My teeth are thinning and causing some pain, why is this?

Q. My front bottom two middle teeth are getting thinned and those two tooth are give me little pain. The pain is bearable but I am feeling little uneasy. When I look in mirror closely I can see a minute gap formation between those two tooth. Can you please tell me what is the reason for this. It will be great if you can suggest me any solution for this?,
