Q. Hello DR Butt, I used to wear braces a long time ago. After my braces were taken off i was given retainers to wear for a few months. But now i feel as if my frount teeth are sticking out to much. Iv still got my retainers will they help push my frount teeth back if i start wearing them again? Or what else can i do? Hope you can help me because they really do annoy me! Regards

A. hi if your retainers still fit they may push them back.if they do not your teeth have moved as you probably did not wear the retainers for long enough or properly and you will probably have to go through some orthodontics again

Q. Dr im 20yrs old n im under treatment from an orthodontist in pakistan since 1yr,i hav upper mildine shift about 1.5mm but it is almost corrected n the dentist says my treatment is almost complete,my upper maxillary right canine was impacted n it was extracted by an inexperienced dentist when i was a child-n that was the cause of the midline shift and lead to class 2 malocclusion on one side and class 3 maloclussion on the other,my problem is that although my midline is correct there is still a space where the canine should hav been i feel that my teeth r gona shift back once the treatment is off,i am being treated with cross elastics in which the 1st runs from my 26 to 33, the 2nd from 23 to 43 and the 3rd from 13 to 46,plz suggest what should be done in this case and whether this treatment would become useless after sometime?,and whether my dentist is doing the right job?as im in 3rd yr of dentistry myself n im very worried about my teeth.thanku

A.

why dont you consider a single wingcantilever maryland bridge to replace the canine and maintain the space or a dental implant once the space has been created by completion of the orthodontics.
you can know how much space is needed by measuring the canine width on the other side and asking the orthodontist to leave appropriate space

Q. Hello Dr Butt, My question is about the Inman Aligner, I have had mine in since the end of August, the first three weeks were great I could really feel and see the difference in my front teeth. The last four weeks or so I have said to my dentist that I can’t see or feel any movement which is the main part of the treatment. My dentist is now telling me that I should get a Clearstep like brace to finish it off for a further cost. I thought the Inman was all that was needed to complete the treatment – what can I do??

A. hi
the inman is usually a 12 week treatment depending on how long you wear it each day
it aligns the teeth pretty well but for fine detail finishing clear aligners can be used.
it depends if you are happy to finish where you are or want to continue and get a nicer result.
it is normal for the inman not to feel as tight as the teeth move into the right position so dont worry about this.

Q. I had braces two years ago, I am 17 years old now and I currently have a permanent fitting at the back of my two front teeth they were placed their in order to prevent my two front teeth from moving forward. However the opposite thing happened now I am starting to see them move forward and they look really ugly. So I was wondering how much would it cost to get my front teeth fixed?…..and what else could be done to them in order to improve their appearance?

A. hi
it would mean that your retainer has not done the job it was intended to
the amount they have moved would determine costs for treatment – if minor expect a cost -1500
if major could be approx 2700
after they are straight we would supply a retainer that would cover the teeth at night that prevents movement in either direction which you would have to wear 2-3 nights a week

Q. i have had a root filling on one of my front teeth years ago which has discoloured it thus making me a little uncomfortable when i have to smile!! unfortunate for me i work as an air hostess so i have to be all smiley smiley all the time!! i am considering having veneers but want to know alot more about them, ie cost for a full set? what the ups and downs are? do my teeth get damaged? how long do they last? i smoke, so would that effect them? and i am a little scared to just go to anybody as i dont want to be one of those cases where things go wrong and im left with infection or no teeth!? sorry!!

A. hi
if the discolouration of the tooth is really bad then a crown may be more suitable.
for that perfect smile your other teeth may be suitable for veneers which is a thin porcelain covering on the outer surface of the tooth , this usually means that the tooth needs to be drilled to avoid bulkiness when the veneer is placed . the downside is that you are comitting to a non reversible procedure . as time goes on and if your gums receed you will begin to see the join to the tooth near the gum -which is more of a problem if you have a gummy smile otherwise you will be ok.
smoking increases the efects of gum disease and also helps initiate gum disease so can contribute to the long term look of your veneers
costs per tooth vary between 475 to 700 per tooth depending on level of cosmetics
if just the position of your teeth apart from the dark one bothers you but not size and shape consider orthodontics like invisalign

Q. Hello I have a problem where my teeth from the k9s inwards are too far forward they are straight as the roots themselves are too advanced. I suspect I should have had braces as a child. The result is when my mouth is closed I look as if im wearing a gum sheild. I saw an orthodentist about 10 yrs ago who said he couldnt really make much difference. I wondered if techniques have moved on since and if you might be able to help?

A. hi
im afraid that without considering orthognathic surgery there may not be any other treatment that will help.
a referral to ahospital orthodontic dept will help with discussio on this as a option
