled and fine except for being a little loose having been infected. In fact the tooth was rotten and had broken off completely. This caused the other post to break whilst eating, causing it to dangle in my mouth. My Dentist who i feel is good at is job is away on two weeks holiday and cannot see me under national health for a total of 3 weeks. I am thinking of having a overdenture which i believe is similar to a bridge. One post i know can be root filled and used, the other may need to come out or be used as a base for a post. I am loosing my job in four weeks so i need to stick with N.H. The Bridge is were my two main front teeth used to be. What ways can a denture be fitted do they need clips. Is an overdenture as I described. I would be greatful for advise. I have asked this question because I believe this advise is for free. Thank you.
A. Hi
A denture can have have clasps to aid retention. I would recommend you have an examintaion to give you all the available options. Unfortunately it is very difficult to say what is the best option without looking at the mouth.

Q. I have a question about veneers, can I wear them? I have front teeth overbite and I do not want to wear metal braces again! I wore braces back in 2001 thru 2003 and now I have a ugly overbite. My teeth has changed my entire appearance. How can I get that million dollar smile? I’m also 47 years old and an artist, I can’t be walking around like this. What do you suggest, Doc…
A. Hi there,
thanks for your enquiry. veneers is not anoption for everybody. It depends on how your bite is and if you grind/clench etc. for some people, then full coverage crowns may be the answer- as long as you undrestands that by having tis treatment, in the long run, you may be comromising the health of your teeth. with orthodontics, your teeth’s health is not compromised. there are several forms of orthodontic treatment available- especially discreet versions that can hardly be seen. I would strongly advise that you see a cosmetic dentist for a consultation to discuss all of your possible options before making a final decision.
Hope this helps.
alternatively, I would be more than happy to see you for a free consultation at my kissdental practice in Manchester.

Q. Hi, I have recently had a crown and a veneer on my two front teeth, the crown came loose after a week and so when i went back my dentist put a harder cement in my gum, hoever when she has put the crown back in it has not been positioned correct and is no further forward than my other teeth. I have been back to the dentist and she has told me if they take the crown out and put it back in there is the chance the root could crack or the crown could crack. Do you know what i can do as i am really unhappy with the out come? Thanks
A. If you’re really unhappy with the way iy looks it might be bets to remove the crown and start again unless a minor adjustment would suffice

Q. Hi, I have been recieving dental treatment fo gum disease for almost 2 years including deep pocket cleaning etc.I have since developed a buckled front tooth which has started to move resulting in a small gap (through bone movement)i would like to have the tooth straightened or repaced. Is this an option? i am about to be discharged with regular dental and hygiene visits as an aftercare opion. Thank You
A. It really depends on how much bone is left under the gum and how healthy the tooth is . You could straighten your teeth with orthodontics provided low forces are used or have the tooth removed and have an implant placed if the tooth is not healthy. You need to find a cosmetic dentist that understands orthodontics and implants so you can have all the options.

Q. Hi I am having a bridge fitted in 3 months but the tooth behind is VERY heavily filled, will this be a problem? Cheers
A. hi
if the tooth behind is nothing to do with the bridge then it will not influence it , however if it is part of the bridge then the weakness will be improved with the crown as part of the bridge or you may need a post to strengthen it , best to talk to your dentist

Q. Hi,i dont know if you can help but, a few months back i had a removable plate brace fitted and it has made my two front teeth overlap! Is this normal?? Thanks
A. There might be too much pressure on the denture pushing your natural teeth. Go and see your dentist and see if can be adjusted. Teeth can also move if the bone holding them is weak.

Q. Dr Dr Moore I am thinking about getting invisible braces. I had braces when i was younger ( I am now 20) and i have recently noticed that one of my top wisdom teeth is coming out. Its not causing me any problems however I have noticed one of my teeth on my top front row has slightly moved on the side the wisdom tooth is growing through. Could this be related? I would like to straighten the tooth, it is possible to have the invisible brace without all the wisdom teeth fully grown out? Thank you for your time.
A. Late onset crowding is not caused by the wisdom teeth but occurs at the same time so it is often blamed for this. Invisable braces work well if you start early, you might also consider an inman aligner as it works quickly and is usually cheaper.

Q. Hi My partner lost a tooth (front tooth) as he used to be a kickboxer and the nerve in the tooth got damaged. He has had a plate with a false tooth on it but this didn’t suit him. He recently had a bridge put in but has had it redone within two months of having it done and less than two months later it has come out again. He takes good care of it. When it has come out it looks like it has been bonded with the same thing that is used for fillings. Should something else be used? It seems odd that it has come out so quickly. He had gained confidence and was smiling again with his tooth but now after the second time of it coming out he feels pretty down. Any help or advice would be appreciated - I can send a picture of the false tooth and it’s fixings if you need it. Thanks
A. It sounds like he has had a marlyland bridge made, the false tooth is attached to metal which wraps round the adjacent tooth. If the tooth keeps coming unstuck i wonder if his bite is off and he is putting too much pressure on the bridge. Ask your dentist to have a look at his bite. The best option would be a dental implant.

Q. I had a porcelain crown fitted in October 08. The crown fractured at the back in April. The dentist who fitted the crown said it was not too bad and ground it smooth. Since then the crown has fractured again. It now feels waffer thin and will snap at any time. What I need to know is, am I entitled to this crown being replaced free of charge. It is a lot of money to pay and 6 month is not a long time considering they should last 10 years.
A. There is no standard guarantee but most dentists would replace without further charge in this situation (As long as no external trauma was resposible)
