Q. Hi, I have a question about treatment for a problem with my lower teeth - the four between the incisors on the lower jaw. They’ve always been crooked, all of them are twisted in the same direction. I always brush twice a day at least, however, I still have had problems. Whilst brushing I felt a piece of what felt like tooth come out. I checked and I noticed a yellow buildup all along the inside of them 4 teeth. And the tooth which part of came away from now feels broken from the bottom of it and sharp. It feels as if the lower quarter of the tooth has just come off. Also, the yellow deposits will not brush off at all. I have no idea why I have these as I always brush a minimum of twice a day. perhaps it’s because the crookedness stops the brush from reaching there. My question was two-fold:

1) What can be done for the tooth which has been chipped away?

2) What is the yellow buildup on the teeth?

3) As a long term solution, can braces be used to straighten the teeth and to ensure that there aren’t any nooks for nasty stuff to build up in?

4) What would be the cost of such treatment?

5) How much of a tooth has to chip off before there’s a risk of it falling out?..

Thanks very much for your time.

A. 1) The tooth that has chipped away can easily be fixed with white filling or a porcelain veneer.
2) The yellow build up on your teeth is tartar. It is hardened plaque and is very difficult to remove by brushing alone. All adults get a build of tartar even though they brush their teeth. For you it is harder because your teeth are crowded. You need to see a hygienist to remove the tartar. All of my patients see the hygienist every 3-6 months to make sure tartar build up is removed and the gums don’t become inflammed. It is a very simple treatment and can take from 20 mins to 1 hour.
3) braces are a very good, non destructive treatment to correct crowding. However after treatment you will still get tartar build on the backs of the lower front teeth and I would recommend that you see a hygienist at least every six months.
4) Your lower tooth won’t fall out because it has chipped. It can however break off at the gum margin if a lot of force is put on it but by the sound of things your chip seems like it can be fixed easily.
I hope I have been of help
Kind regards
Dr Komal Suri

Q. I have porcelain crowns on my fron 4 teeth. Can I get Lumineers on top of them to help with a gap that has begun?

A. Lumineers are made from porcelain as are normal veneers. We can sometimes fuse veneers to existing crowns. I have done it myself over existing bridge work. It will depend on the integrity of the crowns that you have and the reason of your gum recession. It is best to go and see a cosmetic dentist to find all your options. Sometimes it is best to have the crowns replaced but it is very difficult to tell without seeing your teeth.

Q. My daughter has been assessed at a 2D rating for needing a brace. The orthodontist said she does not qualify for NHS treatment and would need to pay in excess of £2000 for treatment. Could you please explain the rating to me as I don’t understand how she does not qualify, yet needs this treatment

A. The fact that the NHS has put restrictions on children requiring braces is very distressing. This unfortunately has been the case since April last year. There is a banding system from 1-5 and only crowding severe enough to come into categories 4 and 5 can be treated on the NHS. The orthodontist has his hands tied. I know that orthodontic treatment is very expensive. It can range from £2000 to £6000.
Unfortunately if your daughter requires braces it will need to be paid for privately.I sorry to bring you bad news. This is the decision of the government and there isn’t anything we can do to change it.
Good Luck with everything

A. There is always something that can be done.
Your treatment options are to have braces or to have restorative work in the form of veneers. You need to have an opinion from a cosmetic/ restorative dentists. We have special training in these circumstances and can let you know what the possibilities are. All of the dentists on this website are very capable of doing this kind of work. If you are in Buckinghamshire or don’t mind travelling, I would be happy to see you for a chat. Failing that you could seek a consultation with one of the dentists on the site. I have attached photos of a patient that had very crowded teeth and had veneers to fix it.

Q. I would like to ask, what is the difference between veneers and laminates.
A. Laminates and veneers are exactly the same thing. They are usually between 0.5 and 1mm thick. They are much less destructive than crowns and can be used to solve many problems.
I hope this answers your questions

Q. Dear Dr Komal Suri I am a recovering bulimic, with the obvious damage to my teeth couse by years of purging and vomiting. My teeth have moderated erosion which includes a slight chipped front tooth. Is there any treatment you can offer to correct/restore the damage? I am desperate for a pointer in the right direction many regards

A. I have treated a few people that have suffered from Bulemia and I know it is a very hard thing to overcome. Usually with Bulemia all of the erosion affects the back surfaces of the upper front teeth and then this weakens the enamel and it begins to chip the edges of the teeth. I do not know the extent of the wear or erosion on your teeth but usually treatment requires whole coverage of the teeth. Crowns are too destructive so the treatment I have done on my patients is wrap around veneers. They are very thin and it is the least destructive way of restoring your teeth. My best advice would be find a restorative/ cosmetic dentist for the treatment.

If you are in the Buckinghamshire area or are willing to travel I would be happy to see you for a complimentary consultation. I have attached pictures of a patient I recently treated that suffered from Bulemia for 12 yeras. Your teeth probably aren’t ask eroded as hers but it lets you see the possibilities. Unfortunately I can’t show you the full face pictures due to patient confidentiality.

I hope this helps

Q. Hiya, I have just noticed a small chip in the front of my left tooth. It is only really small but it is a little sensitive. I have heard about Bonding and would really appreciate it if you could tell me the positives and negatives to bonding- also will my dentist do this for me? or will i have to go to a cosmetic dentistry?- i would really appreciate your help. Hope to hear back from you soon

A. Bonding refers to many dental procedures that involve tooth coloured resins as fillings or glues for ceramic restorations ( veneers, crowns, inlays ) Most dentists do some bonding. Cosmetic dentists will generally use higher grade materials with more attention to the final appearance.. A small chip will often be easily and aesthetically repaired by a tooth coloured filling without drilling your own tooth.

A filling is not very expensive, we would be happy to help if you should wish.

Q. I am 26 years old and terrified of the dentists but always pluck up the courage to make an appointment every 6 months. My teeth are in a very clean and healthy condition but my problem is my front bottom six teeth are really cramped and over lapping giving me slurred speach. I also have an uneven bite. My top teeth are quite straight apart from one tooth which is twisted to the side. What sort of treatment would i need and how much could it cost. I had a brace when i was younger but hated it so much i removed it after about 2 months (the most regretable decision of my life). Cosmetic dentistrty is the only option for me. Thanks

A. I was not sure whether you were discounting a brace / orthodontic solution as a solution now. There are clear shell systems, Internet Search on Clearstep Orthodontics or Invisalign Orthodontics. Also tooth coloured fixed braces which are less visible & can’t be removed by you in a moments despiritedness might be an option. 6-12 months active treatment then long term retention. Guide price £3000-5000. both upper & lower teeth.

Cosmetically you could veneer the front 6-8 teeth upper and lower. You would need a thorough exam before formulating such a treatment. Guide price £5000 both upper and lower .

The slurred speech needs to be looked at to confirm it is connected and whether it will be improved by one of these types of treatment.

If you would like a complimentary clinical opinion you may book a free consultation if you inform the receptionist of my offer 0121 643 0610

Q. I lost my tooth in the front and had a temporary crown which was done very bad, would it be possible to have a veneer put on top of the temporary crown

A. If I understand this correctly there is a permanent solution already planned.
The short answer is poor quality temporaries are best remade from scratch.
If you are waiting a couple of weeks for the final replacement the best option is to return to your dentist to remake the temporary crown.
If the temporary is to last a few months during a healing phase , you could consider a second opinion.

Please let me know if this is helpful.

A. if all the remaining teeth are decayed and causing you pain, then the best thing would be to have the teeth extracted and a denture placed. There are many better options than dentures today, to explore these options it would be best to see a cosmetic dentist. If i can be of any further help please do not hesitate to contact me.
