Dentist said he cannot build up implants enough to provide enough lip support. I have not been back since. What should I do?

Q. Hi I was due to have implants approx 18 month ago. Full implant held denture for the lower and individual implants onthe top. The top partial denture I have now is built up at the front for lip support but after having all my impressions the dentist said that he cannot build up the implants enough to give enough lip support. This meant a full denture for the top which I must say I panicked about and haven’t been back since. I now need to look at my options again and may need to find another dentist. I was wondering how much the implants can be safely built up in this situation? Thanks,
A. There is no hard science but simple mechanics would suggest something like a 13mm long implant of reasonable diameter can support a 13mm restorative hieght above. Assuming lots eg normal bite normal direction of load down long axis etc. A 3D jaw scan CBCT will help with planning in cases were bone is limited. It is also sometimes worth discussing dentures if lots of gum volume is missing. Just crowns/ bridges may look too long.
